PCRI’s Executive Director, Mark Scholz, MD, discusses a range of solutions for alleviating fatigue brought on by androgen deprivation therapy (also known as testosterone inactivating pharmaceuticals or TIP).
0:10 Fatigue is the most common side effect experienced by men undergoing androgen deprivation therapy, and it is most often the result of reduced muscle mass caused by the lack of testosterone.
0:44 The best way to counteract fatigue from treatment is to have a consistent devotion to exercising, specifically weightlifting, 2-3 times per week. Significant muscle mass loss can occur in as little as two weeks, so any lapse in your weightlifting regimen can have serious negative consequences including reintensifying fatigue. Typically, consistent and diligent weightlifting will reduce the amount of fatigue that men experience by about 80%.
2:07 There are other possible causes for fatigue while taking androgen deprivation therapy (e.g. Low thyroid levels, low B12 levels), and so it is important that your doctor is checking for these rather than assuming that the source of fatigue must be from the lack of testosterone.
2:30 On rare occasions the absence of testosterone may cause anemia to occur. If anemia becomes severe then cardiorespiratory reserve is reduced and shortness of breath with minimal exercise will occur. Most cases are mild and do not require treatment, however, if it becomes severe (usually signaled by hemoglobin less than 10 or a hematocrit under 30%) then pharmaceutical agents like Aranesp or Procrit can restore blood levels to an acceptable range. Transfusions have greater risks and should not be considered unless the situation is horribly severe.
3:31 Other solutions for treatment-related fatigue include the use of caffeine or Provigil (modenafil), a stimulant-like medicine typically used to treat narcolepsy. These are most effective when they are used occasionally so that your body will become acclimatized to their absence and their effectiveness will be reestablished.
4:15 Another thing to consider is the pharmaceutical agent(s) that are being used for the androgen deprivation therapy and the duration of treatment. A shorter duration of treatment might be appropriate for elderly men with milder forms of cancer who are experiencing extreme fatigue. Xtandi is more associated with fatigue than its most similar competitor Zytiga, so it is worth considering switching if Xtandi is causing fatigue. Casodex is an oral agent that has less potent anti-cancer efficacy than injectibles like Lupron, but it also causes significantly fewer side effects. In elderly patients or patients with severe fatigue, Casodex is worth considering as a substitute for the stronger injectible agents.

The worst is being in a continual sweating condition. I could drowned in my own sweat. Nobody told me "hot flashes" could be continual! You never talk about that.

Try getting Lupron during 5 months with a catheter. It was/is all I can do to keep my spirits up and sense of humor going. I started taking B-12, 5,000mcg(Methylcarbamol). Now walking one mile a day, I used to do 3 miles about 5 days a week, that is my goal to get back to. Also Chi Kung stretching exercises to work my core muscles. Bets of luck to all, things can get better, stay positive and work hard.

I’ve used cooling towels to control the hot flashes. I get regular B12 injections and espresso every morning.

My doctor at the Cross Cancer institute coerced me into getting a six month shot of Eligard without informing me about all the horrific and long term effects of this toxic crap that totally destroyed my quality of life and continues to keep me well below castrate level 5 months after the six months expired, with little hope of ever getting my testosterone level back to normal. I totally refuse any more if this quality of life destroying, and life threatening poison. She threatened me that she wouldn't let me off ADT for at least a year, and that I could never ever go on Testosterone Replacement therapy. Well, never going back to that doctor. She never mentioned anything about taking Casodex as an alternative. Coercing ADT without free and fully informed consent is criminal assault causing greivois bodily harm.

If on hormone therapy that suppresses testosterone, then you go to the gym and gain muscle, muscle gain elevates testosterone productivity....this leaves me a bit confused.

I am currently swimming 3500 miters twice a week. Is this adequate to impact the the onset of fatigue from ADT?

Hello, I am not an expert, but I have heard a few different doctors at our conferences over the years stress the importance of resistance training to mitigate the fatigue from ADT. My understanding is that while all exercises are a mix of resistance and cardio, swimming is mostly cardio, which is great--especially for heart health, since heart disease is the number 1 killer of men including men with prostate cancer--but the fatigue from hormone therapy comes from muscle wasting which is best counteracted with resistance style training. However, since you swim such a long distance, it is possible that your swimming could be building enough muscle to counteract the muscle loss of low testosterone, but I am not sure. We have a free helpline with a patient advocate who might know more about this or he may at least be able to tell you more about monitoring to see if you are losing muscle mass and whether it would be good to add more resistance focus techniques to your routine.
