In this hand therapy essentials video, watch the fabrication of a radial based thumb spica splint! This splint is simple and quick to make, if you practice you can complete it in under 15 minutes (including velcro and trimming). There are so many different patterns out there, and somehow they're so hard to find! I remember being a new hand therapist and feeling totally overwhelmed with trying to learn how to fabricate orthoses. So let's get some of these patterns out there so every hand therapist has a toolkit overflowing with different splint patterns (since we know that not every patient or every diagnosis is the same, why would we splint the same for everybody?). Feel free to share this video with any hand therapists you know and let's continue to grow together.

Thank you! In my last clinic we had to do walk in splints all the time on top of our schedule so you had to get really fast with it or you'd get super behind. It makes it a lot faster when you have a pattern that you can just crop for different patients!

Hi Kelly.. I am experiencing DeQuervain’s Tenosynovitis.... probably from computer mouse usage and playing the fiddle. I am currently using a wrist brace that more closely resembles thumbhole cuffs...i.e. does not go up beyond the base of the thumb. Would a thumb spice splint be more appropriate for me? Thanks.

Yes - definitely recommend a thumb spica for anyone with dequervains!!

Thank you for the videos. As a physio myself Its helpful to learn the specifics of hand therapy without having to read a textbook. Do you know what the best practice is for splinting/bracing scapholunate ligament tears? I am currently trying to help a client with this issue

Acute tear? Are they going to have surgery or just conservative treatment? I would tend to think a thumb spica would be helpful, but depends on the stage of treatment!

@@ForwardTherapy Not acute (over 4 months) or surgical ,the swelling has mostly settled but pain is ongoing. I was just curious if theres any specific bracing that would target mostly that region and if theres a directional instability that is best to be corrected. I'l give the spica splint a try too

Do they have symptoms of clunking or clicking? Movement is painful or just activity dependent?

@@ForwardTherapy No clunking I am aware of. Movement is painful, especially with pronation/supination.

Hi Kelly- you make that splint look so easy :-) questions: these are pre cut patterns you order- can you provide info on where to get these? I am assuming Northcoast or some other supplier. However, if I want to make these patterns myself what is a good resource to use to get the appropriate landmarks to make the pattern? Also the thickness of the material you used for that splint? It seemed to drape so well- my guess is Aquaplast but can't tell what thickness it is. Also I am wondering- is there a reason the orthos are preferring a radial based ? Why not a volar based thumb spica? Pt more compliant? Less cumbersome? Thank you so much in advance.

They are not premade, we would get sheets of Prism thermoplastic and we had patterns made up so our techs could cut the splints (I worked in an exceptionally busy clinic where we would get walkover splints from the doctor so we needed to make splints as quick as possible). If it helps I can send you a copy of the pattern (a picture of the outline on normal printer paper). The pattern works for most adults and you could just make it smaller or larger if needed. It’s prism micro perforated 1/12 . I actually don’t know the reasoning behind the physician preference, I still sometimes made color based spicas when I felt a radial one wasn’t going to do the job quite right.

And I don't want to ask for too much but if you have a good pattern for a hand based thumb spica I would appreciate it too. I’m going to send you a gauntlet pattern thumb spica (hand based) and a cmc pattern too... often for thumb hand based splints I just take a rectangle with round edges and form it around the thumb without a pattern...but depends what I’m making it for! For UCL issues I usually use the gauntlet pattern (the butterfly looking one, or Pac-Man lol). This will make more sense when I am home and can email you the patterns !
