Dear Dr. Ford Brewer, in this video you mentioned about adjusting your diet and regarding prescription, you now take Ramipril (same as I do), and also Crestor as the statin of choice (I am prescribed Atorvastatin 40 mg/day) following my myocardial infarction in 2019. By the way, my LDL score is below 2. My question is with the statin and excuse me for not checking date stamp of this video and your video about the 2022 JAMA Report on statins (absolute risk reduction). I do not hold you accountable for your answer, but did you not mention that statins now provide little to no advantage versus the disadvantages (liver, kidney, muscle, testosterone, etc. related issues, short or long term)? So, I have a two part question. Based on myself (the little I’ve shared other than eating a very healthy diet, free of most any forms of sugar, yet low carb intake like oatmeal and real rye bread), do you suggest that someone like me continue taking a statin? And are you taking a statin now ( I.e., Crestor)? PS, since watching the video about the JAMA Report, I stopped taking my statin in early November and feel more energy now, and other positives (no more muscle aches and feel stronger).

Thanks for the question. I still take & recommend statins. But - only for documented plaque, not LDL score. The AHA has followed me on that. Also I don't use mid or high doses, only low doses. They manage what we need (CV inflammation, not LDL levels), & side effects are minimal at those doses.

@@PrevMedHealth Thank you does not provide enough, yet I know the good Lord provides in other ways (I trust you know that already). I certainly have documented plaque confirmed by two separate angiograms 17 months apart (14 partial blockages in various locations of my heart arteries). I not only listened carefully to your video, I will document the salient points including the additional tests that you suggested to further understand my condition, one of them which I think is related to insulin response after eating most anything that has sugar in it (too many blueberries with my oatmeal or if I had a Christmas homemade cookie/treat). From your reply, I will continue with my statin as I understand, but work towards reversing my plaque and build up the endothelial layer of that is possible for me to prevent that unstable form from rupturing through and causing me another M.Infarction. I plan to review and hopefully be reassigned with a more advanced cardiologist than the one I have now (he’s in the 30’s - newer in his practice - and he didn’t know about that JAMA Report form 2022). I think we have some good cardiologists here at our Mazankowski Alberta Heart Institute in Edmonton Alberta. Many thanks for your response; I’ll start taking my statin again (reluctantly) until further evidence shows it not very beneficial. 🙏🏼 best regards Dr. Brewer, Robert Pigeon.

Unfortunately due to big pharmacy policies all drs who want to keep their licenses, businesses or jobs have to follow what they are told =becoming licensed drug dealers. What that means is like that : you sat on a tack and because of pain you go to the dr get rid of the pain and he gives you pain killers till you will become addicted but pain does not disappear. Old medicine was looking fo the cause to fix it not just treat the symptoms. Good luck finding a real dr. !

Insulin resistance - I was classified as "pre-diabetic" by my doctor so I started fasting 48 hours once a week to avoid drugs and to move my HDL/LDL ratio towards normal. With 3 months I went from prediabetic to "normal" (albeit middling high normal) and 6 months ago I started taking berberine 500mg/day and now my glucose level at a 12 hour fast is low normal. I saw great strides in my triglycerides and ldl/hdl levels with berberine as well. Overweight sedentary 56yr male w/ BMI of 30.

Thank you so much. I am approaching 70 years of age. I also worked many years in a busy emergency room as an RN. I don't smoke or drink. I am not over weight and very active I wasn't taking any medication until just recently. About 11 years ago I had a calcium scoring nuclear stress test. The cardiologist that read the results said every thing looks good. So, I did not check into the results for myself. A few months ago I developed some chest and arm discomfort with exercise. When I went to my doctor he discovered a murmur over the mitral valve. He also told me that I had a calcium score of 260 in the LAD 11 years ago . He started me on a statin drug. Lipitor 40mg and losartan 25mg. I feel like a ticking time bomb. Also, I feel quite depressed. I began walking every day 2 miles and then I do 50 pushups. I used to run but have arthritis in my ankles and knees. Your video gave me some hope that I can improve even at my age and condition..

Thank you for sharing your personal info, very helpful! I noticed that at the same time your left CCA went from 1.215mm in Feb 2015, to "No Plaque Noted" in Sep 2016, the right CCA increased from 1.209 to 1.493 over the same period. 1. Why would plaque increase in one artery while decreasing in the other? 2. Were the statin and ACE medications necessary due to high inflammatory markers that could not be brought into range naturally due to genetic factors? 3. From the Feb 2015 CIMT report, how do the right and left IMT's of 1.209 and 1.215 end up with an average of 0.884? Thank you so much in advance!

I would love to hear your thoughts on the theories that LDL is simply trying to help heal damaged arteries: - The analogy of blaming the firefighter for trying to put out the fire? Also we hear about good and bad LDL (light and fluffy versus small and sticky). I wonder if it could all be traced back to damage in the liver that would produce the bad LDL or would you say the LDL becomes oxidized in the blood stream? My GP just flat out says that all LDL is bad.

Change doctors?! LDL is oxidised in the pan and bbq. Plaque is more than cholesterol. Cardio-vas. damage is being done by high insulin from high carbs esp refined. This harms ALL organs which have a blood supply (!). Anti- inflammatory diet and supplements and meds all can assist to reverse. Lowering Cholesterol has not been proved to lower heart attack or mortality.
@jeffpearlstein8191
@jeffpearlstein8191 Год назад
You are 100% right. Ancel Keyes way back did all this research on his open heart patients and every time he opened up someone that had a heart attack ldl was at the rupture site. He assumed it was the cause and like Nadir Ali so greatly explains and your analogy same of is ldl the cause of the fire or what’s rushed to the scene to put out the fire. From there that man started the statin era. When ldl goes up every other market goes down. That says it all. Maybe it was meant to be high not low. We have been fooled by big Pharma and our doctors. Statins actually reduce risk absolute that is less than 1% but they lie and only tell you 36 % decease but relative risk. And funny they do the opposite for the side effects:)
@HobzyMcRuse
@HobzyMcRuse Год назад
@@jeffpearlstein8191 Yes, I find it absurd that even an unhealthy liver would produce bad ldl. The body is way smarter than that. Also I suspect that the calcium is also just doing it's job in patching up 'cracks in the wall' although if I mention that to my GP he'll boot me out of his clinic. Ldl gets recycled anyway, goes back to the liver to begin the process. Inflammation is doing all the damage.
@kathygann7632

I do fine with stress tests, but my calcium score is 665. Does this mean the calcium is on the outside of the artery instead of blocking it? I had a really bad case of Covid, and a month of radiation for breast cancer. My LDL is only 50. Statins lower LDL and cause calcification in the heart. There are significant side effects to having LDL scores below 50, but, man, cardiologists really like statins.

One of the side effects of statins is higher risk for diabetes. Diabetes causes inflammation which damages arteries. Eat a very low carb diet, moderate protein and fat to satiation will lower glucose and lowers inflammation. Or you can use drugs and increase the dosage steadily to address the inflammation that increases. It is not widely publicized that 30% of the people in the statin trial were dropped in the early phase due to negative side effects of statins. These folks were not included in the final report and final statistics. Do your research.

This is the best summary for understanding arteriosklerosis, stroke and heart attack. The result of your studies is, that arteriosklerosis is not only caused by LDL levels but multiple other reasons, included genetic failures, inflammation and prediabetes. Thus the therapy is not only life style change but also taking the proper medicines. The difficulty is to find out the most important and helpful substances.

Yes. 1. look for plaque; 2. look for inflammation; and 3. look for root causes. Top root causes are: diabetes and prediabetes, oral infections, sleep disturbances, Lp(a), and a few others. LDL is actually way down the list. LDL is more the mechanism than the cause.

My understanding is it’s not so much ‘liquid’ fat forming a clot, but a piece of hardened tissue (fatty or not) breaking off and that is clogging the artery. And actually you can have a heart attack simply from such a stenosed artery during exercise or a spasm, because at 90% stenosis there isn’t enough blood flowing through to perfuse the heart.

One thing that I noticed that seems to be missing is the thing about exercise - it has already been proven that even a little bit of it is extremely good for ones health - even say 45 to 60 minutes a day of walking may even be the best of things - something to do with a second wind stage thing taking place - a pump in our feet help swirl blood around so much better than any other way of replacing cells at the fastest rate - and speeding up the entire system of things in our bodies - seems most people are just to lazy to ever find out if it really works or not - odd thing ! - and there have been studies on fasting - all though it is said - it is better to use melons to fast with for around 3 days instead of just doing without food - something to do with water and a cleansing effort that you cannot get from no food at all - funny how our tax dollars get used for studies that never even come to light - something to do with people that live beyond retirement take up too much space without giving any useful efforts back - or some dang odd thing like that - and i have read something about using a hot tub daily can and will make things work better in and by the human body - something to do with heating and messaging helps the outside but the inside of the body - which can do things that nothing else can - makes the arteries and the muscles relax much much better that other things do - Athletes sure do Praise there affects for helping to heal parts much sooner that not - and then there is the studies on olive oils and other foods that are keeping patients alive longer than was predicted - but then I digress --

from my understandings -- they work on a little over 50 percent - depending on the name brands and how often you consume them - as for vitamin C and D - without the enzymes that come from foods with those vitamins - they do not work so well - and you need to use a reputable dealer of them - and even then they are not what they are supposed to be it seems by the studies - an independent third party evaluation is needed by the manufacture for them to be of any value - and then the price goes up - but seems to be worth the difference they say -- a bit of a coin toss it seems - oatmeal would need to be eaten almost every day to be of a big difference they say - organic is always better -- and oatmeal is not the only food that works they are saying -

I recently saw info on statins linking them to atherosclerosis, inhibiting K2 and allowing calcium into the arteries Also CoQ10 and something called heme? adversely affected. This was recent study and stressed high mortality rate if on statins. I am aspirin and 20mil pravastatin. I am full of plaque..... Atherosclerosis and PVD. Please respond. From your previous videos, regarding Inflammation, I am thinking I should keep the aspirin and change the statin to low-dose Crestor? Don't know what else to do, but I can feel things not looking good.

It’s a great point. Of course statins are linked to plaque; it’s one of the major reasons for taking them. To assume the researchers understood & rules out that obvious bias would be naive.

No credit for the intermittent fasting. That could have been a big factor, but should have solved the pre-diabetes. I am 78 and have had a TAVR (trans aortic valve replacement). I am doing well and have increased physical ability. Probably above average for my age. A review of the other sites on RU-vid show lots of other options to possibly clean ones arteries. Intermittent fasting, weight loss, increased activity are the low hanging fruit. Many supplements and dietary changes may help also. I would like to know more about my genetic factors also.

You might want to look at Berberine HCL. It has less side effects than metformin for many and equal or better efficacy it also acts acutely as a glucosidase inhibitor as well as systemically. I also take curcumin. You mentioned hiit in the title but didn't mention it in the talk. It's said to help stabilize plaque. I'm 73 dx with t2d at 70 controlled with diet, exercise and supplements. Also hetero apoe4/3 so taking atorvastatin 5mg along with lisinopril 15mg. What is the advantage of ramipril over lisinopril? Trying to find a way to administer trehalose which in mice alleviated plaque in both the brain and the arteries. Some work has been done by bioblast using it IV for some diseases neuro type diseases including Huntington's. Some work also done with oral and arterial relaxation but huge doses and since most is converted to glucose in the gut not a satisfactory solution for diabetics. IV looks to be the best way. Mike

But Ramipril has the best anti inflammatory performance of the ACEi. You probably need that with your risk factors, especially if you're managing insulin resistance with berberine. Lipitor will lower your ldl. But it's not likely to impact your cv inflammation. And that's more important. I have videos re: this coming out next month

I've been on statins (simvastatin) for years due to high LDL. But my problem with statins is that it also lowers HDL, my good cholesterol. My HDL went from around 77 mg/dl before statins to around 52 mg/dl after statins. I used to run marathons before statins. Now I can't run a 5K. So I wonder: are statins the answer for very healthy runners with high cholesterol?
@mikemccarthy1638

You’re commenting 3w ago on a 5-yr old list. W/in reason, your quality of life matters more than a small increase in life expectancy. Listen to your body, not the regime in place when your doc was in med school. LDL is not the enemy; the ~20% portion of it that’s “very low density L” is. If your triglycerides are high, your vldl may be too (standard lipid panel doesn’t break out vldl from total ldl). (If your triglycerides rose as your hdl fell, that’s a clue.) Dr Robert Lustig & others have YT videos on the current science on lipids. LDL w/out vldl is not harmful, and some are looking at it as the ‘fireman’ on the scene fighting the arterial plaque fire.

Hi Doc, Thanks for the great info. I am also considering a switch from Losartan to Ramipril for the anti-inflammatory effect. Unfortunately this study suggests there is no evidence that ACE drugs lower inflammation. Are there other study’s that support the anti-inflammatory effects? Thanks. Curr Pharm Des. 2012;18(28):4385-413. Effects of ACE-inhibitors and angiotensin receptor blockers on inflammation. Di Raimondo D1, Tuttolomondo A, Buttà C, Miceli S, Licata G, Pinto A. Author information Abstract The role of inflammation in cardiovascular disease and in hypertensive disease above all, is complex. Several studies confirm that activation of renin-angiotensin-aldosterone system (RAAS), through increase in the production of angiotensin II (Ang II), is closely related to local vascular inflammation. Over the BP lowering effects of anti-hypertensive treatments, several ancillary effects for every class may be found, distinguishing the various drugs from one another. Given the pro-inflammatory effects of Ang II and aldosterone, agents that interfere with the components of RAAS, such as ACE inhibitors, Angiotensin Receptor Blockers (ARBs), and mineralocorticoid receptor antagonists (spironolactone or the more selective eplerenone), represent logical therapeutic tools to reduce vascular inflammation and cardiovascular risk, as suggested in large clinical trials in patients with hypertension and diabetes. Regarding ACE inhibitors, actually there is no convincing evidence indicating that ACEi's reduce plasma levels of major inflammatory markers in hypertension models. Lack of evidence concerns especially these inflammation markers, such as fibrinogen of CRP, which are less closely related to atherosclerotic disease and vascular damage and conversely are affected by several more aspecific factors. Results obtained by trials accomplished using ARBs seem to be more univocal to confirm, although to great extent, these is an anti-inflammatory effect of drugs blocking AT1 receptor. In order to strictly study the effects of blockage of RAAS on inflammation, future studies may explore different strategies by, for example, simultaneously acting on the ACE and the AT1 angiotensin receptors.
