Let’s first look at WHY you should exercise. Studies have shown that exercise, even moderate, can increase your oxygen consumption, improve blood pressure control, and provide better sleep. The right exercise can even improve your physical functioning and abilities. Exercise can also help you in managing your weight, and that is important - obesity and heart disease are critical factors in declining health. And best of all, exercise training has the potential to preserve kidney function and improve your cardiovascular risk factors!
So, what is the right kind of exercise for a person with Kidney Disease? Well, just like your diet is unique to you, so is the right exercise. You’ll want to talk to your doctor on how much and what intensity of exercise you should be doing. But in general, it is recommended that you get at least 30 minutes of moderate exercise five days a week. And if you are just starting out, try to do this for 3 days a week, taking a break in between days.
The easiest way to do this is with walking. A brisk walk for about 30 minutes will cover about 1.5 miles or 3000 steps and get your heart rate up to 100-120 bpm. It doesn’t need to be all at once either, you can do a little in the morning, and a little in the afternoon. It is recommended that you do not exercise within an hour of going to bed so that you can fall asleep easily. The walk also doesn’t need to be fast. You should be able to hold a conversation with someone while walking. No matter what your speed, just getting in 30 minutes of walking at a speed that is comfortable for you is progress.
It is also great to monitor your steps and try to work up to taking 10,000 steps a day. You’ll want to start off with an easier target, perhaps 5000 and slowly increase it each week until you are up to 10,000.
Aerobic training is also a great form of exercise. There are lots of great videos on RU-vid or you can do what I love to do, Richard Simmons Sweat’n to The Oldies! Add a step board and build up your leg and hip flexibility.
Need less stress on your body, try water aerobics at your local gym or YMCA. Swimming, biking, skiing, and dancing are all great forms of aerobic training. As you can see, there is a lot of variety available when it comes to aerobic training.
Those with less energy or just starting out can do something as simple as housework, gardening, going outside and playing with the kids - anything that gets you up and moving.
Those with more energy can try resistance exercise training. This helps increase muscle strength and size while improving your overall functioning.
Now what you don’t want to do is anything strenuous. No weight lifting, no HIT or High-Intensity Training, no fast running. These types of exercise increase the creatinine in your blood which put stress on your kidneys and lowers your GFR temporarily. If you do this type of training, your bloodwork will not be accurate for about 3 days. If you are at a stage where your doctor recommends more strenuous exercise, remember to cut back at least 3 days before you get any bloodwork done.
Now if you experience any of the following you should stop exercising:
• If you feel extremely tired
• If you are short of breath
• If you have chest pain
• If you feel irregular or rapid heartbeats - an Apple watch can warn you of this.
• If you feel sick to your stomach
• If you feel dizzy or light-headed
• OR If you get leg cramps
If you stop exercising for any reason, speak to your doctor before beginning again.

Hi Sue. I was wondering, did your lupus disease CAUSE your kidney disease? I also have an autoimmune disease, ( Sjogrens Syndrome), and I think it’s attacking my kidneys. Has you nephrologist said anything about it being harder to slow down the progression of kidney decline because the cause is autoimmune rather than from diabetes or high blood pressure?

Thank you for sharing. Sadly we have another issue and that is recovering from a stroke. Thankfully walking again but so much slower due to this. We also some exercises in the chair. Let's hope any activity is better than nothing. Ani

Here’s the thing as someone who is in medical school right now. I don’t think people are necessarily thinking financially they are literally studying everything.Textbook definition, and they are just memorizing things on a flash card. No one is really given assignments to help people think outside of the box or Alternative treatments. Unfortunately, a lot of these people are foreigners and they are taught just to follow order and the chain of command. They are taught not to question anything. A lot of this has to do with profit though creating a finical incentivized curriculum. I highly recommend doing more research on choosing a doctor. I know it’s hard with insurances, but a really good doctor that listens and thinks outside of the box can we life or death.

I was approaching stage five kidney failure. Because I’ve always worked out, I mustered enough energy to start spinning 🚴. I spin approximately four days a week, at one hour each time. Giving me around twenty miles each time in the four days equaling eighty miles a week, along with dips and other exercises. Through (omad) and drinking plenty of water 💦 has made my life much better in living with this disease.

Found your videos just as I decided to fire the Kidney Dr. I have had for several years and find one that can work with me. Current Dr. is poor at conversation and explaining the urgency of this condition. Last week I set in his office with a BP of 175/99/72, he didn't blink. I do not feel he is a good fit for me. I made the first step and began researching. I found MCT/Coconut oil. Imagine, I have a huge jar of Coconut Oil sitting on top of my fridge. Time I learned how to use it. I also began cooking from scratch. Turns out when you are eating FOR you kidney's the meals are simple. I cooked a 1/2 of a red pepper and a Valdalia onion until they were nice and limp. Added the whites of two jumbo eggs. It was a delicious meal and I stayed satisfied for most of the night. (I CAN DO THIS!) Especially, with you coaching me. By the way, I am turning 79 the end of Dec. I have been walking daily, I will try to get one of those watches you mentioned and walk more. THANK YOU for your videos, they are a big help. Plant based protein is great - but make sure it is non-soy. Soy can cause inflammation, which reduces your kidney function and puts more stress on the kidneys. That list was created with the input of multiple Nephrologist and Dietitians to be as complete as possible - with a focus on low inflammation Kidney-friendly foods (to help us all get better quicker).

All doctors have told me exercise itself doesn't improve your GFR, but it impacts other important aspects of your health which then can impact the burden placed on your kidneys. The impact of exercise right before getting labs done is also different person to person based on your muscle mass and other factors - so your doctor can give you the best advice for cutting back any (if recommended) before getting new labs so that they are more accurate. Intense exercise itself isn't bad for our kidneys, it just makes getting more accurate labs difficult (and those are our guides).

Challenge? On the data base it may say 108 mg of potassium how do I figure out if the number is high, low or right on for my body?

You need to find out from your doctor or dietitian what your daily MIN and MAX are for Potassium (and the other nutrients). Then track how much you eat (an app makes it easier) and see what fits in your diet. With CKD, your daily limit will usually fall somewhere between 1500 and 2700 mg. Something that is 108 is easy to fit in. Something that is 1000 is too much for a single item unless you only eat once a day. It takes a little time to get used to what is too much for your specific diet, but a way to start is to keep each meal to 1/3 of your total if you eat NO snacks. If you snack during the day, keep each meal to 1/4 of your total and give yourself 1/4 for all snacks.

Good evening, I very worried about my son. He has had Diabetes since the age of 7, now he is 29 and on Dialysis. He is over weight and unable to get on a Kidney transplant list due to his weight. Any advice for him. He is not able to lift weights since he can’t build muscle on his arm. Please help. Thank you

No. Please avoid anything heavy. Just do treadmill for 40 mins at 5.5 speed. And 20 mins in your community.

Great question and congrats on being so active! You must check with your doctor first, but when I have long days of work (I'm on my feet ALL day moving around, talking, etc.) I add some liquid caffeine to my water (which also contains crystalized lemon, or raspberry, or some other crystalized flavor). I absolutely love True Lemon for the crystalized flavor (not a sponsor - I love their product and use it DAILY). Then find a liquid caffeine you like and you can make your own energy drink. Liquid caffeine can be dangerous - make sure your doctor is ok with you using it and you DO NOT overuse it. It has to stay out of reach of children - I use drops and they are in a locked cabinet with my other meds. Again, check with your doctor before trying and be attentive on the amount you use.

Hi, i loved watching your video. I hope you are ok & everything is well with you. I've been lifting weights for over 20 years, light & heavy. Im 44 years old. I dont have kidney disease yet, but i notice evrytime i pee, i have foamy urine. My creatinine level is 1.31 & my GFR is 69. Im also pre-diabetic. I believe protein is coming out my urine making it foamy which means my kidneys are not working properly. I have to say, my workouts are strenuous. Ive been working out 6 days a week doing this V Shred program, ripped in 90 days. He does a lot of HIIT excercises too. My question is do you think whats happening to me is because of my workouts? Do i have to completely stop lifting weights? I really love working out & lifting weights. I dont want to lose all my muscles if i have to stop. I dont take supplements at all. I get my protein from my food but because of my foamy urine, i may have to eat less protein which i already dont eat alot of. My nutritionists said im not getting enough of it. Im just confused. I just wonder if my workouts is causing of of this. Can i lift light weights? Do i have to stop lifting heavy weights? Can i do calisthenics? Sorry about all the questions.

What do you think about cycling is that too much exertion? I commute to work wich is about 8 miles 1 way once in morning and 8 miles in the evening??? I'm trying to become one of your disciples and join the fight. I started out at 38 gfr after I started my medication sodium bicarbonate and 5 mg amlodipine it dropped to 24 and after my body/ kidneys accepted it it has slowly progressed to 48, my bun is at 30 and creatinine is 1.8 Now

Cycling is a great exercise - as well as jumping rope (both of those are easy on the body while giving it a great workout). To lower your BUN check out Renadyl ( www.dadvicetv.com/renadyl ) - I just finished a 90 day trial of it and my BUN went from in the 40s to 28 - just 3 points above normal. I'll 66 with a BMI of 22. I've been a cyclist most of my life. I do an average of 2 hours per day and I stay in the aerobic zone. My GFR is constantly in the 50s. My energy level is through the roof. The problem is that I've got excessive protein in my urine.

At 66 and with a eGFR in the 50s - you are doing great. Talk to your doctor about medications to reduce protein leakage. Dr. Rosansky discussed them during a few of his visits here, but I'm not sure exactly which videos that discussion is in.

Don't push to hard, but know that Zone 2 cardio for 40-45 min every other day ie 4 times in a week is a game changer. im 25 years into kidney disease with IGA.

First is to visit your doctor - trouble breathing is extremely serious and needs a doctors attention right away. Your doctor can also do labs (bloodwork) to see if you are anemic (low iron, low Hemoglobin, low Red Blood Cell Count). Once you know your labs you can make adjustments.

What kind of exercise do you recommend for the disable? I wear braces and a walking cane, after the accident injury I can not go farther than 3 minutes on my elliptical, my knees gives out and the lower back pain gets unbearable, I refuse to take any pain killers; I was seen by a Nephrologist for over 5 years and all he did was to draw my blood & do a Pee test four or five times per year and all of the sudden I need a kidney transplant and go on dialysis, this butcher railroaded me to his dialysis clinic but, I am working to reverse CKD 4 by doing the Naturopath way .

Great question! While I am not a doctor and am only speaking from my experience, would you be able to try swimming or water aerobics? Those can be low impact, low stress, and move at a more manageable pace. One of the best things I learned about the exercise we need is that it just needs to be enough movement to get out heart rate up a little - nothing strenuous - and that it is accumulative. You can get 5 minutes here, for gardening for 5 minutes, etc. It's ok to get it little bits at a time throughout the day. I use my fitness tracking watch to take care of adding it all up throughout the day, then around dinner time, I check to see where I am. There are days where I set a longer goal (1 hour) and I'll get in a little upper body dancing (my dogs get involved if I dance and move around, so I just get some upper body movements going to some music (or one of the RU-vid exercise channels). Hope this helps!

What about bike riding? that is my way i get around i don't have a car so that how i go food shopping, Now that the weather is getting nice i will ride more and do you take baking soda and do you talk about taking it, if i'm sore i take that and i feel better because i have back pain at times. please reply with the best answer you can give.

Bike riding is a great form of exercise for those with kidney disease. It works your muscles and can be used as a signal if your potassium is too low (you'll get tired much much quicker). I do not take Baking Soda from the box, but I so use 650mg Sodium Bicarbonate tablets (which is baking soda). There is research showing that 650mg's a day can be beneficial to those with Kidney Disease. I use it in place of anti-acids for upset stomachs. I haven't done any videos yet of any supplements I use - I plan to do some in the future. I take both traditional supplements (D3, Fiber, B-vitamins) and some non-traditional ones (for specific issues I am targetting, such as KDIR when I used to get water retention at a lower GFR). I list most of them on my website www.DadviceTV.com and always recommend checking with your doc first before adding them to your daily plan.

I use these as my little pee helpers celery, pumpkin puree, stinging nettle, Hydrangea root, horsetail, corn silk, i also take D3 MY FIBER IS FLAX SEED MEAL and in the morning i take dandelion root it acts almost like coffee to me i get a lot of energy if i don't do something physical like ride my bike or walk or something it will take a long time to get to sleep, i have taken it too late ( i'm talking about dandelion root ) in the day ( that's after 12 pm ) that i couldn't get to sleep until the next day like around 2 or 3pm! So i only take that every once in a while. THANKS FOR WHAT INFORMATION YOU SHARE WITH US! You are appreciated! PS. I was taking honey in my tea is that ok? that was my B Vitamins

Last weekend I rode my bike 360 k.m. I guess this will be the last time I do that again. I am stage 3 A . I guess you can be fit but sick. It was one of my slower attempts at the tour.

The best recommendation I can give is to get with a renal dietitian. Focus on your overall health and avoid anything which is bad for your kidneys (like smoking). Check out the videos with Dr. Ro and Jen Hernandez on DadviceTV.com - very helpful.

You may want to check out pool exercises - such as water aerobics. The Meta Quest (VR headset) also has fantastic programs for those who have mobility challenges - even for those in a wheelchair or bed bound. Your doctor can also be a great source for recommendations.

Cycling is fantastic exercise - keep yourself hydrated. If you do marathon cycling, there is a study that shows it could have a very slight negative impact on kidney function during the event, but can actually improve kidney function during your recovery period. I'm not a doctor, so it's always important to check with your doctor who knows your full medical history.

I am in stage 4 but I get cramping, usually when my sodium and/or potassium are out of balance. My doctor has tested for that, and I can adjust what I eat accordingly. You really should ask the doctor, because there are other things that can cause the cramping too... prayers for your success!

Kidney symptoms can be caused by many other things, so it's best to have your doctor do blood test and urine test to check your kidney function. Only then will you know if any issues are being caused by your kidneys.

I have had stage 3 kidney disease for the past 6 years. For the last 2 years I have been lighting weights at a gym with a personal trainer 4-5 days a week. I don’t lift heavy weights. Is this type of exercise okay for my kidneys?

Welcome to my world. You'll NEVER get a reliable answer on this, not even from your nephrologist. Best I could get from my nephrologist was that if you can do 15 reps, the weight isn't too heavy.
