I've been playing this game for the first time recently too, and I've learned a few things that could help you, without giving away any spoilers. When you change from one elevation to another, e.g. from the ground to the first bridge level, if you do this too close to another bridge section the cars that are changing elevation will go through the bridge section, stopping any traffic on that section until they pass. Each screen has been given a 'complexity rating' by the game author. The road you only had 97 efficiency score on was because you used too much concrete. It's only necessary to connect the roads together in a crossroads with no bridge sections as the volume of traffic is so small. If you press the ? icon in the top right when you are in the map view, you will see a set of instructions, and be able to see all the pictures you have already unlocked. If a screen has a picture, the score you need to achieve is displayed in the top left of the efficiency score window that pops up when you hit the stopwatch, and will disappear when you have reached that score. Only then can you view the picture associated with that screen. Your total efficiency score is determined by A: the amount of concrete used (the higher a bridge section, the more concrete is used when building it), B: The average speed that vehicles can go at (making junctions smoother and roads less bumpy improves this), and C: The complexity score that the author has given each screen. A more complex screen will be more forgiving for the amount of concrete used, while the less complex screens will penalise you more for using more concrete than is required.

For the Mall I wonder if adding that cross section WITHOUT the bridge might be helpful. Or maybe adding a couple slip lanes to ease traffic.
