@@arthurdesrosiersmd US. pilonidal cystectomy that hasn't healed for over 4 years. The pico was the only true measure that helped. We have since found though that some of the failed healing may be caused by a possible diagnosis of Lupus 😞

Jay Hartbarger great question-short answer is No. with this particular device the white part of the sponge is safe over normal skin. The suction pressure is very low -only 70 mm hg at the single lumen auction site- and is dispersed over the large surface area of the device. Also, the type of sponge in this device is specifically designed to be safe over normal skin. Note that this device is different than some of the other devices you may have seen on my RU-vid channel.

tiff2106 Excellent question. Yes this pico PICO is a temporary portable wound VAC that is only intended for wounds that have a very low exudate or very low fluid coming out of them. It is not meant for an acute postoperative wound that has a significant amount of fluid

tiff2106 And just to clarify the two devices are made by two different companies so the device that you’re seeing in this video with the white sponge is made by a company called Smith and nephew that is the PICO. The other device which is made by KCI, this is a Nother company they make the traditional wound VAC with the black sponge, also with the silver sponge, and also with the newer purple sponge those three are made by a company called KCI and then invented the wound vac. Thx purple sponsor is their version of the portable wound vac

Anna Covino yes. The stickers actually protect the skin. That’s the whole point. You don’t want suction on normal skin

The PICO has the advantage of having a small battery pack so it is ideal for portability. However, the KCI wound VAC called the PREVENA has the advantage of also being portable, but being able to handle wounds with some exudate. There are different versions of the PREVENA, including the PREVENA Plus, that has a stronger suction. Both of these KCI wound VAC versions of the PREVENA have a smaller batter pack, much smaller than the KCI ActiVAC. Good luck!

Good question. It’s patient comfort. The cord can be run inside pants for example and then the battery pack placed in a pocket . If the cord runs towards the foot….harder if ambulatoryS Ofc if patient is in wheelchair then doesn’t matter.

Ryan Murphy if the pico is several days old it may be that the pump has burned out and you may need another device. If this is a brand new pico and these are batteries from the box, then the batteries may be defective, they may be inserted backwards, or there is defect with the device. Does anyone know why it does not stop vibrating?

Klaudia Chávez it is equilibrating and adjusting to find optimal suction...the vibration means the pump is working

Divya Gijo I’m not sure of the cost, to be honest, insurance companies will approve it though because this is a portable device the patient leaves the hospital immediately postop and this it saves the hosptial thousands of dollars that they otherwise would have spent if the patient spent 7 days in the hospital with the standard wound VAC

Good questions. Here are the answers: 1. PICO can be used as closed incision NPWT. The other option is the KCI VAC Prevena. 2. ciNPWT can be applied in the OR. It can then be changed as an outpatient in the office. 3. The length of duration of the NPWT should be based on exudate, type of wound, etc etc. I would recommend starting with 48-72 hours as a general, safe rule. Obviously, clinical judgement is necessary and needs to be applied to a particular patient's wound. 4. Yes, these are single use devices. They need to be changed. In fact, the PICO batteries only last 7 days and then the batteries die and/or the motor burns out, intentionally so another device needs to be applied. It is definitely not safe to keep on longer than 7 days. It also is not safe to reuse the dressing. 5. Yes, there is a sponge based system made by KCI, it is called the VAC Prevena. And yes, KCI has both the Prevena and the Prevena Plus, and then they also market a version of Prevena specifically for large, long incisions such as Pfannestiel type incisions or abdominal laparotomy incisions. Autumn Meadows yes you are correct the portable devices are only designed for 7 days. Yes as you stated there is another device for irrigation/dwelling of fluid, the name is called the KCI VeraFlow and it can be used on wounds such as pressure sores. I want to preface this by stating that law prohibits me from dispensing medical advice without evaluation of the specific patient. So I cannot tell you if your family member would definitely be a candidate for this therapy. However, I will state that I use the VeraFlow therapy every week on multiple patients and have achieved excellent clinical outcomes. We are located here in Miami, Florida.
