The upper trapezius muscle is an essential stabilizer of the scapula as well as cervical spine. It is greatly involved in proper scapular mechanics, especially upwards rotation and posterior tilting, making it an important player in amelioration of glenohumeral disorders such as impingement. When it is weak, usually due to poor posture and scapular dyskinesis, it may produce cervicogenic headaches and neck pain.

Crazy that many "experts" say not work this muscle out for neck pain bc it's "overactive" . This could be solution to my neck/shoulder/thoracic pain of several years.

is it possible I'm getting pain overhead pressing because of weak upper traps? I notice when I try emphasise shrugging when doing dumbell OHP, my shoulder feels better. otherwise it feels like my humerus is struggling to be stabilised. of course, this is just a feeling and something else may be going on.

Some teach this with arms abducted 30-40 degrees as they say it lines up with the fibers of the upper trap and the scapulae are slightly upwardly rotated. Is there an added benefit of doing it that way?

Would you recommend training the upper traps heavy as part of a strengthening routine ( e.g BB/DB shrugs) alongside training them the way you are doing here with the resistance band?

what if shrugging with 30 degree abduction make scapula cracking? does it mean serratus anterior is too weak? *UPDATE : After practising the movement seems it getting better, I think my scapula was downly rotated asf, and had problem to create any upward rotation. The great thing is after few reps my hands start to feel warm, like having better blood circulation

Hi Kjetil. I'm hinging at my cervical spine, and I'm working now to be long through the neck. But when I hing, my head drops to the right, and I have weak UT, SA, rhomboids etc. on the right side of body. When I pull my chin down, everything comes in place and muscles start to be active. I'm curious to know why is this problem unilateral, or it is a normal thing in those cervical hinge cases? Best regards!

I think I might cry, I've always noticed my winging going away by just putting my shoulders up and yeah, everyone kept telling me to put my shoulders back and down and pinch my shoulder blades together. I never ever had any improvement. I'm going to try to follow your suggestions. If I get there I'll make you (literally, I'm a sculptor) a statue

​@@user-bt5ur1pr7ryes, my shoulders are up by like 7 cm, I can see it from my collarbones. I can have my scapula stick flat on the ribcage but when I raise my arms still winging. I set up a videocall from my smartphone to my PC so I can use it as instant feedback and learning proper scapula movements. But yes, in resting position my scapular are not winging anymore

My shoulders are up by 7 cm or so, I can see it from the collarbone that now in going up from it's previous depressed position. Both my scapular are resting flat on my ribcage now in a resting position, but they still winging while I move my arms. So I set up a videocall from my smartphone to my PC so I can get a good look at it and I'm teaching myself the correct pattern of movement. It will take some time but I'll get there

@@user-bt5ur1pr7r ok it's the 3rd time I write an answer, hope this time it will show. So, yes. My shoulders are up by 7 cm or so, I can see it from the inclination of the collarbone and it's extremely visible in the before/after pictures. In resting position my scaps are sticking to the ribcage as they should, but during movement they still winging. So I set up a videocall between my smartphone and pc so I can check myself in the big monitor while I move and correct myself. I guess it will take some time, but I'll get there.

If you're SURE that you're not, then you're just having some co-affection of the cervical plexus. Harmless. Strengthen SCM, LS and trap.

my left shoulder often hurts and is noticeably tighter and more "swollen" looking while my right shoulder seems completely fine. any advices or exercises that can help w my muscle imbalance? I've been struggling for almost 2 years now, would be great if u replied! thanks :)

You have another video explaining this exercise. In said video it mentions to pull the shoulder cranially (toward the ear), similar to the levator scapular exercise while in this video you say to elevate the shoulders vertically. Which one is the best way to strenghten the upper trapezius or does it not matter?

I have shoulder impingement (right side) whenever I drive for long hours my right trapezius burns and pains. And when I touch the area it hurts with a tingling sensation. My doctor has advised me not to lift anything heavy and not to do any overhead activity. It’s been 3 months now. But I need help.

Hi Kjetil, Are side lateral raises bad if suffering from TOS ? In other words, is there a safe way (isometrics?) to continue working out the side delts without worsening Tos or is it better to totally stop side delts and shoulder's training ? DISCLAIMER: I'm not a medical professional, just a kinesiology undergraduate applying for pt school. I will say that side lateral raises are not really a good idea because of the long lever arm of the exercise. If you have TOS, you probably have a good amount of problems with shoulder girdle stability, so when you do a front raise or side raise exercise, your upper traps and levator scapulae have to work extra hard to fight gravity when your shoulder has a high amount of mechanical disadvantage and in your case, the trap is extremely weak. This will increase compression on the neck and shoulder complex, and will probably worsen your condition. You might be able to get away with side raises if you use a very light resistance band and do long isometrics or 1 and 1/2 reps, but I would stay on the safe side and just cut them out until your TOS improves.

Do fewer reps. If it hurts, that means it's extremely weak.

Hi Kjetil, I appreciate your videos a lot! I have a question about pain in the trapezius: I have frequent pressure points in my trapezius close to where it attaches to the scarpular at the acromion. Should I strengthen it, stretch it, do both ore something completely different. Some context: I climb a lot, train for it (pull-ups, TRX, etc.) and work in the climbing gym as a route setter, lots of physical activity. Last time I the pressure points got triggered a lot was after wearing a heavy backpack for 15 min. Looking forward to you answer. Kind Regards, Frank

I have lots of trigger points in upper trps because I tense then under stress. So everyone tells me to not strengthen them abd activate them for every movement I do. Yet I suffer from cerviogenic headaches especially in suboccipital area and lev scap. Will thus help me or make it worse?

I did all your neck exercises two day sin a row, just ten reps, but got more of my "already tense' occipital pain which makes me dizzy. so I stopped. I want to continue on, but maybe slower paced? Would you suggest upper trap exercise for someone with bilateral neurogenic TOS? I had surgery on one side (scalenes removed, pec minor release - still have first rib) still experiencing symptoms 17 months after surgery.

@@to564bnyt the pain was intense afterwards, they kept me overnight but only gave me 5 hydrocodones which will not be enough. I also had a partial lung collapse during the procedure and had to have a tube in my chest for a week afterwards. I did do physical therapy afterwards for range of motion and strengthing and the one thing that did improve greatly was my grip strength. The problem is my hand and arm are still desensitized and "heavy" feeling, which was the ultimate issue i wanted resolved. I seriously would try to exhuast all other options before the surgery. My arm feels kinda dead now but thats just my experience.

Not unlikely as it anastomoses with the brachial plexus. Whether it can cause frank palsy, I don't know. But irritation leading to hiccups and similar, definitely.

What if upper traps seem tight? Whenever I do shoulder workouts especially the dumbbell flies my back of neck muscles gets really tense like it squeezes, seems like upper fibres. Mine seems really large and high as well even though I dont workout on them.

It's more likely due to upper trap hypertonicity because your traps are trying to stabilize the shoulder. The problem with dumbbell flies, front raises, and lateral raises is the long lever arm of the resistance. If your shoulders are downwardly rotated/depressed and you're trying to do those exercises, the upper traps and levator scapula have to work extra hard against gravity to stabilize the shoulder since the force couples of other muscles around the shoulder girdle are impaired (serratus anterior dysfunction, mid/lower trap dysfunction, pec minor overactivity, latissimus dorsi overactivity, rotator cuff inhibition, etc.). If you shorten the lever arm (such as bending your elbows more or focusing on multiplanar shoulder pressing motions), the stress on the upper traps and levator scapulae will be much lower. So, in my opinion, I'd say stop using dumbbells for these exercises temporarily and use lighter resistance bands like Dr. Larsen is. NOTE: I am not a medical professional. I am an undergraduate kinesiology student preparing for PT school, but I've spent a few years learning and implementing this knowledge on my own and found improvements by eliminating contraindicated exercises (like the ones mentioned above) and implementing exercises that help improve shoulder girdle function.

The traps SHOULD stabilize the shoulder, they SHOULD be VERY STRONG. The notion that trapezius is overactive is a harmful myth that must be put to death.

After starting serratus anterior and trapezius strengthening after a long thoracic nerve injury I am having good results, but I would like to start strengthening my shoulder soon and I would like to know which order should I rebuild the shoulder muscles for my recovery, which should I strengthen first in which order, the front delts, side delts or rear delts?

What kind of serratus anterior exercises did you do? I've been doing a foam roller on the wall with no added weight at the moment. Did you do something similar and did you add weights to this exercise? Thanks.

@@naturallyraw hi, this is what i found helpful for me. I tried so many different exercises in the past and worked with so many different physios and all of them caused problems, but after searching through studies with the help of an AI chat to find out which exercises showed the most serratus activation, I found the two most beneficial exerices were, push up plus against a wall, makes sure arms are kept straight, for some reason push up plus tutorials on RU-vid sometimes show it bending arms which is the wrong method, after this your suppose to advance to push up plus on knees then press up position but I recommend not to yet and instead move onto serratus dumbell punch laying on back first, start with extremely small weight and work upwards, and then as weights get heavier eventually add shrugs with weights into your routine aswell. It seems as if moving onto push up plus knees and press up position engages too many other muscles which could be weak or troublesome, that's why I found the serratus punch more beneficial as it really isolates that serratus muscle.

Just got huge doms in piriformis after doing first time the hip abductor machine found usually in normal gyms and mistakenly used for training gluteus medius/minimus and some area of gluteus maximus. The hip flexion angle was around 70 degrees, so the pull was quite from the origo to the insertion of the piriformis Having done squats, deadlifts, split squats, and all sorts of free weight movements and skipping machines for years, I have been trying to test the machines what they really do. Yes, some of them really train other muscles they claim to train. But you have benefits if you know what do and if you really need specific target. I had too much weight and I little overworked the muscle. I palpated after a day also quadratus femoris and obturator externus, and they were also trained. Not too much doms but some still. It is logical. You somewhat use gluteus medius in the machine, but it also causes internal rotation in those flexion angles so you need your deep rotators and some fibers of gluteus maximus as fixators. I am considering doing around one month progression on this machine. Not too much weight but doing with exaggerated lateral rotation with abduction. The goal is to target deep lateral rotators and piriformis, not TFL. There is lot of reasons to have more strength in those muscles, so I am giving a try. As a sport massage therapist (got my licence some months ago) I have many times that some people, usually young women have constantly overworked piriformis. I think it is because of this good girl/bad girl machine they use trying to train lateral glute area. The machine should be called "Piriformis- TFL-machine" if you use it same way people usually use it.
