Very good advice DR Gurs about the over-aggressive habits such as over brushing, water pik etc... they certainly can do more harm than good if we are aggressive with usage. I have caused more recession with aggressive flossing.

I had a full upper rack about 14 months ago and have water flossed a couple of times a day on a medium spray, and am using floss threaders with thick floss dipped in mouthwash or peroxide a couple of times a week, because i want to keep the posts clean, no film, but with your advice, I think i’ll reduce the water pic and and threader- floss daily. I live near a dental school, actually 3 of them, and had the work done at the one most convenient for me, and so far, no problems. Thank you for your recommendations , Doctor.

I could easily incorporate an irrigation design directly into the dental bridge. Locate a port of sorts toward the back end of the bridge (back of mouth) where a waterpic tip is inserted and the entire bed of bridge and gum line would be irrigated and cleaned. Just a thought.

Dr. Gurs, I have 2 questions, if you'd be so kind and have the time to answer. Well, one question and a suggestion really. The question: what are your thoughts on basal implants (ihde strategic implants for example)? And the suggestion, for the video editor: please consider using a sans-serif font for the subtitles, like the ol' plain Verdana, or Lato (free from google fonts), it's more readable. Also, the subs can be bigger and wider, as they won't break focus from the subject, but will help in understanding the content better. Thank you.

Hi doctor. Have you seen or researched G4 by Golpa The G4 procedure offices are in Las Vegas, Dallas Texas and I think Virginia I have conus implants on the bottom and a 3D printed upper denture. My dentist wants to use the cement process with my existing implants (the implants look wonderful and healthy) from 15 years ago. I've had no problems with my implants with porcelain bridge however I am wanting the zirconia for the strength. My porcelain upper arch is beginning to wear (after 15 years) and I have to replace it. I have a dental cleaning every 6 months at the dentist office and in the interim., I use a Hydro floss system and good brushing and mouthwash. All seems fine so far (Thank God)!!!!. What is your thought on cemented zirconiz bridges? Do you think it will be too heavy for the cement process? I was blessed to have good upper bone structure when he installed my implants years ago. My teeth were very brittle and they cracked (family issue) and I had a complete arch done of porcelain. I am also having the bottom done as well. My bottom arch now is a partial denture with a few remaining teeth that have failed. Please let me know your thoughts as I have a little anxiety.

Click fit is superior for this reason. They are in until you really pull them (one can eat brazil nuts, chew gum - just about everything with... no problem) and cleaning is a BREEZE. I have under gone full anesthesia with them in when the physicians realize how secure they are. I really don't understand the reasoning behind screw fitted ones at all. Well tailored click fits allow for a very close fit, also. Otherwise, some very sensible advice!

I am a dental tech.It sounds as their are alot of variables as to the longjevity of all on 4s.One fails the whole bridge fails.We do snap on dentures. The cost is alot less than fixed,able to clean very easy as it is removable and if repairs are needed very easy to do.

Hello Dr Gurs, If you have underbite, can you skip braces. Go to double jaw surgery and then full implants upper and lower?

I still can't believe science cannot figure out a way to regenerate teeth. We grow baby teeth then we lose them and then we regenerate new teeth. So we have a way of regenerating teeth like an alligator but we can't do it multiple times like alligators can. You think they would have solved that problem already but I guess there's not enough money involved. But it will happen someday.

What I don't like about all on four is it's not implanted teeth at all it's a denture with studs to stop it moving around but not snap on studs. Original idea was an implant was a replacement artificial root so the tooth worked as normal with localised load bearing - but not such good bite feel. Not bad at all though.

What about dental bonding? I'm hearing its the new thing!

How long does it last? It depends! Say, 5 years shortest to 25 years longest seen, not due to accident. Say, about 10 years on average. Then say, many of the failure can be avoided by doing a, b and c, etc. This may be most helpful. We all know it depends on multiple factors.

You don't mention a possible number of years, you only say they could last "a very long time" [assuming proper cleaning, healthy gums, etc.]. But is "a very long time" closer to 8, 12, 16, or 20 years in your view?

When an implant pulls out, does the bone regenerate? Or does it just leave your jaw as swiss cheese?

You never really said 'HOW LONG'! Given PERFECT conditions... how long SHOULD the 'Implants last'?! Then how many years can the problems you bring up 'knock off' of that?!

I went to dentaprime I wanted all on 4s but they refuse to take out any teeth that are not bad enough, so they want to put veneers over my bottom ones even thought hey are really long because of how bad my gum disease is. Is this normal practice just to cover them even though how long they are, it wouldn’t look like a nice smile if I had it done.

Please, for heavens sake answer me this, I bought the most expensive dentures they offered me 5 years ago, I hate these horrid things and I won't wear them or leave my house. They flop around, 1 bite of food and they are loose and flop out, they get food under them and that hurts, now they gag me. You have to use a massive amount of glue that is awful to clean but does not hold the dentures well, the sticky pads are more comfortable but forget eating with those, no way! I can't keep spending $$$ on relines. Can you take my high dollar dentures, uppers and lowers, full set and put those snap on posts on them? I can't afford another set of dentures . Can they do that? I am miserable.

It last forever. Whether it is still in your mouth, that is another story.
