Have you ever done spreader grafts with a closed rhinoplasty ? I’m confused as I’ve been in several conduits. I was told my nasal bones are short and need an open rhinoplasty. Some of the surgeons said I don’t need it. It’s my natural nose I’ve never had a nose job just ha e a naturally thin nose.

With an increasing number of people getting body piercings, would you please do a video on the risks of such procedures - lip and tongue piercings, bull nasal rings, genital piercings etc. I am a Mum of young children and are concerned about how prevalent this is becoming amongst young adults. In a country with about 1.4 billion people, there are millions of girls who are just as attractive or more attractive than her who would kill for her job. Of course she’s disposable and seen as an object by the industry.

Yep. Very few, especially women, can get away with a blunder like this. No offense to Mr. MJ, but you have to be a Michael Jackson to still be huge in the entertainment world with a bad nose job.

She only has a career because she has had cosmetic surgery in the past so it's kind of a catch 22. I would say she didn't need more but I doubt she would even be in this career without these surgeries in the first place.

Cosmetic surgeries become an addiction like anything else. I feel like many people develop body dysmorphia after their first surgery, especially with nose jobs.

I was planning to get a rhinoplasty because my septum is slightly deviated to the right but had a nightmare a few days ago in which the center of my nose was left deformed. Today, this pops up on my feed. I'm reconsidering the whole thing to be honest, this is terrifying. I hope she is able to get through this.

If it is just slight, that is a very difficult surgery anyway because there is nothing for the surgeion to “work with”. Usally the better surgeons refuse that type of job, so then you are left with mediocre surgeons doing a comlplicated risky surgery. It is a throw of the dice….

Honestly this is so tragic, if you guys look at her before pictures her nose was fine. Actually more than fine, she had the type of nose that people ask for when getting a nose job, perfect ski slope and everything. It just goes to show how much unhealthy pressure there is in the entertainment industry, and how distorting and body dysmorphic it can really get

Asian Rhinoplasty is characteristically an augmentative rhinoplasty and having done an 18 month Facial Plastics fellowship in Seoul, South Korea at multiple different institutions (plus also attending multiple Asian Rhinoplasty conferences) during my time there I rarely ever saw anyone use the closed approach for a primary or major revision rhinoplasty in Asian patients. It is generally all open. Of course, minor revision with some contour camouflage and tweaking is often done closed, but substantial dorsal augmentation and tip multigrafting for elevation and definition is preferably done open. That patient with tip necrosis is an absolute tragedy. She will likely require a paramedian forehead flap for reconstruction and have a fringe for most of her life. As she is young, the nasolabial flap is likely to lead to a less cosmetically pleasing reconstructive outcome, especially at the donor site. Looking at her pre-necrosis photos, this was undoubtedly a revision rhinoplasty with likely previous alar base reduction. So there is compromised blood supply in addition to a less elastic soft tissue skin envelope... that may have also been defatted (either previously or during this operation) and additionally there may have also been damage incurred during skin envelope elevation this time round. The above factors, combined with over-enthusiastic (aggressive) tip augmentation... They may have used an L-shaped silicone implant (big no-no) or overdid the multilayered tip grafts (and additionally may have omitted adding a fascial layer on top as an interface to smoothen any hard cartilage edges)... Leading to tension at the tip and on closure. I was taught that it was better to be cautious and slightly underaugment, rather than risk closing under tension. Otherwise, perform a concurrent subnasale release and advancement flap to relieve tension. Never close under tension... That is the golden rule.

Thank you! I have to get an “ethnic” rhinoplasty as they said here in Los Angeles. The biggest rhinoplasty surgeons in Beverly Hills turned me away because i needed this “ethnic” rib grafting. The closed/scarless aren’t an option for me because my nose 1. Has no bridge 2. Is rather short and needs elongation. Closed nose jobs don’t allow for that kind of reconstruction

Okay I feel like my skin is just naturally under tension, I've watched it split like a squashed grape. Any wound over say a centimetre will burst open as wide as it can meaning I need stitches for things other people don't seem to. No cosmetic for me!

Because she already had surgeries. She got famous by doing this in the first place. She just didn't know when to stop sadly. But you can't say the surgeries are bad or not worth it because she wouldn't even be famous without them.

I can't even watch the image of her nose, this is horrific. Her nose was fine, was cute and pretty, she doesn't need any more changes. She's unlucky to be going to the wrong hands. Hope she heals physically and mentally

Kat31017 i think for sure she can sue if she wants to but whether or not she wins the case depends on if she signed something prior to her surgery. I think most doctors would make their patients sign something to avoid liability should something like this occur. It all depends on the lawyers and the judge in my opinion.

ThatOne no, that’s not necessarily true, especially in big cities. People think just bc something happened in china, there’s automatically less freedom and whatnot but the main thing people are not allowed to do is to criticize the government and yea, people don’t have full access to the internet, but then again, many people use vpn. Also, if someone breaks the law, they will face a harsh sentence, unlike in america, where convicts are freed again and again.

I feel so bad for this girl! I suffer from a botched rhinoplasty and it really effects your day to day life. My nose looked okay in the beginning, but years later when the swelling went down, I noticed how much cartilage was really removed and I can't believe the surgeon didn't take this into account.

I had the open type of rhinoplasty done in Korea about 5 years ago and my nose is still perfect. I had 4 procedures done: fixing the hanging columella, hump removal, tip reshaping and inserting cartilage into the nostrils. I had the surgery for cosmetic reasons, because, quite frankly, it was insulting that my nose was not only disproportionate and ugly, it didn't even function (too narrow to let adequate air through, I always got throat infections) - and if it was going to be a decorative facial feature, it may as well look pretty. But my surgeon completely fixed my breathing (widening it, inserting cartilage) and the appearance. I don't want people to worry too much about this just because of her case - all surgeries carry risks - just make sure to do your research and find a surgeon who is trusted, experienced and if at any time you feel uncomfortable with how you're being treated (if you feel pressured or disrespected) don't do it.

I had the same procedures in Mexico, with an excelent doctor, he Is a certified plastic surgeon and otorryno, i had no problem with the procedure, now i feel my face Is all i wanted.

@@raygin6581 Hi sorry, there is a forum called purseblog forums where many people have shared their experiences with Korean plastic surgery clinics. I researched on there for years to see what people had to say about their experience at different hospitals and what hospitals were better for the result that you were looking for until I made my mind up. There were some fake reviews I think, but not many and people were wary of posts that were overly positive or very negative because they probably weren't genuine. In any case, that forum prepared me very well for my surgery so I recommend checking it out.

Well... This is what happens when we develop a society that is so focused on self image and fitting in. People don't even have unique facial features anymore, everyone gets surgery to look the same.... I truly wish people could feel more comfortable being themselves.

@@LareesieAlice not necessarily, society as we function in it now; that was primarily shaped by Edward Bernays and his book “Propaganda”. The Jesuits do not have as much sway in modern times, Bernays’ influence on public relations, marketing, and certainly media ( the trifecta of true evil) has had more of a reach than anything else. Fun fact: he was Sigmund Freud’s nephew (Freud did not respect him or American culture, interestingly enough)

Why aren’t leaches used in cases like this? They stimulate blood flow. Might be the difference in loosing the tip of your nose or not?

Yes! I watched someone who had the same rhinoplasty as Gao and she had to set up an appointment for leech therapy the same day. It saved her nose.

Yes we use leech therapy but remember it depends on hospital as leech therapy takes special sterile leeches. We also use maggots too who eat the dead flesh the issue is the skin is already dead

They must be really close minded to want to look exactly the same and same weight! They must be afraid to be different. That’s sad cause they r all followers instead of leaders. Young girls especially r followers. Teenagers. Instead of making life fun, they make their lives hard. Hard to compete with perfection that doesn’t exist. Perfection expectations ruin the fun out of life. They should just think about enjoying life, not criticizing it. Then when their old, they have nothing to show for fun life. 🤷‍♀️🤷‍♀️

Well you scared me. Good thing I’m too broke to afford surgery. I really didn’t think she needed any work done. But as a Chinese person myself, I get that China’s modeling and acting industry is cut throat and their beauty ideals tend to be hard to achieve, so I get why she went for surgery.

I'm also chinese (I don't live in China tho) but I've been told I have a rather "western" nose. A lot of people call me a witch lmao and it made me want to get a rhinoplasty, but I'm too broke and terrified to get it anyway. She had a perfectly good nose (even in asian entertainment industry standards imo), I don't know why she even needed to get it done in the first place 😢

@@Lilliz91 as others pointed out, Asian Countries are more strict even though you’re right in the entertainment industry beauty ideals are strict. I know going off anecdotes may not convince you. But consider this, in many East Asian countries there is only one or two ideals. If you are close but not exactly like the ideal, you are automatically considered ugly. Even when in western countries you may be seen as pretty. That’s how Chinese culture tends to be. It’s one way or the highway. And a few rebel against this but usually not enough to make any significant changes in their culture. In the Western media there is more room for beauty and there seems to be a spectrum. So you aren’t tossed in the ugly department just because your nose is a few mm off

@@dokadoka4922 I find what people consider beautiful in China is not always beautiful in the United States for an example. The US prefers smaller noses. Where as in China it’s small and tall noses. Fan BingBing for an example is considered the ideal and yet some people in the West say she has a Prominent nose. Your nose might be beautiful in China.

I got necrosis from a surgery. After I got the necrosis my thoughts on surgeries is get as few as humanly possible. The more surgeries you get and the more invasive the surgeries you get the bigger the chance of necrosis. Less is more

Gosh I really hope she'll be able to find a doctor who can fix it. I feel so bad for her because she probably had surgery so she could fit an unattainable beauty standard. She was so gorgeous and really didn't need plastic surgery. Many people are getting surgery so they could get HER nose. I really hope she gets better.

This is terrifying. I don't want to change how my nose looks, but I get a lot of nasal congestion partly from a deviated septum. But there are so many nightmare stories about botched surgeries that I might rather just live with congested nose for the rest of my life.

If you tell them to do only what is necessary to correct the function of your nose they are going to be doing a whole lot less than if you ask for cosmetic outcomes. Mostly they would just shave a little off the septum in one small spot of each curve they wish to correct. Much less risky.

Just do it. I had the same and did it a couple years ago and nothing happened. Communicate with and explain to your doctor. They are used with a lot of deviated septums as that doesn't necessarily mean aesthetic correction on the tip, etc. etc. There are different things done depending on what the person wants.

Poor babygirl! She was perfect before surgery! It angers me when doctors take their money and botch them up, and they did not need surgery in the first place! I would imagine if you put her out, bandaged her up, and then showed her the big reveal (when you did no surgery at all), she would love the result!

The worst part is she didn't need a nose job and the new nose (from what can be seen of it) doesn't look better. It now looks too small for her face on top of the obvious necrosis of the tip.

I had a nice pointy nose but broke it once and cracked it a second time. Went to the doctor and she just squished it into place and now I look Italian lol No way I’d go into surgery unless I’m dying.

I had my nose crushed in the military! The doctors at the VA hospital in Durham NC were told by the doctor from Duke hospital how to do my nose. They took bone from behind my ear to rebuild my nose. That was in 1974 I still have problems with one side of my nose breathing.

The mad thing for me is that before her nose was just perfect - as other commentators have said the kind of nose you’d ask for yourself. Tbh even if she hadn’t had complications her new nose shape I don’t find attractive at all compared to her natural one - it’s just so upturned with barely any length. What does everyone else think?!

I agree with you but like he said, she went for a revision but the doctor cut her blood vessels during the operation. Her nose was already plastic.

i flew to turkey from australia for a rhinoplasty and my nose still looks great 3 years later. my surgeon is an 'instagram' surgeon too, im pretty sure he put my before and after on insta lol. my friends cousins also went to him first and had great results and no issues so thats what also made me more comfortable to go there and have it done! had a very good experience

I think he’s referring to the barbie / baby noses that a lot of Turkish surgeons do . If you find their Instagram pages a lot of them do 1 nose on all their patients.

My recommendation would anyone seeking cosmetic surgery, Do your research Make sure you can afford pre and post care of your surgery Do not be cheap on your procedure Don’t go in by yourself Make sure your health insurance covers cosmetic surgery or buy an insurance for that type of procedures Know your surgery options

In my experience, I've never had a plastic surgeon offer both open and closed techniques for a rhinoplasty. Is that because some surgeons aren't skilled in both?

I am researching various surgeons for a deep plane face/neck lift. I just recently, on another video, found Dr. Motykie. I live in S.D. so logistically it is more challenging to have such a procedure done at his location. But his work looks really exceptional, on the few videos I've seen so far, he explains details well, and seems to genuinely care....so I'm now reviewing as many vids of Dr. Motykie's that I can to help me make the decision. No I'm not scared, I feel informed, thank you!!

I've noticed a lot of Turkish plastic surgeons and any surgeon on the other side of the world uses mostly open technique. Why is that? Is it easier to perform?

Unless something actually happens to my nose and it needs to get surgically fixed I'm not going to do anything to it. I have a bump on it yes and had it ever since I was a baby but that bump so far has saved my nose from breaking multiple times over my life. I have literally fallen right on my nose in an accident that would have probably break weaker noses without that extra bone but instead I just got a bad bruise and no break. Even the doctor was showed when I told him how I injured my now and he didn't trust his manual assessment and wanted an x-ray because he was so sure it had to be broken. Nope, that extra bone once again has done its job. It's a very skinny and otherwise nicely shaped nose and you can only see the bump from the side and I've grown to love it.

It is so true doctor. The important thing is the result years after the procedure. I had blepharoplasty done 6 years ago. I initially regretted the decision because it looked so ugly and swollen for a good year. But that’s behind me now. I get compliments when I mentioned I had mine done. It looks so natural and well done.

Dear Dr G I very much enjoyed your explanation regards Marylin Monroe. The Plastic Medicine procedures she had and how her looks were enhanced. She is my favourite Hollywood star. The rhinoplasty was not necessarily botched - every surgery has risks and she did not heal the tip of her nose necrosised.

5:42 this hits hard. My nose even 3 months after my surgery looked vastly different from what I saw from the table picture. The doctor offered to fix it, but of course with some charge that I couldn’t afford, and I honestly lost that trust at the time, because what he said prior to the surgery was all misleading.

Everyone in the comments saying they're changing their mind about cosmetic surgery, guys... just do research on your doctor's and legal coverage, a good doctor and PROPER AT HOME AFTER CARE will result in a perfectly fine result.

I’ve had an open rhinoplasty (preference of my surgeon who is board certified and is based in Manchester UK). I’m 5 months post op and still don’t have complete feeling back in my tip. I’m hoping it will return after another few months. My scar is much more noticeable than my sisters (who had her surgery 6 days before me) but I realise my skin is quite thin and shows up pink scars quite easily. It’s a long game but I’m trying to be patient!! She had her surgery in turkey but went through a British clinic and seems to be healing well (fingers crossed!)

Wow… I had severe breathing problems and constant sinus infections growing up. My doctor was surprised that I could actually breathe at all. Anyway, I ended up getting a Septoplasty and Rhinoplasty. Surgery went fine and I was able to breathe for the first time that I can remember. It was preformed “scarless” and my surgeon left me awake under mild sedation because of facial drooping? Or at least that is what he said. That was back in 1992. Unfortunately, I was taking care of my neighbors daughter a couple days after the cast came off. To make a long story short, she was young and discovering noses. She tweaked it and I was worse off than before. In 2004 I went through the whole thing again but this time it was an open surgery consisting of my plastic surgeon and ENT surgeon. They took turns I guess. Anyway, since the second surgery I have had not that many problems. The only problem I have is they made the bridge of my nose too narrow and I have a hard time wearing glasses. I don’t want more surgery but this is ridiculous

I had to have an open non cosmetic rhinoplasty because I have a rare sinus condition so they had to work on my sinuses as well as repair my deviated septum, shave the turbinates, and remove Haller cells. I’m nearly 3 years post op. The nose looks good, but the nerves still haven’t completely returned. My subsequent sinus surgeries have been closed.

Did it help? My nose is allergic to so many things, and being constantly on Allegra and medications to help thin the mucus membrane wasn’t enough to stop my recurrent sinus infections so I lasered off turbinates in both nostrils and also did a balloon stent procedure to open up the passage to the right sinus under your eye. It helped some but not dramatically. What really annoys me is that I can breathe through my nose when I am awake but when I lie down, they close up and I can’t breathe through my nose. My septum is slightly deviated but more than that they told me the passages to my sinuses are all very thin and narrow so they would clog easily. The balloon stent and laser turbinate reduction were relatively non-invasive procedures I was able to just do in my doctor’s office but if it would improve my quality of life, I might consider something more drastic. But invasive procedures on the nose/face terrify me so even though I also have TMJ I haven’t done anything about my jaw either. Both my nose and jaw issues are mild enough that no doctor has been like “you really should fix this surgically” but the issues they cause do lower my qol. Sorry for the rambling comment but I was just wondering how you decided to get these procedures done and if they’ve helped.

@@Satopi3104 I developed Silent Sinus Syndrome and surgery was my only option. I required a 2nd surgery and will likely need another to clear out my sinuses. They were all short stay surgeries, so I was under anesthesia. My daily life includes Allegra, Flonase, at minimum twice daily sinus rinses (NeilMed). I also had allergy testing and now have regular allergy shots. I still need to use Guaifensin on a fairly regular basis. I also was directed to sleep with a cool mist humidifier next to the bed. The cool mist humidifier is helpful in moisturizing the mucosal membranes and it can also help your skin with trans-epidermal water loss which is helpful with aging. The upside is that I haven’t had a single sinus infection since my surgeries. In a few months I have to have another in office endoscopy and see my ophthalmologist for the eye affected by my sinuses, together they decide if I have another CT and surgery. Your ENT would be the best person for you to speak with. Perhaps you may find relief with sinus rinses or allergy testing? Do know that about 85% of humans can only breathe out of one nostril at a time, we essentially have two noses separated by a septum. The nasal flow switches every few hours (average is around 4 hours) as airflow slows due to tissue swelling with increased blood flow. It is possible that your deviated septum makes you more aware of this cycle and symptoms of congestion. I hope you can find relief and your ENT can be of aid to you. Oh my! That poor girl I’ve worked with patients suffering with Necrotizing Fasciitis and I can only imagine how bad the smell was that she would have had to put up with (so close to her nose too agh ! 😩) Truly awful . I hope they managed to debride all the dead tissue quickly for her - in fact how does that even work on such a small delicate area as the nose?

The surgeon who did it wasn’t actually fully qualified for rhinoplasty (but other plastic surgeries) so it sounds like this was due to inexperience and lack of supervision on the part of that doctor. Apparently he was only fined c. $5,000. How did she even end up on his operating table!? Surely actresses have better resources for these things…

I am +5 years post op (I had 3 rhinoplasty). I need a rhinoseptoplasty. one of my nostril is collapsed + nose deviated! I need rib cartilage 100%... I waited this long because I am scared but I cannot breathe through my nose and it does impact my day to day life! I feel like a closed approach for such a complexe case like mine would be impossible.
