This is a very good question. High fT4 with normal TSH is possible in early stages of thyroiditis where stored T4 is released from the inflammed thyroid gland yet TSH is not suppressed. Another aspect is 4th generation (ultrasensitive) TSH assay should be done to detect minimal TSH suppression at the earliest.

fT4 stands for free (unbound) t4. Both are measured in labs. We hve to ask specifically for fT4.

Free T3 and T4 refers to thyroid hormones not bound to any transport proteins in the circulation. We do fT3 and fT4 when there is possibility of altered protein composition especially albumin in the circulation. For example: In Cirrhosis of liver and Nephrotic syndrome, where there is reduced serum albumin we go for fT3 and fT4. Also freeT3 is very useful in the diagnosis and monitoring of response to treatment in hyperthyroidism.
