Good information, I was diagnosed with diabetes 2 weeks ago, my A1C is 7.5. the doctor said I have to take medicine metformin 500mg twice a day, I told the doctor that I wanted to try diet and exercise first and that if I did not work I would take medicine. she said no, I have to take now. So I am not taking the medicine and I am exercising and no carbohydrates, I want to try this for 3 months, some doctors just like to prescribe medicine😕

Just got my HOMA test back : A1C = 4.7 Fasting glucose 1 = 83 Fasting glucose 2 = 75 Fasting Insulin = 3.2 HOMA Index = 0.7 [Been doing Mediterranean (75-100g of carbs) + IF + Monthly Prolonged fasting (2-5days) + Strength Training for 2+ years.]

Dr. Sten Ekberg thank you. My brother just recently had a mild stroke and lost his ability to walk unaided due to resulting deteriorated motor control on the left side of his body (i.e. left leg, left arm). His PT said that it is still possible for him to recover with proper care and the right PT. He is type 2 diabetic and was previously regularly administered insulin therapy prior to his stroke. I warned him before about the adverse effects of this insulin therapy such as stroke. After his stroke, he never wants to undergo insulin therapy again. Your thought please. Thanks.

For T2D the insulin test is really the most important test yet it is not part of standard care for endocrinologists, internists or cardiologists. I had to beg my endocrinologist to order the test, which he did, and it revealed my insulin was at 11 even after I had already lost 10 lbs. Now I've lost 30 lbs and still falling at 2lbs a week on a 1000 calorie daily deficit OMAD diet (I'm within normal BMI). Hoping the insulin is dropping. I probably had high insulin for a decade or more but since my pancreas could keep up the BG and A1C didn't show any problems until it was way too late. Now I have to have a triple bypass. Turns out the insulin test costs $28 if you order it yourself, assuming your State allows it. I think it should be added to the standard metabolic comprehensive panel because high insulin seems to be the earliest specific scientific indicator of future T2D and appears to be curable.

If you have cardiac issues, reducing insulin is vital but I would also encourage you to Reduce the deadly seed oils that are so prevalent in most cuisine and America presuming you're in America. Seed oils are deadly and hopefully there will come a time when that is shut down and people no longer are consumed Ming them.

I am the poster-child for your blue chart. I am on Keto & OMAD, but still struggle to lose weight (very slowly). I think the root cause is insulin resistance, but liver health is also a huge factor. This is the most under-diagnosed and unreported medical issues of our time. Whoever figures out how to unravel this knot should get the Nobel Prize in medicine.

Me too, the only thing that works is 36+ hour fasts. I just keep doing long fasts (up to 5 days) and finally losing weight. My fasting insulin is 24, fasting glucose is 100. HOMA score is 6. Thank you for your videos! I am learning so much and have hope again that I can get healthy.

I had my blood work done a week ago for my annual physical and I had asked my doctor to include a fasting insulin test so I could calculate my HOMA-IR score. My fasting insulin was 1.2 and my fasting glucose was 69 making my HOMA-IR score 0.4. That made me happy and my doctor was happy as well. At least I know I'm on the right path.

I have been trying to improve my wife's IR for a few months now and things are improving slowly. My A1c was also nearly pre diabetic. We both did some fasting and my weight came down 10kg fairly quickly. My wife has lost some weight, but needs to lose some more. I am fairly sure that my HOMA IR is now normal, but my wife's remains slightly raised, and we will continue working along the lines that you mentioned. Thank you for your videos, they are extremely helpful and not many doctors in England have heard about it either. I am a retired GP, and my wife has Altzheimer's. Wow! Thank you for this video. On Sept, 27, 2018 (when I had my Stemi heart attack!) my weight was ~265, my BG (not fasting) was 238, my A1c was 10.1 !!! Holy cow! I freaked out and started doing my homework and changing my life. Today, my weight is ~212, my (fasting) BG is 99, my fasting insulin is 4.7, and my A1c is 5.2 !!!! Using your calculations my Homa-IR is 1.15. I can't believe I did it. And will keep doing it, by the way. :) After the MI (and especially my research) my one and only goal was to improve my insulin resistance. Not even reducing my weight - that just happened on its own as a great by-product. I've been like Ben Bikman says, "every time I put something in my mouth, I ask myself what that will do to my insulin." LOL Kinda OCD, I know, but it's working! I am keto/carnivore, OMAD (mostly - I've learned to be aware of my body, and if it's really hungry I feed it - very low to no carbs) and occasional 48 hour fasting - but not much, to be honest. Your Homa-IR info really helps me know I'm on the right track. Thank you again! Cliff

I sent an email to my Primary Care Physician requesting that he order a HOMA-IR test for me. I received this response back from his Nurse Practitioner: “I have never heard of this test. I will send an e-consult to your Endocrinologist about it, and get back to you.” “I spoke to your Endocrinologist. She notes that the HOMA-IR is for research only and has no application in management of patients. Even with a result, she is not sure what we would do to change or adjust the management of your diabetes.” My response: “Okay, I will go elsewhere to get this test.”

Or do this: Ask MD for your fasting glucose, and ask for your insulin level...do the calculation yourself for HOMA-IR = (Fasting glucose x insulin number) divided by the constant 405. Dr will order the insulin lab, even if you have to pay for it out of pocket. Come back to this video and compare your number to Dr. Ekberg's examples to find out where you stand with insulin resistance level...and take his advice to lower IR.

NEVER too late to learn something new ( unless you’re dead). I’m 72 and I guess I’m a late learner, but not too late. I’m requesting this test on my own. my local lab charges $59. Well worth the money. My PC orders basic panels at my yearly, but I’m not so sure my insurance would cover it and I’m not waiting for 6 months. Thank you DR for all your information.

I'm a clinician in traditional medicine and I must say you are the only one that explains IR, IF, weight loss and IR effects on autoimmunity, etc., that is simple and yet comprehensive. It is just terrible that our traditional MDs won't teach this nor do insurance companies cover homeopathic medicine. Unless individuals say no to pharmaceuticals and seek medical truth, our lives will never change for the better! I am hoping that changes. I know I teach it when given the opportunity. I wish I had this information 24 years ago. Im 58 yo male diagnosed with hypothyroid after Graves disease, then diabetes, low testosterone, and high estrogen. Over the years I gradually gain 100 pounds. I was told by many doctors that I should accept being fat. It was in January that I started, keto, exercise, and fasting. In combination, I lost 29 pounds today, May 24. My journey continues. Thank you.

I asked my Kaiser Doctor for the HOMA-IR test and he had no idea what that was! He checked with the lab and they also never heard of that test. Scary!!

I have Kaiser as well. My doctor has type 1 diabetes and is overweight. My a1c is 5.4. Said I dont need a c-peptide but ordered it for me. I'm getting the test tomorrow fasted. I figure at this point, I need to educate the doctors or at least get them to work for what I need. He said they normally to dont provide meds until blood sugar reaches 150!!! Are you kidding me??? I'm trying to reverse type 2 diabetes. Good luck with your journey. Keep asking for what you need.

Update! I found the way to do HOMA-IR. I ordered two tests on walkin dot com: fasting glucose $26 and fasting plasma insulin $27 - no doctor prescription needed. When I got the results, I calculated my HOMA -IR 2.1 - early insulin resistance. I've been on keto diet for three week, and my fasting glucose dropped from 110 to 91. Wow! I am sorry I did not do insulin test before keto.

Ty! You have unlocked the puzzle for me! I’m in that last example, where I’m fast adapted but still IR. I also have hypothyroidism. I try to fast and I’m consistently on a 16-8 IF routine. But nothing happens for me for a month at a time. Now I understand. Again TY! ♥️

I know I’m number 3. I have been insulin resistant for a long time. I have lost 23 kilos in 18 months, but now have not lost any weight in months. I’ve been doing 18 hours fasts, daily. Eating a meal at 2 pm and 7 pm. Nothing else but water and coffee and tea with cream. I am waiting for bloods that were taken a week ago, had fasting glucose done, but will talk to dr about you’re extra test, see what she says. You make everything sound so clear and easy to understand. Thankyou. I will tell you my numbers when I find out.

Hello Dr Ekberg. Following your lecture of Homa-IR test, I got tested and got this result. It seems to be below your range. Don't know what to make of it. Fasting Bood Sugar(FBS)(HEXOKINASE)= 89.02 mg/dl; Insulin Fasting (ECLIA)= 2.28 uU/ml. Calculating HomaIR = 0.498...is this good or bad? Guess I am NOT Insulin Resistance? Hope you would enlighten me. Thanks and God Bless.

Got my HOMA IR test after watching this video, here are my results, can somebody please help me interpret it. I am confused, if I just use the formula ((BG*Insulin)/405), I get whooping 5.17 whereas in test result it says 2.99. I cross checked it with HOMA 2 calculator from oxford university, it says 2.99 as well. So, which one is it. Glucose 88 mg/dl Insulin 23.8 µU/ml HOMA2 %B 214.2 HOMA2 %S 33.5 HOMA2 IR 2.99

This is the exceptional! I am case #4 with fasting insulin at 20. Always keto for the last three years. I did 5 day fasting and lost 80 lbs but went keto OMAD and gained it all back. My insulin is simply too high! Even I fasted for 8 days which was a struggle since I was light headed and felt bad my insulin refused to drop and only went to 6. Now fasting is much harder. I actually think fasting raises insulin because of high cortisol. It does not resolve or fix insulin resistance. So what does? What is the secret? I am now experimenting with melatonin 24 and 36 mg at night to drop my insulin at night during my 5-6 day fasts. Eat only keto on refeeding days and skip melatonin. Also exercise as you said. The melatonin is making fasting easier. As I transitioned to using melatonin at 12 mg per day with keto OMAD I actually lost weight for the first time ever. I hope this is the secret for me!

This brings together everything I have been trying to understand with diet, IF, excercise, testing BP, testing Blood Glucose, etc. The only thing missing was what units you and other doctors were refering to as they are often hot mentioned against numbers. I found that elsewhere and now I feel in control of reversing my Type 2 diabetes diagnosis.

What a wonderful presentation. I’ll be having my first IR measurement tomorrow. Although my fasting and my A-1C are quite good, I feel I cannot be sure how well I am doing until I see my fasting insulin numbers. Your video has provided a clear explanation of exactly what is involved and how and why we, and our doctors can be misled by seemingly-normal BG and A-1C. readings.

Thank-you very much as a 50yr+, 80kger with a history of an unhealthy relationship with food I decided when NZ (where I live) went into lock down I would address this. In the first three weeks I lost quite a bit of weight power walking 7 days a week/ 2-3hrs for 3wks. But, for the past 2 wks I've notice no real difference in my stomach & powering up more hills has just been leaving me feel fatigued. However, in watching your video on Cushings I can now see why. I'm doing the wrong type of exercise & probably eating way too many carbs. I plan to educate myself by watch all your videos so as to make informed choices. Thanks for the science behind my issue.

Absolutely Excellent, I was on keto for 9 months , lost 55 pounds and then plateaued for the last 3 three of the 9 months, started Intermittent fasting and found it real easy to do. Went to OMAD and started losing weight at a steady 3 lbs a week. I exercise 5 days a week with easy swimming 3 of the 5 days and weights in the other days. Bulletproof coffee in the morning. I think you are spot on, I feel great!

Any progress? How's it going the last 3 months?I think I'm in the same situation.I can't seem to loose any weight even though I do intermittent fasting and lowered my carb intake.and my a1c dropped from 9.6 to 7.1 and my average sugars reading for the last 6 weeks is 131

This is a very insightful video. It would be really interesting if you could explain the effect or interplay of cortisol on insulin and possibly the access to fat stores or even weight loss? Especially in the case of exercise, like higher intensity or longer duration training sessions.

Definitely good information. My doc wouldn't do the fasting insulin test so I paid for it myself. I'm at 11. Fasting blood sugar is 99. So that would put me at 2.6 Not good but I know what to do. And im slowly making progress. Thanks Doc. Keep up the good work! Cheers!

I have been trying so hard to lower these numbers but haven't found anything that works... would you mind sharing the things you have tried as you work on lowering yours? This is a great and very usefull and informative video DSE. For anyone in Australia or other countries that use mmol/L, the constant will be 22.5. eg. FBS 9, insulin 15, constant 22.5 -> 9x15/22.5=6

You described my situation to a t in the very insulin resistant scenario. I lost 40 plus pounds. My belly is still big and I get tired easily and still feel like eating. Getting my first c-peptide test on Monday, and that was a struggle to get, my doc does not think its necessary. I have had one 36 hour fast and it was torture; I was very hungry all the time. I will add more fasts but it's a tough fight.

This was brilliant and eye opening. You have described me to a tee as another person wrote. I know I need to get all 3 tested and then a few other tests as well. I have Hashimotos antibodies. Love your videos. Do you suggest walking ALOT, like 4 to 6 miles a day? I know insulin sensitivity improves with exercise. Nobody talks about that though!!

Dr. Eric Berg is great in summarizing the whole idea and make you understand what is going on. but when it gets to the details and a very deep understanding no one competes Dr. Ekberg. in this video, you are addressing my problem 100%. I have been on keto for the last 2 years, and I lost 34 kg (75 lb) in the first year, from then I'm struggling with the last 10-15 lb all concentrated in my belly. I did the fasting insulin test a year ago it was 12.5 and the fasting glucose was 90. according to the above equation, my HOMA-IR is 2.7 which is not very bad but now I'm sure my problem is still the insulin resistance. I will exercise and fast longer and ill search where I can do the tests for toxicity and thyroid. thank you so much, Dr. Ekberg

I'm in the last category. I have always been slim but my belly is huge compared to the rest of my body. I don't eat much and i fast. I must admit i don't do excercise though because of painful joints and laziness. I will ask my doctor for the test and take it from there. Great informative video.

Great info, thank you so much. I'm going for my 2nd A1C in less than 2 weeks. Had a fasting blood glucose result of 8.2 about 4 months ago and I've been on a weight loss journey ever since. Dropped 36 lbs. and my 1st A1C about 2 weeks after the fasting test was a 6.4. I fully expect my blood sugar to be down this time and I'm also getting my 1st glucose tolerance test. But no mention was made of checking my insulin levels. I do still have belly fat though it's dramatically reduced in the last 4 months. I'll certainly ask about if this homa ir test is something I should get as well.

HbA1c is not as reliable an indicator as HOMA-IR. For the past 6_ years my HbA1c has been around 4%, but 2 years ago,my HOMA-IR was 2.2. That was when I increased my fish oil supplementation 1g + of EPA and DHA. Now my HOMA-IR is 1.3. I have read that Omega-6 fatty acids degrade Insulin Receptors on the cell membrane, while Omega-3s can repair them.

Thank you for this! I think I might be that 'special case'. I have never dieted & have been skinny my whole life. 10 years ago while pregnant and addicted to Pepsi was told I had gestational diabetes. I quit the Pepsi, bread and pasta only while pregnant and afterwards went right back to it. I have always eaten one meal a day just because of my work schedule and I would crash immediately after eating. I got the Mirena put in 7 1/2 years ago and in the last 5 1/2 years put on close to 50 lbs. I use to always be hot and now I am always cold. I started taking my temp. daily before getting out of bed and found my core temp. running 97 degrees 6 out of 7 days. I was also a smoker for the last 30+ years. I quit smoking March 19, 2019 & knew I did not want to gain anymore weight so I started Keto May 3, 2019. I lost 9 lbs the first 21 days, felt great and had a ton of energy. I started tracking my glucose levels. I am at pre diabetic levels most days. (low 100s 17 hours fasted). I decided to get the Mirena out on June 24, 2019. Even though I'm 49 I am now having a regular period so not in menopause, YET! Then all of a sudden my numbers started going the wrong way. I would wake up and weigh in and be up a pound or up 2 pounds. I am swelling up and puffy and my muscles feel so weak sometimes it's hard to lift my legs! And today I step on the scale and I gained another 2 pounds over night and have gained back ALL I lost even though I'm eating 1400 or less calories a day. To the point where some days I feel like I'm starving but I refuse to eat because I want to stay fasted to eat my body fat but it isn't!! UGHHH I take Potassium, Magnesium, Calcium, Zinc and salt every day but I'm having bad cramping at night. I do not cheat and most days am under 3% Carbs and around 80% fat. Between the quitting smoking, quitting carbs and getting the Mirena out have I just shocked my system too much too fast? Or is something really wrong with me? I can't take the weakness in my muscles anymore! Help!!! Thanks!! XO

I am a 64 year old male. My height is 5' 10". I've been doing low carb for about four and a half months. Pretty quickly my weight dropped from 185 pounds to 165 pounds, but then it plateaued. I have tested my blood glucose a few times, and it always seems to be about the same. The lowest reading I've gotten after a 16 hour fast is 99. The highest reading I've gotten about an hour or two after eating a meal is 107. Since going low carb my energy levels seem to be perfectly consistent. Before going low carb, if I ate ice cream or a shake I would have a bit of a sugar crash after an hour or two and feel extremely drowsy. I drive a truck over-the-road and the vast majority of my time is taken up so I don't have much of a practical opportunity to exercise. At least my BMI is now in the "normal" range. I will experiment with one meal a day and see what happens.

I haven't been able to find a description of the process of doing the Homa IR, ie. is it like the OGTT, requiring multiple draws over times with the glucose drink, or is it just one draw from a fasting patient?

It is a single draw of fasting insulin and glucose. You put it in a formula to calculate your score. OGTT is expensive and invasive.

Thank you doctor for your videos! Just wondering if you could provide some information on lowering cholesterol ? I'm doing Keto/IF and so far I've lost 20 lbs. I'm 6 ft and 185 lbs so not overweight but my cholesterol is slightly elevated. My doctor suggested I take 10 mg of Crestor and it's lowered my cholesterol but I would love to stop taking a statin if possible. I have no other health issues. Thanks again for all your advice!!

The ratio of Total cholesterol/ HDL is the best basic indicator. As long as it is around 3 you are fine. In my opinion a cholesterol of 220-240 with a HDL of 70-80 is ideal. It is neuroprotective. Reduces the risk of dementia.
