How would you send an SMS message using that system?

It depends on the mobile network operator if SMS is supported in the NB-IOT network (there are some, e.g. in Italy). The module is able to handle SMS. As SMS is transfered in the control plane as well it should be comparable with nonIP traffic (but I would place this as to be discussed further).
