I never expected to find videos on gout that were actually entertaining as well as informative, bravo! I suffer from gout and find your videos extremely informative, clear and useful. I won't pretend to understand everything but I feel I have a much better understanding of why I suffer from gout and how best to tackle it. So much more helpful than simply being handed a list of high purine foods to avoid! Thank you so much

I think! Allopurinol and Febuxostat used to decreases production of uric acid not increases excretion

Truly enjoy touching on your channel from time to time, although I am retired now. Many patients eating low carb high fat (or even carnivore) diets report cessation of gout symptoms. Is it possible that the other mentioned risk factors for gout (soft drinks, hyperglycemia, alcohol, high triglycerides) are the most important risk factors? Thank you.

@@MedicosisPerfectionalis Thank you. One paper that expresses some congruency with this notion is "Hyperuricemia and Insulin Resistance" DOI: 10.1210/jcem.78.1.8288709 I have no academic or clinical research on red meat, only anecdotes.

@@2almar2 what are you noticing now? The trend seems to have been moved to meats and fruits, seen any flare ups? My anecdote: SAD - acute gout flare up and elevated uric acid Keto - no gout same uric acid 1yr Carnivore, high purines - no gout, lower uric acid (wtf?) 8mo High carb tubers and fruits, less meat- just had a gout flare up. No uric acid measurement. So my n of 1 is not correlating with this information. They said 20% of uric acid is added by diet though so my issue may not be food related.

@@shawnmclean7707 Hi Shawn. I’ve been fairly consistent with my low carb, mostly protein diet. I don’t measure uric acid. A few months ago my cholesterol panel was as one might expect: normal HDL, high LDL, super low triglycerides, super low CRP. I almost never eat green vegetable because of the oxalates. I’ve not been bothered with kidney stones for about seven years (since I went keto). But, when I fall off the low carb wagon I usually fall pretty hard. Only for a day or two, but the effects are nearly immediate: muscle cramps from hell, headache, and later, nausea. And yet I continue….wtf anyway. Why do it? I believe it is a sugar addiction.

We usually don’t use them in acute attacks! When the patient has acute pain, we manage the pain!

One cheap and effective remedy for gout is Potassium Citrate. Very strange its not mentioned as therapy

I am glad it worked for you. But the sample size of the study was just 8! Moreover, they gave potassium citrate to all of them, and they didn’t divide the patients into 2 groups, so we have no idea what would have happened to them had they not received the potassium citrate. The data is not reliable enough to reach any meaningful conclusion. Thank you so much for your comment! Stay healthy!

Agree. I think it is reasonable to conclude that if you increase renal excretion of UA significantly that you are going to reverse Hyperuricemia and in turn Gout. It took me 14 weeks, but I was able to correct my non gouty hyperuricemia by alkalizing my Urine to an average pH of 6.75 using 80 milliequivalents/day of alkalizing agent (and supplementing VitC a uricosuric and Quercetin an XOI). There are studies out of Japan showing the relationship between urine pH and UA excretion. My personal experience was that I increased my renal excretion of UA from 500mg/day to 1,000mg/day.
