Per the logistics y'all two sorted out during med school / res. training: I went to veterinary school an 8-hour drive away from where my husband worked and his income paid for my tuition / living expenses (I worked a PT job in vet school, and in summers). No kids. We saw each other q 3 weeks, and summers. Conversely, as he got promoted in his career after I graduated, we followed his career not mine. We have been married 41 yrs

I think ur low T treatment journey is so important to talk about. As a practicing physician, i think it would be so good to hear people having person experience with taking meds that u can’t get from Lexicom. Thank you so much for shearing!!!

I think that all health insurance companies should be required to be Non-profit by law. They also should be required to pay for every procedure, test, visit and RX that the patient's physician recommends. They should not be able to deny coverage to anyone who is enrolled. Also, no person should be subject to pre existing conditions. These health insurance companies are literally killing us.

I’m a semi-retired OBG in private practice. The reimbursement fiasco is making it increasingly difficult to provide individualized care to patients. If we were owned as a hospital based practice our salary would be dependent on how quickly we could cycle pts through the system. If we found a dangerous issue on a physical they want the pt to come back for a second appt.

2. In private practice, If I found a breast mass on an exam, I could take the time to comfort the patient and get the steps of the workup scheduled. Can you imagine the emotional toll on a patient to be asked to make a separate appt to discuss the finding, and on the physician who could expect reprimands by employers if the visit time went over 15” ?

The good thing about American culture is it's practical. The bad thing about American culture is it's practical. If it can't be seen, quantified, and measured, it's seen as having less value. Procedures can be measured, so they are perceived as more valuable.

The surgeons controlled the AMA back in the day. They set the fee structure for Medicare. This became the fee structure for everyone else. While this was happening Peds was busy being leftist and money was evil, till they woke up and found they had become not only the lowest paid group, but WAY lower. Over the 1980-90s, the gap between the proceduralists and peds came to be vast. As in triple or more. This has contributed to the delta between peds and the even lower paid peds specialists as well as the delta between peds and IM specialists which is driving a shift away from peds. If you are interested in GI, and can make $700k in IM or $180k in Peds, it's not hard to see why the 2 year fellowship in IM GI is going to be more appealing. Same with hollow organ IM GI vs hepatology.

As an optometric technician who just got accepted into medical school -- the ophthalmology lobbying was hilarious. I totally agree that abusing contacts should at least be a misdemeanor, haha!

Thnx for the heads up about DB. I've had ocular rosacea and blepharitis for years and years and my annual eye exam is this month. It would be nice if I could get something better than what I am currently using.

Interesting to hear about peds being so low this year. I had heard about emergency last year. My husband will be matching this time next year and I am TERRIFIED. I just have nightmares about not matching, scrambling to find a program and ending up I have no idea where. To be fair he's going into family medicine but I'm still terrified. Also Lady G, I feel you on the away rotations! We have two children and I want to scream when I think about him being sent off for six weeks at a time. What the heck am I supposed to do with our two kids by myself!?

Imagine what it's like to be a single mom medical student! When my married male classmates would complain about how hard it was to be a parent and a medical student, we often had to do some deep breathing exercises and NOT point out the single parent women or married parent women whose husbands didn't know how to do a load of laundry and didn't grocery shop or prepare meals. One had a husband who was running a farm 100 miles away while the kids lived with her.

Endo here. There are two newer formulations of testosterone too, Xyosted which is a subq auto injector (easier to give, more stable levels) and Jatenzo which is a twice daily pill. I’ve had decent success in switching patients on IM cypionate who have excessive fluctuations to Xyosted. I’m not sure how well they’d work if your existing testosterone production is 0 however.

I never thought about men getting hot flashes from low T. Dr. G will likely be very sympathetic when Lady G goes through menopause. It's rough to have when still trying to function at work and elsewhere. That sleep deprivation, crankiness and discomfort is no joke, and most doctors blow off how bad it is when they can't relate. Glad Dr. G got a better treatment.

I work in a urology clinic in Washington state and all the physicians here recommend aveed for trt, not sure how it works with no testicles but it’s two shots a year and keeps a stable t level

Ugh, I hate when easily testable things get pushed off as "oh it's just stress" or some other bullshit excuse without at least running tests for common things with those symptoms. I had a doctor ignore increasing exhaustion at doing literally everything as psychosomatic and suggest I see a therapist. No exam, no tests, no questions, shortest appointment of my life. Like I went from swimming multiple times a week as a lifeguard, to getting tired walking from my bed to the bathroom (which is literally 7 steps). I had to ask a different doctor to do blood work and, lo and behold, my iron levels were 1% above blood transfusion levels. Psychosomatic my ass. He was replaced. And this isn't the only time something like this has happened to me. Apparently my "drama" needed surgery. That or I'm "self diagnosing" myself with a condition I've had for 7 years and been diagnosed with for a bit over 6 just because they've never heard of it. Yeah, can't imagine why a non neurologist has never heard of a neurologic condition that's fairly rare. Like, statistically speaking, there's more neurologists in my city than people with my condition.

Contact lenses are _great_ for playing sports. Glasses and sports don't mix. The glasses will get mashed into the face somewhere along the line and bend the frames. So glasses for regular daily wear and then switch over to contacts when playing sports.

Dr. Flanary, if you’re content with your current Testosterone replacement protocol then disregard this message, but I understand testosterone injections to be superior (for most patients) to the transdermal or pellet methods if done properly. If you select a longer lasting injectable ester, such as cypionate or enanthate and split your dose into at least two weekly injections you should feel no surge and dip in test levels. With half lives around 8 days your levels should remain quite stable after achieving steady state serum concentration. With smaller gauge needles (like insulin needles) these frequent shots can be done subcutaneous and relatively non-invasively. Additionally, you can more precisely modulate dosing to arrive at the proper, minimal effective dose unlike the pellet or gel methods that are dosed at standard increments. Plus it should be the most cost effective way. I’m just some random guy on the internet halfway through med school but if you aren’t 100% happy with your protocol maybe approach your urologist with some of these ideas.

@@bigdoze172 you asked if it’s more than 100K, a $30,000+ difference is a lot… especially when we’re talking 100 hour weeks for what is essentially $5 an hour. My husband’s intern year in BOSTON he was making $52,000. This was 3 years ago. Who in the world can survive in Boston on $52,000? Oh and btw Boston was his 16th choice so he definitely didn’t want to come here for residency. He ranked more affordable places higher on his list on purpose, because he knew he wouldn’t be making enough to survive in expensive places.

Sounds like you’re saying you should unionize? Based on inflation you should compare your percentages to the nurses and techs who get a 50cent raise per year. I really can’t sympathize with you guys. From my experience most doctors, student loans or not, started off way better off. And you guys make 300k per year at least probably . Ask a nurse if how they compare to your fight against inflation. Sorry i dont feel sorry for you 🤷🏻‍♂️

This is not third world problems. This is not ‘merica problems. This is top 10 percenter problems. Im editing this cuz i just got to the point of her saying non traditional students. Wow 🤦🏻‍♂️ so disconnected from normal peoples reality. Elitism mindset. Wow. Grow up in the shoes of one of your patients who grew up in an actual bad situation. This validates why doctors suck in my opinion. Imagine growing up without roots and a support system. You cant even try to imagine how good most of you hqve had life handed to you on a silver platter. Keep working hard 👍🏼🫡🤷🏻‍♂️
