I had robotic surgery to remove my prostate. I’m 75 but run and do a lot of body weight exercises. Surgery went well but PSA crept up to .5 in about 18 months. I had a PSMA pet scan and only thing that was found was a lymph node with a small uptake of the nuclear material just south of the gallbladder. With that I had 5 days of SBRT and now after two PSA test it has dropped from .5 to .28. It is going in the right direction. Another PSA is scheduled for July 19th. So far no hormone therapy. Still exercising and eating very healthy. Praying for a good outcome. I have to stay healthy to see my granddaughter to great things with her life.

I contacted your help desk and the person who responded said I needed to get a PCMA scan done before anything else. I got the PCMA procedure approved by going to another provider. In fact they had me see the director of the university’s entire oncology department. The PCMA scan came back with results showing only a small amount of cancer in the lymph nodes around the prostrate. No other cancer was detected other than in a small lesion on my hip. The plan is to come up with a treatment plan that is tailored just for me. Darolutamide will be given by itself to see if my PSA and testosterone will drop and can be kept under control with the one drug. A first generation pill will be added if needed. No more injections so everything will be given in pill form so if the side effects become too severe I can discontinue the treatment right away. I will start with just Darolutamide on September 10th. Thanks so much for your concern and input.

I had a Gleason Scale 9 back in 2015. Doctor said "you have aggressive prostrate cancer , and we need to do something about it". The surgeon did robotic prostrate removal and I then had 37 radiation treatments. I'm doing fine. I'm alive and healthy. I take daily hormone blockers to keep the testosterone at zero. I'm certainly not as robust as I was with my normal testosterone levels but Life goes on and I am here. I suppose there are many options for this cancer treatment but mine has worked well......

good reflection because I have just had surgery (September 2022) scale 4+5 72 years and don't have to do IMRT with hormone. good continuation for you How long IS the Time for hormone ?

I am 69. I had 25 proton treatments at the Maryland Proton Center in Baltimore 3 years ago, followed by brachiotheropy one month later. They followed this with 18 months of Lupron, ending about 18 months ago. My doctor has been monitoring my psa, which has been

I would like to see a study conducted between doctor-recommended treatment and determining treatment by throwing bones on the ground and reading them. I suspect there would be no difference in outcomes...

Everyone needs to get up to date in 2024. Talking about what was acceptable in 2000 or 2010 or 2020 is unacceptable. Unfortunately most men with prostate cancer are clueless, until after the damage is done. Prostate cancer business is big money.

Very informative video👍.. I just completed 28 sessions of EBRT last 5 Aug 2022( Monday thru Friday) and 4 injections of FIRMAGON( hormone therapy) initial dosage of 240 mg and 80 mg injections for three months. Hormone therapy completed 5 Aug 2022. My concern is , all these treatments worked out? I’m worried, my next appointment with my radiology oncologist is Dec 2022. My PSA before treatment was 9.8 and my biopsy( 12 cores) resulted in Gleason scores of 3+4( 2 cores) and 3+3(10 cores.

I was on Lupron Depot injections for 2 years, honestly wish I just had 18 months only.

I wanna thank you both for all the info.i got diagnosed in 2015 but didnt do anything for 5 yrs it locally spread then i got radiation at loma linda ca. Ive been on lupron for 3 yrs now. I dont know whats next but the psa has been like o.3 so far. Thank u for all the good info.the lupron drains me but well see what happens

My father found single metastatic at right pubic bone region on june 2021 PSMA PET CT. After that he ungergoes SBRT immediately and two ADT at three months interval. Now recent PSA pathology reports show incrrase in PSA level 0.05 to 0.17 . Our question is if we found metastatic at same location can we go for sbrt for same place as a curity treatment? What are the alternative if we can not go for SBRT at same place again.

If the second opinion on my prostate biopsy results were done at Johns Hopkins and is quite different (a 4+3 changed to a 3+4) from the original pathology report (USC), which one should I choose to go with?

Hello Alex, thank you for everything you and Dr. Scholz do. I am the 66 year old patient you asked a question about in this video and now I have another question because I was able to get a PSMA Pet scan of my high risk prostate cancer. My Pet scan came back showing no spread outside the prostate so now I am proceeding with the proton beginning today and this I believe is a important question you can ask Dr Scholz for me please. My biopsy came back showing 10 of 12 cores positive and as I said one Gleason 8 and one Gleason 9 and the others 6 and 7’s . Now I am getting my radiation treatments and I asked the radiation oncologist if the lymph nodes and seminal vesicles are going to be treated since I have a clear PSMA scan showing no spread outside the prostate and he said yes they would be and I am wondering if this is the correct course of action or not ? I know my prostate is covered pretty much with cancer and high risk in some areas but I thought getting the PSMA scan would help cut down on some treatments but maybe this is a case of better to be safe than sorry. I just thought maybe better not to treat the lymph nodes and seminal vesicles but he said they would be partly in the radiation field anyway. Could you please ask this question for me and thank you very much.

Hello, I will add these questions to our list for future videos, but in the meantime, we have a free helpline that can help with your questions. You can find the contact information at pcri.org/helpline. For the first major question, the PSMA PET scan, if it is negative, can inform physicians that the risk of there being spread outside the gland is lower than if the scan had not been performed; however, there is still the risk of microscopic disease, even with a negative scan. In the case of Gleason 9 prostate cancer, even with that negative scan, most oncologists would still consider the risk of microscopic disease to be significant enough to warrant adjuvant therapy like hormone therapy and/or radiation to the lymph nodes. Unfortunately, it is a game of odds; it is possible that you would be cured without that adjuvant therapy, but they consider the odds of relapse high enough that adjuvant therapy, even without having concrete evidence of spread, will improve the odds of a cure and be worth the potential side effects. And regarding the second question, I am not sure about side effects of radiation to the seminal vesicles specifically, but Dr. Scholz has said before that the side effects of radiation to the lymph nodes are insignificant (maybe some temporary fatigue) unless some lymph nodes have already been removed via surgery, in which case there is risk of lymphedema. Our helpline would definitely be more informed on radiation side effects, so feel free to get in touch!

Tim, I had a very similar situation 4 years ago when I was 65. They put me on testosterone blocking meds for a month, followed by Lupron shots before I had 25 treatments of Proton. After a month break, the doctors implanted 19 seed of brachiotheropy, which caused some issues, but I made it through it. Since my Lupron shots have stopped 2 years ago, my PSA has remained

How do I find treatment? - other than surgery. So far I am being pushed into surgery by two institutions in Northern California - even though the biopsy has a case note in caps stating surgery is not likely to be successful > Gleason 8, 5 PIRADs, PSMA not metastatic, locally advanced ECE/SVI, inter ductal. ? > Can I get Proton therapy with ADT?

sir my uncle have detected with prostate cancer in dec 2021 having oligo metastatic stage in prosted , pelvic lymph nods and single 12mm spot in pelvic bone 4+5=9 glison score psa = 3.11 before ADT age = 64 year we have started ADT from January 2022 firmagon 80mg /month zelgor 250 (abiraterone) 4 tablet/day after 90 days of ADT we have done psma pet scan again and found good results, the size of tumor reduced in all three sides and PSA 0.10 now. we are continuing our ADT and our doctor have suggested to continue ADT only and have active surveillance. Doctor sir is advising we will go for radiation (igrt) latter, since the ADT is responding well. SIR MY REQUEST TO YOU IS PLEASE SUGGEST SHOULD WE GO FOR RADIATION IMMEDIATELY SINCE IT IS HIGH RISK STAGE OR WE SHOULD WAIT AND DO ACTIVE SURVEILLANCE WITH CONTINUED Androgen deprivation therapy. PLEASE HELP

I am 29 I have metastatic Prostate cancer. Gleason 8 PSA 200. I really don’t want to have surgery, currently in aggressive treatment. I want to live but at the same time I would like to start a family. Not sure what my realistic survival rate is(my doctor is optimistic), so I am not sure if I should get surgery if my doctor advices it after treatment. There isn’t much info on survivors that are near my age. I know 29 is already crazy, but I just don’t know what to think.

Jaime, it will give you extra peace of mind if you can maximise your options with treatment. That could be involve cryonic sperm storage for instance . Are you near a Center of Excellence to assist your decision making.

Wilson, Tuve el mismo numero (9) hace 6 anos. El Medico saco el prostrate con robotic surgery. Tambien hubo 37 tratamientos de radiation. Me dieron pastillas para blockear el testosterone. Estoy con vida y me siento bien. Hay pocos problemas con el fuerte que tenia--porque estoy mas debil (con las pastillas.) Sospecho que Usted puede salir bien si sigues las instructiones de su Medico. Espero que le he ayudado un poco con esta carta que hice. Yo vi que usted quiera mas information de su enfermedad. No hablo muy bien la lengua suya.

I had a biopsy come back Gleason score 7 in one core out of 15. I took your advice and sent the slides to John Hopkins they said I had Gleason 6 my radiation doc wants to watch it my oncologist wants to put me on Casodex and my urologist wants to do surgery what is the best course of action

Sadly after biopsy prostate and Colon wall were intertwined. Due to having an infection, which despite my see the urologist three times in one month before I was given antibiotics!

@@RH-xd3nx my professor surgeon asked me if I had been in a crash, after prostate removal. And predicted I would need further surgery. One week later I had to have life saving surgery and further surgery every three days for two weeks. Recovery is slow!
