I read that Serena was got entry into the US Open, despite having a WTA ranking of 402, by being given a "special ranking" of No 16. Is this the "protected ranking" referred to at 2:46?

Yes, ATP uses the "protected ranking" term, while it's "special ranking" for the WTA. Both basically are the same thing, available to a player who has been absent for 6+ months from the tour due to injury, childbirth and other reasons.

1. Can a player who was in the qualifying tournament still receive a wild card if they lose in the qualifying? 2. If a player withdraws from the tournament after the draws were made, how do they determine who fills that spot?

Lucky losers are a player who lost in the final round of qualifying but are given a second chance by being awarded a slot in the main draw if another player withdraws after the draw is set.
