Fun fact as the number fog gates goes to infinity the likelihood that no fog gate stays the same is about 37%. So if the randomizer uses a true random method we should expect about ~60% of randomizer runs to have at least one fog gate that hasn’t changed.

11:00 The Radahn fight is typically at night, which is triggered by the cutscene. I believe there is also a fog effect which doesn't go away after dying.

As to the point you are making in the intro - most people play this with dungeon crawler mode, so running trough the same dungeons 10-50 times wouldn't be fun, it would just be tedious.
