How can you tell if your meniscus repair didn't heal?

Most orthopaedic surgeries have good success rates, and patients return to their sport or exercise. That fact doesn’t mean that every surgery will result in full healing without a setback. In this Ask Dr. Geier video, I discuss how some surgeries fail for one of the most common injuries in sports medicine.

Tim in Alberta asks:
I had a meniscus repair surgery done January 27 of this year. The knee will not extend. It becomes locked at a certain point. They put me under and tried to straighten it, but it failed. I am just wondering if this guy botched my meniscus repair. I still can’t extend the knee fully or bend it back.
Meniscus tears of the knee can affect active people from adolescents to older adults. Surgical options include trimming out the torn portion of the Athlete holding kneemeniscus and actually sewing the tear back together. For the ones that can be repaired, we know that a decent percentage don’t heal. In this video, I explain why some meniscus repairs don’t heal despite proper surgical technique and rehab.
Please remember, while I appreciate your questions, I cannot and will not offer specific medical advice by email, online, on my show, or in the comments at the end of these posts. My responses are meant to provide general medical information and education. Please consult your physician or health care provider for your specific medical concerns.

Dr. David Geier I’m currently 8 weeks out from my meniscus repair surgery. I had a complete radial tear of my lateral meniscus. My surgeon went in and discovered I had a discoid meniscus and sewed back the separated pieces. I am currently experiencing some acute pain only in my meniscus but not experiencing any restriction of motion. I also did not experience restriction of motion before surgery though. I was wondering how I could tell if it was healing or what kind of chances I may have of healing.

For those who had mensicus surgery, PRIOITIZE FLEXABILTIY AND LEG MUSCLE BUILDING. working and building the leg muscles will dramtically cut down on any pain and aching. Will reduce the chance of re-tearing, and any pain.

I am almost 6 months into my recovery from surgical repair to a posterior medial root tear of my meniscus. I debated whether to have surgery or not, but the 80-85% success rate seemed worth the risk, and as an active late-50's person, did not want to have a total knee replacement yet, though probably will have to at some point. My surgeon used the trans-tibial tunnel technique, and at the site of the repair injected a glob of treated bone marrow aspirated from my hip bone to help facilitate healing. I was non-weight bearing for 6 weeks, then began partial weight-bearing while wearing an "unloader" brace on my knee. I began PT in the 2nd week of recovery, and have stayed consistent with exercises. My PT had me in a swimming pool as soon as it was safe. The buoyancy helped me learn how to bear weight again. I have now progressed to full range of motion exercises, but I am careful with squats. I am on a return to running program, have minimal swelling and only occasional joint line pain. I will have MRI at six months to confirm status of repair.

I am 4 months post op. I’m 45 and still young. Skateboarding and snowboarding as a kid did a lot of damage to my knees. Started skating 4 years ago and last winter torn meniscus. Was laid off a week prior couldn’t walk for months. Had the surgery to take out a small piece of meniscus. Recovery is slow but I’ve seen a lot of progress. I’m far from 100% still. I have a feeling a year is really the healing time. Sure doctors will tell you can return to normal a few weeks after surgery. They don’t tell you yes you return to normal in 2 months but it’s painful and stiff normal. Do your research because to get back to normal is most likely 8 months to a year. This depends on your age. If you’re in your 40s late 30s you don’t heal being at that age you don’t have blood flow threw the meniscus. If you are younger than 30 you most likely will heal on your own without surgery because you still have that blood flow through the meniscus. Good luck folks. It’s a different world when you are temporarily disabled from this common injury. It’s been a fight to find work, seems like everyone wants to let me go with this injury! Best wishes to those looking into this procedure! It’s painful!

I tore my meniscus 3 1/2 years ago as a result of a pitbull attack. I was 64 years old at that time. I had surgery to remove the torn portion of the meniscus. Then I went to physical therapy for 7 weeks. As time has gone by the pain has increased substantially. Recently I purchased the King Brand system and have been using it consistently. The long-term swelling was gone in a week! However there has been no reduction in the horrible pain. I try to stand up straight and put weight on the injured knee and leg. The pain continues, and wakes me up several times a night. I went back to my surgeon a couple weeks ago. They took x-rays which showed my leg bones are in excellent condition. The surgeon told me there was nothing more he could do for me except send me to physical therapy again or to the pain management clinic. The original physical therapy didn't seem to help much and I don't want to go to pain management which I feel is just a bandaid. I'm exhausted from getting minimal sleep at night, and I'm desperate to eliminate the pain. I can hardly walk, and I've fallen 3 times, I want to get back to my very active life renovating old homes, even climbing 3 levels of scaffolding, painting, tiling, you name it, I was doing it. I'm desperate to eliminate the pain and to get back to my life. I was very active and had very good physical strength before the injury occurred. Please tell me what I can and should be doing. I'm desperate for healing. I will greatly appreciate any help you can provide me. Thank you. Cheryl

Thank you for your video - very inspiring! It is interesting to see the inside of the knee and see what the meniscus looks like. I can't believe how fragile it looks! I had a meniscus repair on my right knee on 17th October (8 days ago). It was a bucket handle tear however, the surgeon told me that it was repairable (I'm thinking that perhaps the tear didn't look as serious as the ones in this video!). He told me that there were no issues. ** A little background story of how it happened - I was stepping down from a stack of stone slabs, the height of which was about 3ft above ground. As I stepped down, my ankle gave way and over flexed (due to hyperflexion) which then transferred the forced to my knee. The same of which "popped" and I feel suddenly to the floor, unable to get back up. I was on my own at home at this time so I thought I dislocated my knee. I sat up straight in a 'L' shape position and tried to straighten both legs. I heard and felt another "pop", as I tried to ignore the chronic pain, I noticed that I was able to stand up on my leg and limp inside the house. As I've had numerous sports injuries in the past - I quickly noted the Rest, Ice, Compression and Elevation that needed to be done. This happened 1st November 2022 and since then, I've had the same "popping" incidents 10 times throughout the year before being admitted to surgery. The last time being July, where I was playing rounders and ended up colliding with another person. This time, the "pop" trick I used previously did not happen and I could not fully straighten my knee for months until the operation ** My leg is in a full brace and I have been given crutches and I have been told not to put ANY weight on it for 2 whole weeks. I have been given blood thinners to inject myself with each day for 14 days. I have been toe touching with my foot on the ground but not putting any weight on my operated leg. (I mainly stand with all my weight on my left leg). For the first few days it was a struggle to get anywhere in the house! I have been subjected to the downstairs only, sleeping in the lounge and using a commode for bathroom time. I was given co-codamol to ease the pain which have been helpful. I was not toe touching within these first few days as I listened to the pain (it was hard to ignore!). Mainly getting a lot of rest was the goal here. The latter part of the first week (from day 5 to day 7) I was getting fidgety. Unfortunately I am a busy body and I am not someone who can stay in one place for too long! Before surgery, I was up and about all the time. It would have been hard to keep me still as I would have always found something to do. During this time, I would use my crutches to try and get up the stairs (luckily for me, I live in a small house!). I would try to move around a lot more at home. I have even managed to buy a mobility scooter and have gone to the local shop for some light shopping! This has, weirdly, brought me extreme happiness as the fresh air made me feel 100 times better. In terms of pain, this began to die down. I still take the co-codamol however, not as many per day as I did before. I've even forgotten to take some at times! (so I don't need it so much). Now, at the beginning of week 2 (8th/9th day) I am feeling like I can fly between the lounge and the kitchen will very little pain. I can get up and down the stairs (carefully) at a good pace without pain. Any pain I may experience will only be from tensing my muscles in my leg and around me knee. I am feeling more mobile and happier and in less pain. There are times when I worry about whether or not I am putting the meniscus healing at risk by not being static every day. However, I am not that type of person and moving about makes me feel better. I have, at times, felt some pain just below the knee cap - around the area of where you would usually get tendonitis. I don't have tendonitis however, after calling the nurse to check some things - she told me that this is perfectly normal. It doesn't get in the way of my usual daily activities but it's annoying when it happens. At the moment, I still have yet another few days to wait it out until my next hospital appointment. This is with the hope that they can remove my leg brace and transfer me into a bendable knee brace. I would imagine also at this point, I would be allowed to put weight on my operated leg (as much as the pain allows). I am 30 years old and will be 31 next week - I work part time in an office and I drive to University to study full time. It saddens me that I will not be able to drive for quite some time however, I am hoping that the wait won't be too long now. I take 15g of Hydrolysed Collagen (Bovine) with Vitamin C in my coffee every morning (and have been doing so for a number of years). I take many supplements being mainly vitamins and minerals (again, have been for many years) and I am hoping that this will assist in the healing of the meniscus. If there are any tips or, if I am doing something that I shouldn't, then please do let me know. I get quite paranoid with my knee at times. I've broken bones and sprained joints however, meniscus is a different ball game - one of which I hope that I do not participate in again! I fully appreciate that this comment has become rather a long essay but in light of the above, I still have my provisions.

I am 4 weeks post op of arthroscopic meniscus repair of the root where surgeon used anchor. Surgeon also found bone fragments in my knee and cartilage detached of which he removed and this has left me with a crater of removed cartilage and this is because of a rare condition called AOCD (adult form). Surgeon's post op instructions said to ambulate and use crutches as needed. At 2 week post op appt, surgeon tells me I should have been using crutches. Much of what I've been reading about OCD and post recovery is very involved, specifically NO weight bearing for 4-6 weeks. Also, 2 months prior to my surgery, is when my injury began while hiking down rocks at Yosemite and developed a sprained knee. All together, it has been 3 1/2 months of inability to run, go up and downstairs normal and walking with a stiff leg. It is very frustrating, to say the least, that some surgeons provide patients with very specific post op instructions and exercise one should begin post op where as other surgeons offer little instruction. My surgeon, provided me with contradicting instructions regarding weight bearing and provided me with no post op timeline of what I should or should not be doing. At this point, I'm very concerned I may have caused damage to my tear and perhaps it's not healing. I have very little mobility but working on that as I've searched RU-vid videos to find such exercises. Really??? Shouldn't these surgeons be providing us with such exercises?? Why should I have to find my answers on post op exercise for a torn meniscus repair on RU-vid??

I am in the same boat here. Vague post-op instructions. (Billing is another thing) I showed up to the post-op without the knee brace as it felt good, and got the same thing; Where is your knee brace? Did you tell me? How long? Should I put weight on it? Should I be doing exercises? Who knows. If it doesn't heal, how am I to blame?

Dr Geier, I had a partial lateral meniscusectomy in November 2017. The tear was impinging between my femur & Tibia causing my knee to lock for prolonged periods in a 30 degree or more bend. I had to manipulate my leg with my hands to unlock. Although the locking was less severe post-op. The overall pain was worse. Permiating to my foot. I recently chose a different surgeon. MRI shows additional tearing & debris in the joint. I have another knee procedure next week. With the probability of either a full or partial knee replacement in September. My current Dr pre-op provided a custom fit Breg knee brace as well as an ice therapy unit delivered to my home. The previous surgeon hadn't offered nor mentioned either of these items. His only initial instructions was to use crutches for 2 days only. He told me "do not show up at my follow up with the crutches". New surgeon seems much more diligent about my recovery even prior to the surgery. Everyone has an opinion on this & I understand bearing weight promotes blood flow. However, the prospect of having 3 surgeries on my knee within 12 months is a bit more than I signed up for. Is there a standard, cut & dry, clear answer on how soon after the meniscus repair should one be weight bearing on the leg? I don't want to be disparaging towards the first surgeon but knowing my post-op recovery or lack thereof. I can't help but blame the theory of "get up & walk on it ASAP" as the culprit causing my need for a 2nd surgery only 9months later in attempt to stabilize my knee in preparation of the knee replacement. Are there standards on post-op weight bearing? Apparently not, however I would appreciate your opinion for the "tie breaker" in my situation. Thank you in advance.

I initially injured my left knee by twisting my knee two years ago when I got on my horse. I had meniscus surgery on Wednesday. according to my surgeon my meniscus was in really bad shape. It had shreds in some areas. I was surprised I came out of surgery with no pain at all and within a day I could put weight on my knee. I started driving my car for the first time today. The only thing I struggle with is that I drive a manual car and when I drive for a short distance changing gears my knee starts paining a bit inside. Not sure if it's due to the movement and it becomes irritated by the movement. I would like to know if this is normal and if it might get better in time.

Had an injury at work. I'm a flatbed truck driver and haul steel. I do lots of hard work consisting of lifting, chaining, stooping, bending, climbing etc. I Tripped over a strap. Fell on my left kneecap, and at the same time, my right leg got snagged at the knee and slightly hyper extended it. Tore the left knee meniscus under the knee cap,(more like smashed it) and on the right leg tore the meniscus in the rear. Kinda like pushed it in some way. I feel, not sure. My Doctor told me he could shave away some of the meniscus where both injuries occured. Its been a year since the injury and to date my left leg knee has actually come around well, with exercises I've learned online. Not so good with the right, both knees do the popping, but the right has a hard pop and feels stiff in the rear where it was injured. Both are very strong, but the right rear has a thickening feeling. No pain on either knee. Any advice would truly be helpful.

Just had medial meniscal repair surgery, 'large bucket handle tear'. 6 weeks post op, just got off crutches and out of my ROM 0-90 degree brace. Feels good to put weight on it, bending it is the only time i get discomfort and its only around the medial repair site, i can bend from 0-100 comfortably at the moment, past that is where it gets tough and is un explored territory so far, just started light physiotherapy to get range of motion back. I really hope it heals and i dont have to have another surgery, before this i was training BJJ 5 days a week for the last 4 years and my surgeon says it will be at least 9 months before getting back to training.

MRI with contrast showed more loose bodies especially a large one in area of knee that was very painful. surgeon did another surgery in February to remove loose bodies but also found small tear on meniscus which he repaired. Recovery has been very painful. one of the loose bodies was actually attached to my knee and now I have to see rheumatologist to test for rheumatoid arthritis or for any disease that could cause those things in my knees. without trauma what could be the reasons for constantly getting loose bodies?? I am very discouraged and nervous about recovery especially because I had meniscus removed on other knee last November but now pain is also bad on that knee too since it has to compensate for recovery of current knee.

Why can’t I do splits or turn certain ways when skate since my meniscus repair. I got a meniscus repair surgery and they did a autograph. I heal great was back to normal activity in about a month. I can bend my knee all the way back make knee straight. It’s just I have the issue that I cannot do a split because I feel pressure in my knee. And when I skate and turn certain ways I feel that saying tension as if my knee will pop out of place if I keep going. And I got my surgery probably about four years ago

My 61 yr old husband has had problems with his knees for years. Went to the Dr the other day. He the doc couldn't believe that he was walking around. Seems that he is none on bone with his knee. Also, without having an MRI yet only the x-ray the doc said it appears that he may have a torn meniscus. Dic gave him a cortisone shot and he goes back in 10 days, which is this Friday. What should he ask the doc? Is surgery necessary or is it something that can be medicated and repair itself. Any help would be appreciated.

Had lateral meniscus repair surgery (90%torn) 8 months ago. Felt good until I played baseball last month and ran on it. Was followed by severe swelling for 10 days, now that swelling is down, the knee feels “weird”. Site of surgery feels loose and I can feel floating body in it when in flexion. Fear that the stitches have slipped and it is matter of time before meniscus flips out again. :(

I am 59 yo fairly active female (skiing, hiking, gardening, etc..) and am in the beginning of my 4th week after meniscus root repair. I am to be NWB x 6 weeks. I accidentally stood on my operated leg for a few seconds and sat down as quickly as i realized my mistake. My foot was numb for a moment. I am now very concerned that I have caused damage that would affect the success of the surgery. Would you have any comments about that. Does this happen to some patients and what are the consequences?
@perrymulhollen7406

Marianne, I am 4 weeks post op of meniscus root repair. Surgeon also discovered bone fragments and detached cartilage of which he removed leaving me with a crater of where cartilage was. Also, diagnosed me with a rare condition called OCD where blood doesn't properly flow to the joint, causing bone breaking off and ultimately detached cartilage. All this said, surgeon's post op instructions said to ambulate and to use crutches as needed. Since my surgery, (4 weeks) i've only used my crutches one time and I seem to be doing ok. I believe you will be ok with a slip up of bearing weight as I've been bearing weight daily for the last month. Quite honestly, I would have liked to have had a surgeon like yours, instructing me of no weight bearing. I only hope my weight bearing does not cause complications down the road. Good luck with your recovery.

I had a meniscus repair surgery 12 days ago. I have an upcoming post-op appointment scheduled in 3 days. Last night I noticed major swelling on my knee and foot. The swelling had subsided about 3-4days after surgery but returned last night and it appeared to be worse that those first few days after surgery. This morning the swelling subsided slightly but still is present. Is this a normal part of the healing process? Is it a possibility I may have re-injured the area while going through the recovery process?

i am 26 years old and when i was 17 back in high school i had a meniscus injury and i only went to therapies but i always had little pain here and then or probably after game.. then when i was 24 my knee started hurting again and i had an mri done but they just sent me to therapies again and i got the same results, now im 26 and i looked for a second opinion and my dr decided that surgery was the best option since i was limping and my lifestyle was being affected.. so i had a meniscus repair about 6 weeks ago, and i was only able to bend my knee 40 degrees after the surgery..., i have been going to therapy since the surgery and doing some exercises at home and now i am able to bend my knee 105 degrees... my knee has been drained twice but as of today it is still swollen and im also very concerned because the part of my knee where i had a very small tear ( medial posterior horn) still hurts and the pain is worse than before, i cant go on the tip of my toes because i cannot stand the pain, or in therapies when they grab my foot with a band for me to do exterior rotantions i cant do that because the pain too bad like 50 times greater than before the surgery.. i am hoping this is because of the healing process and not because my dr messed up my knee.. i know i have to be patient and wait to see what happens but it does make me worry to think its going to stay like that.. i am hoping it gets better and allows me to play soccer the same way as used to play before all of this happened.. if you have any comments or opinions in regards to this please respond to this comment :) i would really appreciate that..... and thanks for the videos it is very nice of you to help people understand all of this stuff

I have been to physical therapy many times and it has been no help. I have poor balance and I can't do any sports that require lateral movement, like skiing. It hurts a lot and there is no strength left. I wish there was something I could do. They say a knee replacement is not justified. I tried injections a few times but they didn't help much. I was pretty much walking without crutches a few days after the surgery as it caused no pain. I’m not sure if that’s bad or not 🤷🏽‍♂️ I was told not to bend my knee for 6 weeks, we are now in week 3 and I’m worried I may have messed with the surgery as I’ve accidentally bent it a few times while getting in or out of bed. How would I know if it’s still in tact and also if it doesn’t hurt is it ok to bare weight on it?

Presumably stem cell injection, harvested from bone marrow, does a similar job. What is the mainstream medical view on such treatments, and other novel approaches like prolotherapy?

Dr. Geier, I am a 44 year old male. I had an ACL reconstruction surgery in Norway, along with the repair of my longitudinal posterior medial meniscus tear in the red-zone in 2016 after a football (soccer) injury. A year and a half later, in Malaysia, I went in again to get the meniscus tear in the same location repaired once again, and received another ACL as the OS told me that the previous reconstruction in Norway had resulted in a loose ACL. 9 months after that, I played my first football game, and unfortunately, tore the meniscus once again, same location (even though rehab went fine, and my knee felt good otherwise) . 4 weeks ago (July 2018), I went in for a meniscectomy but when I woke up, I was told I received yet another repair. OS (same OS who did the surgery 9 months prior) said he would have had to remove %50 of my medial meniscus, and he could not bring himself to do it. He told me that he wanted to try stem cell therapy. I was surprised to find out, cord blood MSC treatments are fairly advanced in Malaysia, and relatively affordable :-). Do you have any experience and/or opinions on the use of these stem cells to aid the healing of a case like mine?

It’s been about a month and a half after my meniscus repair and I have no pain walking or doing normal activities but I have a clicking noise from when I bend my knee and extend it and it reoccurs every single time. Also I have loud popping in my knee and crunching is this anything to worry about? I am a 14 year old high school athlete and I’d love to know soon as possible!!

Nice informative videos. I have a question. I have horizontal (grade 2) tear on posterior horn of the medial meniscus, and full acl tear. Is it possible that i have only acl reconstruction and not go for meniscus repair? I don't like the idea of trimming my meniscus. Does grade 2 tear needs surgery?

That is always possible. The question would be whether the meniscus tear bothers you after you recover from the ACL surgery.

I play lacrosse for Hampden-Sydney College and I suffered a full tear to the lateral meniscus to my left knee and a fractured femoral condyle in the process. I had meniscus repair surgery on Feb 15 and today is May 9, so I just started slowly running. there is some discomfort and occasionally my knee pops it makes a cracking noise. Is this normal? I had meniscus repair surgery in december of 2017 and everything has been good up until about 3 weeks ago when I felt pain. I waited a few days and then played a soccer tournament which made it swell up and stiff. I went to the doctor and he says to continue soccer (after taking 2 weeks off) but my knee doesn’t feel right. It always is uncomfortable and when i twist it or bend it I have a sharp pain. I had arthroscopic surgery for a meniscus tear Feb. 7, 2019, over six weeks ago and it hasn’t helped my pain at all. I had physical therapy and that didn’t help either. I am walking with a terrible limp due to the pain and now my back, hips, shoulders, and other joints are hurting. My doctor said we would have to re-evaluate in 4 weeks. He thinks I may just need more time to heal. I hope so but I injured my knee in November, 2018 and it’s been painful all this time without much relief.

Hey Gayle, I injured my meniscus back in October of 2018. My ortho told me that physical therapy would help to alleviate the pain I was experiencing which was in the inner right part of the knee in the meniscal area. I’ve experienced swelling, sharp pain, and had to R.I.C.E for quite some time now and over the course of 5 months of physicals therapy we couldn’t come to the conclusion of why I was still experiencing pain in the knee so he suggested an arthroscopic diagnostic, I had the surgery May 24, 2019. I’m now 26 days post op and I’m still experiencing really sharp pain, intense swelling of the knee and since it’s now been well over 6 months since the injury occurred my hips, backs and everything else is starting to hurt as a result of over compensation and limping. How has your progress been thus far? I’m hoping to find a glimmer of hope at the end of this dark tunnel of my recovery process.

I am 1 1/2 weeks post op from acl reconstruction and meniscus repair. My doctor told me that i could bare weight on it and said it was good for my meniscus. But i am in a brace locked when i walk but i can unlock it when i sit down. But i am also restricted to a 90 degree bend.( I have already hit that mark). My leg feels pretty could except for swelling still and a little pain, but that is expected for a while. I am kind of confused tho because a lot of places i read online say that you should keep your leg locked for at least 4 weeks? But my doctor said i could already bend it? Thank you for these videos. I'm a 40 year old marathon runner. I had a medial meniscus root tear repaired on April 22, 2016. I was non-weight bearing and in a locked knee brace for six weeks. I quickly gained full range of motion back while still in the brace, and have been going to PT twice a week working on strengthening exercises and I'm doing a lot of walking on my own. I'm still having a lot of sharp pain in the knee while walking up and down stairs, and certain PT exercises such as squats and lunges exacerbate it, and even just walking around some days I can feel it. The pain feels almost exactly like it felt when my meniscus was torn, which makes me very nervous that the repair failed to heal. Is it normal to feel sharp pain during recovery after a meniscus root repair? I'm 11 weeks post-surgery, and I'm still hoping to be cleared to start doing some light jogging at 12 weeks if released by the doctor at my 12-week checkup. Thanks!

I’m 3 weeks post op For a meniscus tear and acl tear. Only issues I’m concerned with are that I still can’t fully extend my knee. Mostly my fault as I didn’t have my knee straighten in the brace for the first few weeks. Also when I do walk with the brace on I still feel instability like my knee wants to lock up when it’s almost fully extended. It might be psychological but my therapist claims it’s all normal. Any suggestions? FYI meniscus was cut out and acl was taken from my hamstring.

Hi Dr. David, Hope you are doing well!! 2yrs back for right leg knee arthroscopy for torn meniscus(Menisectomy), and I believe I haven't strengthened my knee enough that it is supossed to be even after few PT visits. Recently, I had started playing volleyball, running and doing Squats, my Knee seems to be little swollen and feel little discomfort while folding and it is not folding completely as it should be as a regular other knee and I dont feel much pain while walking, but there is little discomfort, not feeling as regular knee, feeling like something happened, I couldn't fold completely might be because it is swollen, also I am little worried while playing volleyball and jumping near the Net and it might become worse. Is there any fluid that need to be drawn from knee that is causing swelling? What should I do to make knee as regular knee and keep active in volleyball? How do I avoid swelling again? Thx for your time and replying...
