Urgent Care means something else in the US. Urgent Care is a separate entity with offices that you can walk in without an appointment, and are usually open from early morning to late at night. They’re great for when you need to be seen the same day, and are much cheaper than going to a hospital emergency room. For a doctors office, they usually won’t have same day appointments, but if you have specific symptoms and need to be seen sooner rather than later, they can sometimes squeeze you in if you call early enough, or someone calls to cancel their appointment.

You may be surprised to hear this: in Japan, before Covid-19, you did not have to make an appointment to meet a doctor when you were not feeling well if you are a Japanese. You can just go to any hospital without an appointment and the hospital usually accepts you if you have a valid public health ID card. All you need to do is to just to write your name, address, and symptoms, and just wait. Doctors can usually make time for seeing you. This can be done because every Japanese citizen is required to be a member of a public health insurance: hospitals do not need to worry about if you can afford your medical fee.

So here's a fun nuance thing for people who really care about precision: the difference between "you (verb)" and "your (verb)," or other variations. Consider the question "Did my taking your pen upset you?" There's a difference between that and "Did me taking your pen upset you?" The first asks about the act, and could be rephrased "Did it bother you that someone took your pen?" The second refers to the person, and could be rephrased "Did it bother you that I was the one who took your pen?" It's rare that you'll encounter someone who is aware of this difference.

Дело в том, что английский я учу как и все еще со школы, и сейчас я уже окончил первый курс в университете на факультете ин.яза, но как я открыл для себя пару дней назад - я никогда не работал и даже не начинал над своим произношением, ( максимум, что я делал, смотрел в словаре как правильно читать то или иное слово), уровень английского у меня примерно средний, но даже уверенно говорить еще не могу, понимаю, что работы предстоит нереально много, я вот работаю над тем чтобы научиться говорить ( старался как можно больше слушать, учить и повторять), но также когда я наткнулся на твое очередное видео о произношении я начал паниковать, так как не знаю теперь за что хвататься и как начать работать над произношением, послушал несколько твоих видео с Веней, канал которого я нашел недавно, как Вы разбирали всякие звуки, и мне показалась довольно непростым вся артикуляция языка, и правильное произношение и честно говоря я очень расстроен, так как вообще не уверен, как теперь дальше учить английский, если все прошлые года, я даже не задумывался над произношением, ( в том числе такое элементарное сочетание как TH- в словах THink, THought и многие другие) я в полном разочаровании....я так понял сначала нужно отработать правильное произношение, а дело акцента в английском - совсем другой вопрос! Я очень удивился от того как может поменяться значение некоторых слов, фраз и что тебя могут вообще не понять, я не думал, что это может быть настолько важно, и почему то не в школе, не в университете на 1 курсе никто не сказал обращать на произношение должного внимания. Мне уже 19 и я реально упал духом, как же я потратил свое время, и каким образом я могу это исправить, если это возможно....??! Я был бы очень благодарен тебе, если бы ты озвучила свой совет, и поделилась своим мнением относительно моего вопроса! Спасибо огромное за то, что ты делаешь! I wish you would make a video linking your educational videos (like the one I saw recently where you highlight mistakes with article usage) and the videos like this one (where you yourself make mistakes by omitting articles). It's okay to make mistakes, but you come off as if you're judging people for not speaking perfectly, yet you don't speak perfectly either. Making mistakes and being able to recognize your own mistakes and learn from them is essential for self improvement. If you could demonstrate that you, too, are in the active learning process with English even as you have achieved expert level, I believe it would be a wonderful coaching moment between you and your followers.

an existing patient??? does that mean like a regular patient of that doctor?

It's weird and pretty common they asking you " the reason you are making a medical appointment", instead of asking if you need urgent care. The reason doesn't matter. It's embarrassing. Nobody calls the doc if they don't really need it.

Lucky me, cuz you know what, we don't need an appointment, to go to a hospital or just want to see a doctor, just come in and you'll see them, of course on the day work, we only need an appointment if we want to see they on the weekend or freetime.
