Partient had photophobia as well so the question was of migraine. Also patient had particular trigger "hypoglycemia" was mentioned. So it was clear cut case of migraine.

Sir altered sensorium wala q me dusra recall dekha tha but usme ble icu management but me diya tha ipd with iv....so bura laga tha..but ab dekha ye ansr apke hisap se mera correct huwa...thnks sir❤

Answer is magnesium sulphate. It causes fastest evacuation of stool in 3 hours. B and C in 6 to 8 hours and lactulose in 24 hours.

Sir this question was which among the following conditions JVP will not fall on inspiration. Options were Tension pneumothorax Hocm Cor pulmonale pericarditis. Sir my doubt is , why not tension pneumothorax? If there is tension pneumothorax because of high intra thoracic pressure the heart will be compress and JVP will not fall.

It was not astrexis, patient had sensorium problem, he had probably asthma and because of beta agonist, pt had tremors. Since patient age was 70+ , he might be taking the inhaler more frequent. It was not astrexis 100% sure. It was a case of drug toxicity of beta agonists. Question was from pharama.

Altered sensorium in asthma is not drug toxicity. That is the main teaching always. If you want to edit your comment and change sensorium then drug toxicity is on.

Sir there was already given about dysphagia like symptoms why best diagnosis was not a manomatry it could help in differentiate between aklasia cardia and ca esophagus

In cardiac tamponade we see sharp x descent, and absent y descent while in constrictive pericarditis there’s sharp x descent as well as sharp y descent.

Sir, Limb edema is still possible in DVT leading to PE. And since it is sudden onset of symptoms, it is more suggestive of DVT and PE rather than RHF secondary to MS. Also, patients usually have swelling and heaviness in both limbs rather than pain.

Bicarbonate. machine can measure oxygen and carbon dioxide which are gases and then uses a formula called Henderson hessbach equation to derive bicarbonate . (Kyunki aapko formula ka name pata hoga isliye usne formula na puch kar uski importance puch liya)
