After meeting a few physicians in my area, it seems that they'd rather you just shut up and take your medication... I was written the most pretentious, arrogant letter you've ever seen by one of these "physicians" about why the HOMA IR test is a bad idea and isn't worth it. He said it made him "uncomfortable" to authorize it. For the first time in my life, I felt like I woke up in the matrix. The reason many docs don't test for IR is because they don't know about this .. the medical industry does not believe in food, and the food industry doesn't believe in health. Many doctors that I've had interaction with are often egocentric with their education, and get indigent with their lack of knowledge on nutrition.... Being a T1 D my entire life, it's been challenging to say the least! Interestinly, the word "physician", actually means teacher, yet many are run by the pharmacology industry, so their TIME SPENT WITH THE PATIENT IS MONEY THEY DON'T WANT TO DO. Sadly, is a business, and HEALTH is not a concern.
Interesting perspective thanks for sharing! I agree that most physicians just simply don't know about this yet. It takes a while for research to work its way into the educational system.
we have come to realize that there is no money in healthy patients. the philosophy seems to be , keep your patients just shy of catastrophe, i.e. chronic illness, for the sake of income!
My Mom got tested for insulin resistance and her HOMA/IR ratio came back elevated. She then discussed the results with her endocrinologist and she said nothing about correcting the diet and lifestyle but suggested that my Mom should start taking Metformin.
@@inquisitivenessandcontempl9918 If your Mom is being treated for Type 2 Diabetes, she can reverse this through diet. If she doesn't, she will eventually have to take insulin thru injections. Look up Dr. Jason Fung, who is a nephrologist in Canada, he has several books, and the one I'd recommend is; The Diabetes Code

My Mom decided to decline Metformin for now and she eliminated refined sugar and flour products from her diet and already lost 3 kg in a month or so. But she finds that kind of diet somewhat stressful because she's been eating high carb, high sugar all her life. I hope she stays motivated to continue on the right track:) In a way, the HOMA/IR test was a reality check for her because before getting tested she would categorically deny the mere notion of the low-carb philosophy.
There should be more Drs like you. I ask my Dr questions about things, and she does not know. I am looking for a more knowledgeable on. I have exercised all my life, stay in a healthy diet and now my sugar level are high. I stay on a low carb diet, about fruts(just berries) and it is high. I went to see a nutritionist and she show me a different way to eat. I have drop 4 pounds even thou I am not overweight but a few pounds are OK. . i am now learning more about insulin resistance and plan to work on that. Maybe that is my issue.....thanks for your information........MINDY

I heard that most IR people have a Pancreas that's producing 5-7 the normal amount of Insulin. Building muscle will absorb this high amount of Insulin. However, the Pancreas is STILL producing Insulin in extreme overdrive. At this rate, no matter how well your body can absorb the extra Insulin, the Pancreas will burn out and stop working. The key is getting the body ultra Insulin sensitive so the Pancreas will return to only producing Insulin at a relatively normal level. That way the Pancreas will not burn out and shut down and the IR is addressed. Otherwise, building muscle to absorb excess Insulin is a very short term solution until the Pancreas burns out and cannot produce anymore Insulin. THEN the person automatically becomes a Type I as they cannot produce their own Insulin.

What sort of Health Care provider or Research facility can I go to If I want to have the 2hr test done? What do I ask for specifically? Would I get the same information from a CGM?
Insulin fasting test, (6min), HOMA equation at (need a glucose result and fasting insulin result) 8:36min. Also a 2 hr test(~9min) for insulin after a glucose drink (30min.....). Mentions Bikkman book at 3:18min. What if have no signs of insulin resistance but learned young not to eat high sugar foods or get shaky . Maybe is a storage problem? SOmeone said they did the insulin test with a meal and not glucose drink as never drink glucose.

OMG your eyes are beautiful! This video is excellent coverage of ways to determine insulin resistance. I wish we could get tests done or even get a CGM without a doctor's prescription. It is outrageous that I need a doctor's approval for a simple sensor that pokes through your skin. What a joke.

My metformin stopped working. I started weening myself off of it my glucose numbers are getting better super slowly. I asked to be put on glipizide ..I’m now down to 1/2 pill one meal..I eat usually 1 or2 meals a day after 1 pm ❤️

How long should a session of resistance training be? How many sessions of resistance training one should do? I don't have access to gym. Are body weight or dumbbell workouts a good example of resistance training? How many sessions of cardio one should do a week?

I wonder how to interpret my numbers....I have type 2 and consider myself pretty well-controlled. Been maintaining my A1C at around 5.1 to 5.4 for the past two years (diagnosed back in July 2018 with A1C of 10.8). I try to avoid sugar as much as possible and virtually no white rice, pasta, breads, cakes (unless it's homemade using almond flour keto flour, flaxmeal, etc). My last blood test back in April 2021, my fasting insulin was 7.1 (report says normal range is from 5-20). A1C 5.3 and fasting glucose tested at the lab was 102. (my usual range at home is about 80-90). I am on just one metformin ER 500 and have lost 45 lbs in the first two years since diagnosed and maintaining it. My doctor had told me a while ago that my body does produce insulin...but it's not enough due to me being overweight at that time (5'8" 195 lbs). So judging from my number...which is true? That my body doesn't produce enough insulin.....or it's producing enough, but my body is resistant? I would think that 7.1 number is better than say....if my fasting insulin were at...say 19-21...and my fasting is 105? Curious of your thoughts. Thanks Doctor!

The Kraft test is a test similar to the oral glucose tolerance test where you would drink sugar then get tested for several hours. You can also go to www.walkinlab.com/ an get an insulin test from a lab, or get an at-home insulin test at www.zivli.com/testing.

I do have PCOS, I’m a small person. I’ve been having episodes of low glucose symptoms even if my levels are normal. At times if I’m not careful my glucose will shoot up. Whatever I eat even if it’s a good meal even my vitamins\supplements I tend to drop and even if I’m in the 90s I feel sick shaky, sweaty, etc etc. even working out my glucose drops… can this be insulin resistance?

Hi. I have gotten lean over time but my weight is about the same (84 kgs/182cm). While following-up with different Doctors, I am also trying to figure out why. Insulin-resistance is something I suspect. My lipid profile is not normal (high cholesterol, low HDL, high LDL). My fasting sugar recently was just about at borderline (first in lifetime). My question is - is it necessary that blood sugar is the only marker for insulin resistance or are there other ways to find out if I have it which is causing all the above issues?
Dr. Morgan you talk about insulin for the risk of diabetes but I want to hear about the impact on heart disease. As I had minor heart attack and got an stent. Please let us know how to reverse heart disease and clean our arteries again.
Great question, there is absolutely a link. Dr. Bikman's book Why We Get Sick would be an excellent read for you. In it he describes how high insulin and insulin resistance leads to heart disease. We may also talk about it in our interview. It has to do with how insulin leads to both inflammation, endothelial damage, and water retention. (insert link to Dr. Bikman's interview here).
Is there any symptoms to insulin sensitivity ? As you compare with the effect of alcool. Like maybe really feeling the insulin buzz. Not sure if I make sense.
Good talk! But was is the ideal homa score then? If 1.5 is borderline IR, we want to be under 1? Also, fasting insulin is very variable due to pulsatile insulin release from the pancreas. Your homa score xan vary a lot! Any solutions/ideas for that?
Great questions, Ignacio! I typically recommend getting fasting bloodwork and not first thing in the morning because cortisol is elevated then and that will raise glucose. Unfortunately the research on ideal insulin resistance scores is a little slim especially compared with glucose levels. I'll add that to my research topic list.
@TheIgnacio777 *The HOMA-IR has been formulated in order to 'normalize' around the numerical value of '1' .* In other terms, the numerator of formula (fasting glucose x fasting insuline) should be around 400 because the denominator is 405. In this case, the ratio expressed by the HOMA-IR is around 1. So slightly above 1 is decent, less than 1 is excellent, 1.5 is borderline IR, 1.8 mild IR, etc.
If insulin resistance is caused by habituation (like in alcoholism or other substamce abuse), then why is type 2 diabetes so uncommon in traditional populations who eat more than 70% carbs? And why does high fat diets cause higher fasting insulin blood levels?
We cannot only contribute insulin resistance to diet alone. Interestingly in countires with a traditional higher carb intake, much of those carbs from from starch, not the added sugar. Starch is 100% glucose that can be metabolized anywhere in the body whereas added sugar is 50% glucose, 50% fructose, the fructose heads straight for the liver and can quickly turn into liver fat and contribute to insulin resistance in the liver. Further, those countries may also have different levels of physical activity throughout the day (better carb tolerance), a lower BMI/better baseline body composition (better carb tolerance), and have healthier meal timing (better carb tolerance). Regarding the fat..fat quality matters. If much of the fat is coming from refined PUFA oils like canola, corn, soybean, etc, those will contribute to insulin resistance more than fats that are not as easily oxidized.
@@drmorgannolte of course there are other factors than diet. I am mainly questioning the idea of insulin resistance developing like alkohol tolerance. that is not at all backed by science. On the idea of PUFAs causing insulin resistance more than SFAs, do you have studies you can reference? Would love to know if there are any studies on unsaturated plant and fish oils driving insulin resistance more than saturated plant and animal fats.
Usually, the reason given for high carb populations living longer or being healthier is because they are more manual labor oriented and lifestyle is labor intensive, in other words they work their keister of every day and burn most of it off. Also they only eat four big Macs with each meal, so, you can see the difference with our society's eating regimen. The part about high fat and IR is a vegan deception! If you are low carb your pancreas is on low idle, no carbs, no extra glucose so, no extra insulin needed. Pancreas keeps small amount of insulin "prepackaged" to be ready if nedded, but, its not ready for some idiot doctor to give you two cans of Coke to drink just to see what happens. Takes some time for pancreas to react and rev back up! No carbs, no glucose no IR!
As for pufas, watch Dr Pradir Jam nada s video" the fat lies, he explains problems with pufas v( omega sixes) vs omega threes out of balance and also lists ALL the studies over the years that plant based medicine says condemns low carb , but, actually shows the opposite results, much to big medicines shame!
@@AI-vs7sm Yeah, that is a well known fact by now, you need to give the pancreas some time to readjust. I agree energy over consumption is a much stronger driver of IR than the ratios of carbs, fats and proteins.

80% of the big people have metabolic disorder....40% of thin people have metabolic disorder. That's every 2-3/10 slim people that are just as sick. Think about it, everyone knows a few T2 diabetics; how many are big and how many are thin? I actually know more thin. So everyone should test for this.

This is a BAD idea for someone who may have reactive hypoglycemia, the 75g of sugar can push them down into the 50s range (unsafe) three hours later. May want to do that supervised.

Dr Morgan , thank you for all your great videos! Iam just little confused still Glucose 82 Fasting Insulin 3.1 A1c 5.3 So I calculated : 82 x 3.1 /405 = 0.62 Am I insulin resistant ?

Becky Gillespie has a video on this. This woman might also, I don't know. She is really good too. Your insulin resistance according to Becky is your waist is getting smaller, you aren't craving foods, etc.

I'm not confident in that test you're talking about. What if the meal is heavy with fat and a portion size of protein with no carbohydrates? The result would be completely different if the meal included carbohydrates, fat and protein. Testing at thirty minutes, in my opinion is way too soon. An hour and fifteen minutes I think would be better.
so, question...after six months on low/no carb diet, why is my fasting glucose still showing 140 to 150 mg/dl. this is odd, because my HbA1c has improved from 6.7 to 5.3 in that time. any thoughts on this?
See the doctor's answer to a previous comment; that might be that if you take your fasting glucose the first thing in the morning, your cortisol is naturally elevated and that stimulates glucose. So your glucose may be a little bit high in the morning only because of the elevated morning cortisol, and your improved a1c may indirectly point to that. But I'm not a doctor so please take my opinion with a pinch of salt. Best of luck!

You are the rare doctor possessing real knowledge of insulin resistance/hyperinsulinemia. Which is quite strange, given that it is ravaging huge numbers of people on every continent, destroying health, destroying lives. I prefer the term 'carb overload', because that is easier to grasp and it points at both the cause - eating too much carbohydrate - and the solution - sharply limiting carb intake. There is going to be a revolution in nutrition in this country and the world. There simply has to be, as the scale of this health catastrophe is massive/pervasive and yet getting even worse day by day. People will eat much less carb/sugar, much less veg oil, much less processed man-made food. It WILL happen. The question is how soon.

“I can’t do your push up for you.” “Nothing works unless you do” - You need to reframe your language. That’s not motivational. It’s actually insinuating that the listener or viewer is lazy or not engaged. It’s the wrong orientation as a therapist or trainer to say those kinds of phrases
