Physical assessment doesnt include checking "down there"?? Here where I am from it is standard, not a full internal "gynecological exam", but I always get mine spread and examined visually.

They were probably told to pick a complaint to go with the exam, which was what they were being graded on. They do that because in real life patients will come in for a basic exam but have something that came up they need to be seen for. Also, many things can be the cause of certain problems so yes, always a head to toe to see if anything else is going on.
