Hi dr, You need only one appointment for Indian birth registration and passport. You will be submitting all documents for both birth registration and passport on that day. They will do the birth registration first, and they will send the birth certificate by post to you.. Once you received it, you have to scan it and send that copy to them. Then only they will process the passport application. I already mentioned this in detail in my Birth registration video. Please check that out.

If they are asking present address, you should give UK address and if they asking, permanent address(to print in passport) , then you give Indian address.

I think, that should be okay. Annexure E is for baby. So we filled baby name and details there. Also we, both parents signed along with baby’s thump impression.

Hi, In ‘passport printable address details’ you should give your Indian address. In ‘other address details’, you can give your UK address. You can give father’s or mother’s contact details in emergency contact column.
