I believe the ones with the horn helmets are the "heavies". If you do Hoover Dam near max level, the ones that show up all have good weapons and those helmets and tend to have considerably more health than the khans you meet at their headquarters.

What is the level range you use for some of these battles? I usually hit Black Mountain mid-game or later and is pretty much exclusively brutes and masters by then with miniguns, super sledges, and high damage weaponry. There also seems to be more as when I go, there's usually around 4-5 super mutants near the crater area alone. I might have check it out early on the next run for a Raul companion run.

They seem to skip almost half the mutants by taking the booby trapped shortcut. Better pathing than me when clearing Black Mountain! I only use that shortcut to rush New Vegas Strip at the start of the game

You should make more unique scenarios now. Try using areas that make firefights more interesting even if the combatants arent native to the area. Maybe try some randomized faction distributions. Or create a faction tournament?
I recomend using mods like military expansion program, more ncr population, or the factions reloaded. Some of these locations can be built up with force in a lore friendly way and then you send troops at em. For example the fiends against camp macaran but with the military expansion mod, more ncr rangers mod, and more ncr population mod. And voila
Put all the legendary, bosses, high tier enemies and monster together including machine monsters and biological monsters like legendary bloat fly vs Burn man and other bosses in the game
