1. SUMMARY OF POSTINGS AND ADVERSITIES
and you will also need to prepare another document:
2. SERVICE-RELATED TRAUMA "Trauma Headlines".
That information from these documents is pasted into:
Online FORM 1: Background information
After that complete:
Online FORM 2: Symptoms and Severity
Also you will need corroborative information by sending a link to Online FORM 3 to people who can help corroborate things for you.
