I wrote earlier, but after more thought I have to say that it was a great observation of some things we mortals do in a game. There's the "I had a plan, but then ...", the "I played this and then realized ... (...c4)" and "I equalized, so I thought it was my turn to play offense", etc. Most any good player will have done these common things and recognizes it. What to do about it? Learn. We aren't always going to see all the moves. Top GMs do it better. The observers and even the GM coach didn't see ...c4. The range of plans was challenging because they were quite different. The idea of playing offense was perhaps most out of line since the Black king was NEVER in any danger. Personally, I'd have not played the pawns that way, but I can see your points (backed up by the engine). I think the net is that an incorrect view of Black's king safety shaped a lot of the decisions. Just keeping the good Bb2 to build on was a good option, whereas in a blitz game (common territory for the development of many of our ideas) the immediate damage at f6 had to be done. History shows its value. The oddity that is b3-b4 (at several times) relates to control of space, and clearly that wasn't high on Levy's list of things to do. But, that would have helped restrain Black a lot.

I feel there was un underestimation of the threat from the queen side for black, because of a focus on attack on the kings side by Levi. The clue that there was a threat is when Djurabek allowed the disruption of his defence. He might have also felt that Levy is not the best at changing tack, and might be blindsided.
