(Quick links to topics are below) I explain how the QRS is created, helping the viewer understand rather than memorize when it comes to EKG reading. I review function of the His-Purkinje system, narrow vs wide QRS, bundle branch blocks, ventricular pacing, what is meant by "QRS fusion," and many basic concepts of physiology and anatomy. The video is intended for anyone who looks at and interprets electrocardiograms or telemetry! I hope you enjoy and find the content helpful!
Sections in the video:
1.Normal electrical system 0:00
2.Bundle branch block, fascicular block 4:20
3.AV block and escape rhythms 13:17
4.PVCs and Ventricular tachycardia 19:13
5.Fascicular and junctional tachycardia 26:00
6.WPW and ventricular preexcitation 37:33
7.Ventricular pacing and cardiac resynchronization 49:36
8.Cardiac resynchronization optimization (AV and VV timing) 1:05:47
9.His bundle pacing, left bundle branch pacing 1:16:59
10.Take-home points 1:23:09

Thank you, Dr.Cooper ! This is beautifully done. Decided to apply to EP and not IC after watching your videos. Can’t wait for devices timing and different operations.

There is no better compliment than that! Different specialties suit different people, but we can only consider areas we are exposed to during our training, whether that is in-person exposure or remote. It is such an honor to be able to inspire you through this medium!

I have worked with excellent electrophysiologists and EP technicians, who unfortunately did not have the ability to teach. I wish I had seen your lectures in a much earlier stage of my EP career since you have an extraordinary talent for teaching. THANK you very much! Your videos are an inspiration to EP.

I think this video is sensational. I’m not in medical field (I wished) but I am hobbyist very interested in arrhythmia and ecg and this video is such a tremendous material and explanation. Thank you doctor.

Thank you, Dr. Cooper. I love how you apply the basic principle and logic to explain various phenomena. I now understand better about BiV timing and how you can assess individual contribution from RV/LV leads and native conduction by looking at EKG. Beautifully done!

Thank you Dr Cooper. Truly excellent electrophysiological and anatomic explanations of different phenomena. The only part that I would have wished to be a bit more in depth is on how to localise the origin of a pvc based on surface EKG. Will definitely explore more of your very educational videos. Thank you again.

Doctor I also have a question. If you have SVT with aberration, would it mean the normal (after sinus rhythm is restored) ecg should show LBBB or RBBB morphology? Or can you have aberrated SVT without regular ecg showing any block? Thank you 🙏
@dr.joshuacooper-arrhythmia9917
No, just because SVT has aberrancy doesn't mean sinus rhythm also will. Each bundle branch has a recovery time (aka refractory period), and a heart rate that exceeds a certain critical rate can show aberrancy. This is called a "rate-related bundle branch block" and can result in aberrancy during tachycardia even while sinus has a narrow QRS.
