I understand what you went through. I was 3+4 in 9/18 biopsy samples in 2 lesions, and one 3+3 sample in a 3rd lesion with 2 perineural invasions, and psa of 7.7. Also had urinary issues due to enlarged prostate. I’m 68, in excellent shape, and opted for the surgery. I spoke to a radiation dr. and a cyber knife dr. and opted for the surgery. In my 4th month now, and after 3 months my psa was .008. Only occasional, very minor stress incontinence, can have O’s but only semi-firm at present date. Sure, I still have second thoughts but come back to the same decision that this was the best route to eradicate the cancer rather than manage it. Over one year my mri went from one insignificant minor lesion to 3 visibile lesions with the perineural invasion. I had a PET scan and it confirmed 3 carcinomas in the prostate but no escapement outside the prostate. Post surgery pathology confirmed same. Cyber knife would have required 3 months of ADT hormone treatment, then 5 and possibly another 15 radiation treatments. With the perineural invasion threat I, along with my wife, did not want to risk metastasizing in that time since it grew significantly in the past year. You’ve beaten cancer and got it out of your body and can have that peace of mind. Of course, the undesirable side effects are a significant price to pay, but the reward is that your prostate, the one threatening cancer breeding ground in you, is gone and hopefully you can live the rest of your life sharing and caring for your loved ones. @@rightnow4450 : still working on minor stress incontinence, but not life altering, psa is undetectable after 10 mo’s. Semi to 3/4 stiff now. Able to have penetration. Hoping for more improvements with time. Thanks for the reply and concern. Best to you.

Here it is 12 Days Later and I have been fighting off a scrotum infection as well as some huge hemorrhoids which I've never had to deal with before and those are more painful than the surgery itself can't sit down hopefully those will go away and I can get on with healing but having some good luck with laying down and stuff sometimes when I jump up or something I have some incontinence issues so we'll see how it goes once these hemorrhoids clear up oh well it's always something right the universe is a comedian

@Aristotle was Not a fan of Plato well it's been two years since my surgery I don't have much problems with leakage a few drops here and there I still have some strange nerve issues in my groin and impotence I'm alive and finally back to work and my PSA it's still less than 0.01 good luck and request an MRI before you get the biopsies that way it gives them something to Target so they're not just shooting blind I did that and my surgeon told me it was really smart of me to ask for that

10 out of 12 for me 4+3 and some 3+4 and able and getting 6 weeks of radiation. had my first week already . also on adt plan with a loss of much T, but they needed to starve the cancer for a month before nuking. I'll look for an update to your case. I'd like to know how your doing.

Best wishes. Surgery for me is out my psa went to 230, Gleason 8 in biopsy but no perineural involvement. I am in month 5 of ADT and it has reduced my psa to 8.3 last test. On the other side the side effects are nasty and my mental health is in the crapper. No mood swings I am not anxious just profoundly depressed. All other side effects are roaring along. I am going to try some resistance training but I have two bad hips limiting it. I'd like to get my hips replaced before radiation to get some improvement in quality of life. Right now I have a kind of tiredness that's just tired of living. Also had two stents put in recently#6 and 7.

My husband diagnosed with same numbers as you this week. Focal perineural with diffusion. Idk. All Greek but same as yours. 4+3, 6 of 9 biopsies pos. PIRADS5. Left side.

Reading your responses to more recent comments, I am very glad for you that you are doing so well! My situation was very similar to yours. I was diagnosed three years ago, with 4+3=7 Gleason Score (no perineural invasion, though), PSA 9.2, with 90% cancer in all of the biopsy cores. I agree with you wholeheartedly that the biopsy should be an MRI fusion biopsy. I had had a blind in-office biopsy two years earlier, with all of the cores benign. As it turned out, my cancer was growing in a highly unusual location (at the apex of the prostate), and was initially missed. In any case, I'm actually glad that it worked out that way, because there was no mushiness in choosing treatment. I was relatively young at the time (62), in otherwise excellent health, and wanted to avoid the non-prostate related side effects of radiation down the road. I underwent a radical prostatectomy, and my PSA has been undetectable since. I am doing very well, and look forward to many more years of life!

I have a couple of newer videos that are only a few months old glad you are doing well definitely surgery seems to be very good if the cancer is contained in the prostate at least in my case

My PSA went from 4 to 9! Requested MRI. The results showed one spot . I visited my urologist he told me that the % of cancer in that area of the prostate is 15%. I had a biopsy non rectal Three hits 2x6 & 3+4 =7. Just had a radical prostatectomy with bilateral lymph node dissection done. Waiting for the pathology report tomorrow. Radiation was an option.

If you can avoid surgery don’t do it. I had surgery and regret it.

What is the deal man did you even watch the video It's five years ago I had surgery and I'm 5 years cancer free I'm sorry that you regret it I don't have an incontinence issue I'm healthy and strong and happy and I have revived my intimacy using trimix believe me I did not want to get surgery either and I went to three different doctors that specialize in different therapies they all told me to have the surgery and now it looks like that was the magic for me do I have some physical issues with nerve weirdness and stuff yeah but I'm alive and thriving. Look into some physical therapies and some exercises you can do I've been doing core exercises for over a year sit-ups crunches squats just strengthening everything that got pulled apart down there and things are definitely doing better

Hi, In the UK & Being looked after by the team at WEston Park Cancer Centre teachibg hosptial in Sheffield, England I was diagnosed with 4+3=7 Prostate Cnacer and after 6 months of Hormone treatment to kill off my testesterone I had five weeks of high dose radio theray which ended at the end of May. now on another 18 months of hormone treatment. Sugery was never and option as cancer was in the outer skin of the organ and would have spread. I am now having Acupuncture to help with extreme fatigue & hot seats. Onwards & upwards, Kev

Not looking forward to loosing my 2nd and 3rd most favorite part of my body (prostate and seminal vessels). But I don't want my guts being cooked by radiation like being i a microwave oven. I'm having surgery and hopefully can get the cancer out of me before it spreads.

@@sd906238 I'm not saying surgery is for everybody and I'm not saying that I don't have after effects and issues but I'm alive and its 5 years later and I don't have any cancer. definitely get several opinions multiple MRIs to Target those lesions. good luck

Thank you for this. I am Gleason 3+4 with a psa of only 4.8. My doctor is fantastic and made me have a multitude of tests before I had to decide on the treatment I wanted. I had an abdominal ultrasound scan, a CT scan to determine if there was anything else in the tissues, a radio scan for the bones, an MRI scan to target where the tumor was, urine flow tests to make sure that nothing was blocked and several blood tests. Then I had the biopsy under general anasthetic. With this wealth of information I then had a joint meeting with the radiation doctor and the surgeon who each went through every step of their proposed treatment outlining the benefits and side effects. In the end I have decided on the operation next week as all the doctors agree that at my age (68) and level of fitness (very fit) with no signs of any cancer outside the prostate i will have a better chance of a full recovery with the option of radiation at a later date if other cancer appears.

@@douglasf6109 yes things are quite good I'm in very good shape been working out I've been on a carnivore diet for a long time and I am at the weight I was when I was 17 in November I get my 5-year PSA test the last four years have been clear

Steve. It Worked out well as far as getting all the cancer and incontinence...not so well on sexual function.. but I am very healthy and 155 lbs taking karate and working 30 to 40 hrs a week. Looking into trimix as viagra not so good I will make a video if it helps after November 2023

Maybe I should say it's good to hear that you are in a better place psychologically. I just learned about my prostate cancer. I'm going to try to get, I'm going to get, a bone scan to see if it has spread. I'm worried. The process seems to be a hurry up and wait thing what with getting orders and awaiting insurance approvals. I've told a couple of close friends about this, but they don't seem interested in helping me with something as simple as talking about it. So, thank you so very much for the communication. I saw a radiation oncologist yesterday. He was positive. He spelled out the options including prostatectomy. I didn't think much about the sexual function aspect . Husband psa 3 urologist found lump mri showed 6 lesions 1 large and 5 small mri aggressive 5 took 16 biopsi samples cancer in all Gleason score 4+3=7 bone scan negative lesions seems to be local he decided to do total prostactomy seminal vessels and lymph nodes robotic laparoscopy surgery done April 3 rd waiting on results Dr said everything looked good fingers crossed husband 72 years old did not want radiation

Hi everyone update after April 3 total prostectomy. 3 month visit and blood test 0 psa recital and surrounding ultrasound negative for cancer . Still has urinary leakage when he picks up babies he is training himself still . When lifting or using force he does his kegel of holding is urinary flow we expected this dr says doing the levels is the most important thing he is 73 no cancer

@@rightnow4450 so glad you are doing well bladder control excercises And remembering to tighten bladder when picking up great grandbabies getting up on tractors tighten and hold kegel excercises the depends are running about 50.00 a month he has tried pads but states they are rough on his penis have you tried comfortable pads that are not abrasive ?

Same diagnosis a week ago only 18 cores 9 of which were cancer..urologist wanted to cut and like u was going in that direction then did some due diligence and cancelled the op..now looking at radiation..either HDR brachytherapy or Calypso treatment..it's a journey that's for sure...good luck with the hems!

I had the surgery 4 years ago this December I just had my PSA test fourth time since my operation and still less than 0.01 it took me 18 months to recover enough to get back to work and now four years out I'm in really good shape and don't have any urinary problems some other problems but learn to live with them super grateful to be alive and to have had the surgeon that I did

I think it’s your age that made them insist on surgery. I’m in almost the same situation as you, except fewer core malignancies. But I’m 71, and they are not pushing surgery, which I didn’t want anyway. Do you watch Dr. Scholz’s weekly clips on prostate cancer? He seems to think that with modern technology surgery becomes less and less necessary or desirable, and creates the worst side effects. Every case is different, of course. (He also says that perineural invasion is not indicative of potential metastasis with prostate cancer as it is in other cancers, and should essentially be ignored.) Hope you’re doing better.

Prostate cancer is curable. With modern radiation technology you have options. I was recently diagnosed with 4+4 in 3 out of 12 biopsies. I am 66 years old. It’s devastating to find out. I keep my faith and believe that your desire to live will help you fight and cure yourself from this evil.

Good luck now three plus years out coming up on four years in December I'm doing really well still cancer-free I've gotten over my issues mostly I don't have hardly any leakage I do wear a pad from time to time when I'm at work or if I'm at karate which I am now doing with my son I've lost 40 lbs since the surgery feeling better and better everyday I got off of my high blood pressure medicine using cannabis to treat that don't recommend that but anecdotally it works for me good luck on your journey I'm glad that my video helped you some

Did they do psma-pet scan. The psma-pet scan is the best scan showing were the cancer is. I'm also getting 20 samples pulled in Jan 7. I would talk and think able cyberknife radiation, but maybe because it's outside the prostate. That they want the surgery. Just me.. I'm going to go with HDR brachytherapy having the same results as surgery. It's to bad cyberknife is not as well being done as a treatment. It's a newer tool and not enough doctors or hospitals are doing it. Because of not having the experience or cost of the equipment. I'm in the VA health care system. So, I'm maybe limited to my choices. My doctor was surprised on how much information on prostate cancer I got watching videos on youtube. I pretty much told him no surgery for me. I'm going with HDR brachytherapy. When and if it's needed. Most likely they won't get all of prostate and secondary operation will be needed or radiation. My father had his prostate removed and they had to do a second surgery to get. What was left behind. I lived a good 15 years after all of it was done. It was his heart that he die from and not cancer. Please start working out at a gym and I don't just mean just walking or the treadmill machines. Work out on the upper parts of your body and reduce your weight. Heart disease may kill you before the prostate cancer does. The gym can prolong your life and be your best friend. I go to the five days a week and I'm 72 years old. The doctors are surprised of the shape my body is in and take a second look at my age when they see it on the charts. I give working out at the gym for energy I have now.

Less than 0.01 on psa for 20 months. Have some nerve issues in groin and testicle and some leakage when doing some things . Happy to be alive😁

​@@rightnow4450man hope your doing better. High volume is not good for radiation. Surprised you have urinary issues with such a small prostrate. Today options are a bit better. Cyber knife because of your small prostate. And there's a newer Robotic Surgery that is suppose to be 80 percent continence rate day 1. Of course finding these procedures scare me. Brachy therapy and Cyperknife. Do sound attractive. Radiation scare me. Getting permanent cystitis is a concern. Although rare. But you did your due diligence. Maybey if you would have had yearly psa starting at 45 they would have got it earlier. I stopped statins for a year and my psa jumped to a 7. Now I'm beating my self up along with depression. On to the journey. Mri tomorrow. Have lesions already 3 years ago that were negative with a perineal biopsy. Never get a Trans rectal. But yes all.surgeons want to do Surgery.

When they removed the prostate, did they send it off to the lab to check for how much and what type of cancer it was? You said that they were going to do this.

They did and I'm pretty sure I made a video but I don't remember. My gleason score was 7. 4+3 adenocarcinoma all biopsies 19 of them were 4+3 and the lab confirmed 11mm×3 tumor and 3mm×4 tumor prostate was normal size not enlarged

Here's a link to the last video I made several months ago at 13 months post-surgery incontinence is doing really good except I have trouble sitting in hard chairs when I get up sometimes I have some leaking my PSA tests have all been less than 0.01 which is considered undetectable still have some pain and some swelling issues where my lymph nodes were removed and where I had those infections and have some pain around some of my incision I'm getting stronger I just got set back several months because of those infections I'm going to make a video very soon just been very busy with school-age kids dealing with school closings and re openings and then re closings.2020 was and 2021 is rough. Bur i am upbeat and happy to be breathing

Not sure what you're talkin about I had 19 biopsies done before my surgery 18 we're positive they were all scored and graded after my surgery they sent my prostate to the lab to be dissected and it was confirmed Gleason 7 4 + 3

That's what biopsies are for to get your Gleason score your question is quite confusing

I'm doing real good working full-time. Still, what on my PSA test at 4 years. I've lost weight I do ice cold showers every morning and exercise have meditation program taking methylated vitamins to increase my digestive capacity and increase oxygen to my body. Love and life. Still having issues with fatigue. But hey i'm 60 and feel like I did when I was in my thirties

I have the same Gleason score and I will not do surgery, perhapes radio. In God l trust and nothing will happen without His will. "God is our refuge and strength, a very present help in trouble." I will prayer for your healing, your salvation and peace in your heart and your family too.
