The error was operator telling captain to leave Baltimore port with major safety violations. This ship was experiencing constant blackouts for two days prior. No accident but criminal negligence.

The one error, IMO: Not learning the lesson of the Sunshine Skyway Bridge by building robust dolphins to protect the pilings.

What was the one error mentioned in the title?

Nit-picking: the force was like being *pushed* by 3 Saturn V's not being hit by them. That would be a whole different story.

This was an unfortunate disaster crew and pilots tried their best to stop but it struck anyway. RIP to those construction workers.

If you look at the footage being aired you will see the front the ship you see about 20 ft section missing this was taken out by the bridge its self so you can figure the bridge shifted half that also keep in mind this was a toll bridge so it might not be paid for yet

There is no question why the bridge fell. The real question is why it collapsed so rapidly like a deck of cards, especially the section beyond the second pylon whilst the two ramps remained unaffected.

This doesn't mention the huge fiery outbursts seen in the night videos, nor the impossibly wilted girders over the north pillar. SOUNDQUALITY has an 18-part series that goes deeply into the most likely cause that explains alllllll evidence.

It took 10 years for CALTRANS in CA to remove and replace the east span of the San Francisco Bay Bridge which was damaged in an earthquake and by a cargo ship colliding with the bridge. The west span of the bridge was also supposed to be replaced but hasn’t been. So it’s reasonable to expect that removing the wreckage of the Key bridge and constructing a new Key bridge will also take at least 10 years.

Very few ports will allow this poorly maintained tin can in their ports. Have a look at who owns her, who leases her, where she is registered and who has the right insurance on her.

There were a vastly insufficient number of dolphins/bollards to protect the bridge. The fact the Key Bridge was "grandfathered" in after the Skyway bridge disaster in 1980 was a huge mistake. The act of grandfathering in buildings, bridges, etc. when it comes to safety is insane. And thus the insanity we observed when the Dali struct the Key.

Well, the guarding around the bridge was built years ago when the ships were not as wide. If you notice the front of the ship is sloped and very wide at the top and narrow near the water.

(Liked this video.) 2:18 the simulation shows the Starboard anchor being lowered, it was in fact only the Port anchor that was lowered on this vessel.

Honestly folks: The bridge was deliberately destroyed..! The name "Francis Scott Key" had to go, and if you ever get a chance to see the whole video this is easily seen...! The ship was passing the bridge completely, then the lights came back on and smoke came out of the exhaust stack due to acceleration...!!! Then you will notice the ship turning around towards the main bridge support, and turned it's lights out again and struck the bridge support head-on..! You can't make this stupidity up, and you can't hide it neither..! Thank God someone video taped this collision, because it shows that it was intentional..! (America is being dismantled: Statues, laws, and bridges like the "Francis Scott Bridge" have to go!)

I wouldn't be surprised if the ship lost power because of neglect. Maintenance and repairs delayed or skipped entirely due to cost. Management decisions that resulted managers getting bonuses and promotions for cost savings, for keeping the ship in service when it should have been taken out of service for repairs and maintenance. As an industrial mechanic, I've had to watch the machinery destroying itself because the production manager demanded production on scheduled maintenance days, As lead mechanic, excessive downtime due to major breakdowns were blamed on me. Fortunately, the production plant I worked for didn't crash into anything or kill anyone. Of course, the managers that made the critical decisions resulting in this accident hold themselves blameless. Technicians have probably been disciplined or fired.

If no one did anything when they lost power the freighter would have moved out without hitting anything. It was when they got power that all hell started the freighter toward the bridge pilling. This was not a mistake. If everyone was doing their job as they should this should not of happened. Just as it has not happen for over 50 years. This was not a bridge problem.

I drilled thousands of holes for rebar on the Knight Street Bridge in Vancouver. The columns were connected with sheer walls and the bases were fortified with plinths. I thought it was overkill and could it survive a magnitude 7 earth quake? I guess it's better to be safe than sorry.

The problems continue with the steel for a new bridge needing to be specially manufactured, and maybe a lot of the skilled bridge builders are already in work.

Thanks for a very fair report on this disaster which, in addition to the loss of lives, will have major negative economic repercussions for the state and country. The question can be asked(not dealt with here) as to whether or not this disaster was the result of the sabotage of the ship's electronic controls? such equipment failure is most unusual and it's unlkely the incompetence of those who are said to have serviced the ships's controls just before its departure, is to blam While it's human to err, the probability of such individuals who are well trained to check and double check, renders mistakes extremely unlikely. However, a conspiritionist, may speculate that perhaps someone unknowingly used equipment which had been deliberately infected with a specific virus for the purpose of shutting down the ship's controls at the critical time when exiting the harbor. Hopefully, the NTSB will examine the electronics of the ship along with various theories as to who if anyone, was responsible for the problem with the ship's controls. Whether or not we will learn the truth even if discovered, this should be a wake up call for the security of ports and ships; new policies should be put in place to ensure that ships entereing/leaving major waterways, are escorted by tugs bow and stern, to at least push a disabled ship away from the supports of major bridge spans if power is lost. Of course, such will add to the cost of freight and product shipped but what's the alternative to ensure safe passage?

Wasn't the DALI in that port before? Was this the first time that ship ever entered that port and was unfamiliar?

I would like to know if the ship deployed the starboard anchor or the port anchor. The course line of the ship was naturally to the starboard side of the bridge channel (like staying in your right lane on the highway) and thus if any effort to veer the ship course was to be made with the anchor, it should have been the port side anchor that was deployed. Having seen some ostensibly accurate AIS course information, the ship apparently began to veer to starboard towards the bridge pylon not too long before the collision. I wonder if the starboard anchor was deployed causing the starboard veer, whereas a port anchor would have tended to steer the ship more towards the port side and perhaps cleared the abutments. Hindsight is always 20/20. I feel badly for all concerned, especially the families of the workers killed.

On each end of the bridge were found they have rain guards. It goes across the street on each side of the bridge so the water can flow off the main metal structures into the semen down to a drain pipe yeah that’s why the bridge fell so quickly go on Google maps you will see what I’m talking about.

The comments about more protection for supports is sound. Preventing impact is the issue. You can't prevent a collapse like that as that would be expected. Protect the support system. The recovery operation is amazing. And thank goodness for the early warning that saved lives.

They should have some kind of " Hard or Soft Rubber bumpers" whatever they think would be better in the middle of these "Support Truss" Base from the outside where these Ships go thru mainly in the middle since this is the deepest part & where the Biggest Ships pass thru. The nxt Key Bridge they build, hopefully will have some. If they would have had some from the beginning maybe this could've been avoided.

I'm having trouble buying this explanation. I've seen at least two videos showing the impact of the ship and the resultant collapse of the bridge. At least one of these videos has been doctored. One video shows an 18-wheeler traveling across the bridge and it gets about 80% of the way across before the ship strikes the bridge. At the moment of impact, the 18-wheeler ceases to exist. It doesn't fall into the water. It doesn't continue on its way across the bridge. It simply ceases to exist. The other video doesn't show a single vehicle still remaining on the bridge at the time of impact. Both of these videos cannot be full, actual accounts of what took place. So, why the doctored video(s)?

Why still, is that the only video footage? There must be hundreds of cameras there.

Sure are a lot of experts here. Accidents or tragedy's like this are rarely if ever caused by a single error. Usually a string of much smaller errors that have piled up

I went to the bottom of 12 feet. 10 feet is shallow

the square ring around the bridge support survived. But, the bow of the dali reached over and past that ring and struck the support. A bridge can be effectively protected from a strike and collapse by ANY SHIP!!!

WHY should the TAX PAYER pay for it?

Why did the Pilots not ordering both anchors out, at the first black out? (6 minuts left before collision with the Bridge) Both anchors at the bottom of the channel with 4 - 6 lengths of heavy chain in the water might have stopped the ship before ramming the bridge. A black out is always a warning saying, something isn`t working correct and it´s better trying to stop the vessel before it´s ramming a bridge etc. When they had power again, for some seconds the ME should have been sat to going astern.

There are only three questions that matter, and this video did not mention or address any of them. 1) Exactly why did the ship lose power three times? 2) Did the ship have rudder control even when the power was down? 3) What made the ship take a half-right turn into the bridge support, just before the ship reached the bridge?

Must have been avoided the collision. Those Tugboats are so essential for Manuevering especially passing to the bridge. A different body that are responsible for the Safety routes,before and after passing of the bridge. The Ship was going in a wrong direction route to collided towards the Leg posts concrete foundation of the bridge. Four minutes,before the collision is not enough to correct the right lane routes,as even though whether there is or no engine failure,due to super long of the ship,big loaded,continuous drifting of the ship (8 knots) Different to a Corvettes that can easily change its course. I heard a U.S. Aircraft Carrier do passing the bridge for only 5 knots. In the first place from the start of navigation,when departed from the port of origin,if not use radar,the compass must set to desired degree course. Where in the Center of the span,arc shape of the structural,in that blinkering light. If not there must be two(2) pilots Fwd & Aft,handheld radio directing what’s happening,the position of the ship,towards the blinkering light,Center of the span. Especially in the Fwd pilot. There is something happened engine failure or drifting,Ship is still in safe position. But already 4 minutes Ship is still,not in a proper location,heading towards the Leg posts concrete foundation. The Ist time i heard they were 33crews & 2 pilots,but the 2nd time 22crews & 2 pilots. Is there an undocumented passengers?

What really happened (?) It got hit by a very big heavy ship (plain & simple) and of course the Tax Payer will foot most of the cost.

Comprehensive video. 1. Is anything keeping tugs from escorting huge ships under bridges also? Do tugs have power to stop or steer the ship away from bridges in worst cases? 2. What else has the power to bring down a bridge? Planes? Terrorist with a bomb on a motorcycle? 3. If all the pylons protecting all the US bridges are inadequate, what are we gonna do about it? More tugs? Smaller ships? 4. How slow can a ship move to be steerable AND do less damage when it does hit the bridge supports?

Yeah, I heard a rumble in at like 130 almost 120 so I’m in the morning live in Curtis Bay Cypress St., Avenue. The reason it fell so quickly is they got rain guards up on the bridge and it’s like 2 to 3 feet down with no semen for rain coming off the metal until the bridge. If you go on Google maps you will see what I’m talking about, the metal beans on the ground where the rain goes into goes across the road that’s where the bridge claps from if you go on the map, you will see that rain guard going cross it that’s why it fell so easily
