The grip seems "loose". Like the shooter isn't holding it (quite) right. A little practice towards technique, might clear it up. Some dry fire practice would be good too. Getting that trigger press just right is important. [That said: Almost every 1911 I have ever fired, has put spent brass on my forehead.🤷🏼] Start with the fundamentals and try again. If you can eliminate operator error that leaves ammo and gun as the possible culprits.
