I test similarly to this video description, but I would add that if you are testing for basic volar nerve sensibility (i.e. ulnar nerve compression/injury vs median nerve compression/injury) , there is no need to test medial and lateral sides of digits 1, 2, 3, or 5, as was mentioned in the video. Both the lateral and medial proper palmar digital nerves arise from the same sources: median or ulnar nerves. Like he said, the ring finger is the only significant finger to test on both sides. If a patient had an obvious low nerve injury in the hand, like a laceration in the 2nd webspace, then it may possibly be warranted to test each side of digits 2 and 3 to help determine the extent of the injury, but it's likely not that helpful. The 2PD test is not very sensitive or reliable anyhow. I'd use the Semmes-Weinstein in that case.

I agree! The reason we taught it was due to the amount of digital nerve injuries/lacerations we see. In that case, it is very important to test both sides.

I usually just keep it simple. For instance, static 2-point discrimination noted to be 5mm on the thumb, index and middle, and radial half of the ring finger. However, 7mm noted on the small finger and the ulnar half of the ring finger.

Awesome Video! Just a quick question tho, can you explain what is the difference/ when do you conduct between a Monofilament test and a two point discrimination test.

Great question! I usually start with Semmes Weinstein to test their ability to feel light touch. It that test is normal then I usually don’t test 2-point. However, if the test is not normal or more than 2.83 or even 3.61 then I like to follow up with 2-point. That’s just my preference though!
