Great video. I have been using retinoids for about 25 years. I started using ROC when it came out many years ago. I am currently using prescription tretinoin 0.05 nightly. I use the buffering method. I put on my moisturizer on slightly damp skin and then wait 30 minutes to use my retinoid. Magic! 😀

At age 52 I am new to the retinoid game (and kicking myself for not starting much sooner!) I appreciate your thorough explanations about different products, and between your info and info from the gents @Doctorly I managed to not run away screaming from retinoids after my first experience with a pretty scary bout of retinoid-induced irritation. I also love your videos from Hawaii--I spent 4 years in Hawaii when I was in the Navy, and it will always have a special place in my heart! Keep up the good work!

Hey there, I’ve been using retinoids for years. This completely varies from person to person. Personally, I used Tretinoin 0.025% for 3 weeks (3-4 times per week). Then I went to 0.05% (3 times per week). I’ve had some awesome results with Tretinoin ☺️
I see this question all the time and discuss it in my latest video. It’s recommended to use it up to the orbital bone. Tretinoin can migrate 2cm under the skin. It knows where to go ☺️🦋

Thanks for this myth busting video. I am 65 and pretty new to using retinol products. I was torn between Olay and Neutrogena, but I watched the comparison video. I am leaning more towards Olay now. I am glad you mentioned "go low n slow." Every 3 days for 2-3 weeks, then increasing to every other day for a couple of weeks, before applying daily is a good plan for one with sensitive skin. I am also enjoying the comments. Like reversing the order of the moisturizer abs retinol cream to help lessen irritation.
Terri here. Oil of Olay...the original beauty fluid. No matter what my skin care of the year might be (not so fickle anymore, tho), I wind up with fresh, current Oil of Olay moisturizer. So many of my friends’ mothers used nothing but Olay...beautiful skin till the day they passed. A nice tradition and ode to those who paved the way...
I use tret 0.05% primarily but I also tend to use a lot of retinol to review in the last year. I will probably alternate between tret and a retinoid on a daily basis. I also use AlphaRet a few times a week as well which I am told has tret 0.1% in it. I tend to tell ppl to not go stronger than 0.05 if you’re doing well with it as the stronger you go, the more irritation you will get with not much more benefits. Hope that helps. I really don't have any skin issues so my Derm Dr friend (I'm not her patient) a long time ago gave me .05 J&J Tretinoin Cream and still does because I'm cheap and won't pay for it lol. Never had a problem with it and started using every night and I still do. Thank you for a great video and I liked and subscribed so see you next time!

I’m new to Tretinoin and 0.05% is the only available percentage I can find (I work and reside in a country where option is limited). I’m not sure if I use it correctly but I experience very little peeling to no irritation considering this is my first time using Tretinoin. I do make sure I moisturise my skin adequately, products like Cerave cream in tub, using just a pea size of Tretinoin for my whole face, sunscreen with no fragrance etc.. in between my every 3 days break using Tretinoin, I also use Retinol serum. Since starting Tretinoin I see result in less than a month and though I’m only using it on my face, somehow the severe acne on my back has improved as well. I’m not sure what’s going on.. though I’m pleased with the result, I hope what I experience so far is normal (if that makes sense)?

I haven’t heard that. I’ve used Tretinoin for years and always wash my face morning and night. You just want to use a gentle cleanser. I use the Cerave Hydrating Cleanser. ☺️🦋

Thank you Dr. Sugai, I'm new to skin care and I've learned a lot from your channel. However, there is one question which I couldn't yet find an answer.. and that's can we use hyaluronic acid together with retinoids? For example, I learned from another dermatologist who said don't use cleansers with HA as it can cause the retinoids to penetrate deeper into the skin hence causing irritation. Yet someone else would say HA is the exception and recommends that the Cerave Cream to Foam cleanser with HA is best paired with retinoids. Would be great to hear your take regarding this, thank you.
Love your channel. I am also from Hawaii and have hyperpigmentation on my face, chest and arms. Therefore, I have been using (for the past year) the Neutrogena Rapid Wrinkle Repair Retinol Pro+ .5% serum. I have not found a review of this product from you. Would you recommend this product for an experienced retinol user?
Hi Dr. Sugai, I have a question, please...I'm 45 and have been using retinol, 2 - 3 times a week for a couple of years now. Since starting, I have noticed NEW hyperpigmentation coming to the surface of my skin despite religiously wearing SPF 50 and avoiding any sun. Is it possible that my skin could be "purging" the hyperpigmentation that is under the skin's surface (like it would with someone with acne)? Thank you for all of your informative videos. Dr Sugai, I was wondering if I can use the retinoid/retinol serum on my eye lids? I started using retinol serum about 2 years ago, gone through the "Purge" and now using it every night. but sometimes my eye lids still gets red and feel sensitive, sometimes stings a little, but the rest of my face and neck are ok! even under the eye area are ok. can I put retinol serum all over my face? Hello Dr. Sugai! Been on tretinoin for a couple of months now but I want to do CO2 resurfacing laser or microneedling for my deep pitted scars. Do I have to stop my tretinoin use for a couple of days/weeks before undergoing laser/microneedling?
I use Tretinoin regularly and am soon getting C02. My Dermatologist has asked me to stop using Tretinoin/Retinol for a week before. Let me know what results you get from the C02! ☺️
Moisturising over the retinoid just spreads it around to delicate, sensitive areas which can cause dermatitis. Personal experience. Rather, apply moisturiser, then retinoid CAREULLY after
Moisturizing should be done after the retinoid has been applied and fully sunk into the skin (I wait 5-10 minutes). And sensitive areas can be safeguarded with a barrier cream or Aquaphor/Vaseline.
Hi Dr. I my main issues at 62 is blackheads and uppet lip and crows feet wrinkles. I was using the ordinary Grenactive retinol serum. But didn't seem to do much especially for blackheads. I just ordered the 1% differin adapalene after watching your videos.Do you think this will address those issues before going to a more aggressive tactic like Trentinoin? Hi Dr. Sugai! Can you talk about if topical retinoids are safe for people with Meibomian Gland Dysfunction? I have permenant dry eye disease after two rounds of Accutane, and it's been alleged that because vitamin A is so toxic to our meibomian glands, topical retinoids can in fact still migrate throughout the skin and in turn affect our meibomian glands even if we apply them away from our eyes. Is this true? I'd love to know as, I have acne still and Adapalene 0.1 really seemed to work well for me but I'm scared to use it as I don't want to cause anymore damage to my meibomian glands! :(
Still waiting for results from my olay 24 retinol. Weird that I had no irritation since I'm sensitive skinned. 😕 the retinol percentage must be low in that product.
I use the Cerave Hydrating Cleanser and love it! I recently just posted a video on my channel of my morning skincare routine. At that time, I was using a Paula’s Choice Cleanser but I much prefer the Cerave one now ☺️🦋

Another awesome video! How would you recommend incorporating a vitamin C serum if a patient doesn’t usually wash their face in the morning? My skin is pretty dry so I usually only wash once a day in the evening (then apply tretinoin) but I don’t know if I can put it on without washing my face first in the morning. Thank you!
Right, but the first layer is the buffer. I've just heard from other dermatologists on here that applying moisturiser on top of Retinol enhances absorption and irritation because you are forming a barrier on the skin and pushing the retinol deeper/preventing the skin from breathing. You also run the risk of spreading the retinol into sensitive areas like your eyes
@@fruitpunch5260 Everyone has a different reaction to retinoids. Some folks find that second layer of moisturizer helpful. Some folks don't use the first layer of moisturizer either and some don't use moisturizer at all.
