My mother was just diagnosed with 1b breast cancer. It’s terrifying, but at the same time I remind myself that out of all of human history there has never been a better time to have this disease. This was once a death sentence, but now there is treatment, information, community, and counseling. There is hope. I will pray for all of you as I hope you will pray for my mom and I. 🙏

Thanks for sharing the real truth; watching this video has prepared me for my upcoming appointment with my oncologists; I anticipate that they'll share this shocking of the ovaries treatment, which sounds completely devastating. I now understand why conceiving would be hard after such treatment.

Juicing mostly 80% veggies helps, use a slow juicer…. Celery juice… ginger and turmeric are anti inflammatory , juice both with a green apple/golden apple plus lemon or lime….

Really helpful information Dr. Grigg. My question is what do you do when you have an Oncotype score of 29 and a Mammaprint indicating low risk on a Stage 1, Grade 1, 8mm tumor with no lymph node involvement and ER/PR positive and HER2 negative in regards to chemo? Would you have recommended chemo or no chemo? The oncotype was the outlier based on my clinicals and mammprint result.

This sounds like a difficult situation. The 21-gene assay (Oncotype) has the most data supporting its use, so I can see why this is perplexing. At this point, your preferences play a role in deciding the next steps along with the guidance from your medical team.

@@yerbba I continued my research and learned that the Tailor X study did not include tumors that were below 1.1cm unless your tumor was intermediate or high grade for which they did study 0.6cm and above. My tumor was Grade 1. So given my specific grade and tumor size was not part of the research, most doctors have indicated they would not have ordered the Oncotype for me. So given my tumor characteristics were not part of the study, my Dr and I agreed we would not do chemo.

Decisions about radiation therapy are based on the risk of locoregional recurrence. In someone who had breast conserving surgery (lumpectomy), radiation is almost always recommended with few exceptions. Chemotherapy recommendations are based on a number of factors, including the size of the tumor, the hormone receptor status, the HER2 status, and, in people whose tumors are sent for genomic assays, the results of those assays.

Hello Dr. Grigg. Unfortunately my mom has been diagnosed with HER2 + and has been set to take the Enhertu chemotherapy. She’s still in the first stage. Do you recommend that she starts with that chemotherapy ?

Hello! Your mother is fortunate to have you. With HER2-directed therapies along with chemotherapy, the chances of disease free survival are as good as with people whose tumors are HER2-negative. So while I don't know things like the size of the tumor, the grade, the hormone receptors, etc., I would encourage her to follow the recommendations of her medical team. Most people tolerate HER2-directed therapies like Enhertu very well.

The recurrence rate is about half of what it would be if this were a non-mucinous carcinoma. The prognosis is very good with mucinous cancers. It is hard to make a specific estimate without knowing other things such as the size of the tumor and the lymph node status.

Mayo Clinic has newer radiation procedures. Find that video. More agressive at first, every day, and over in 3 weeks. Also beginning studies of proton use.

Yes, a lot of centers around the country that are giving what is called hyperfractionated breast cancer, which is just as you describe. It is not possible for everyone to find a treatment center that offers hyperfractionated radiation. At this point, proton therapy is reserved for people who have already had radiation therapy and need treatment again in the same area or as part of a clinical trial.

In treatment now for triple negative, taking Taxol it's effective but the side effects are crazy , I'm pushing through them 5 more treatments to go them on stage 2 of treatment.

Not everyone needs to have lymph nodes examined. For older people with small tumors, omission of lymph node assessment may be appropriate.

Dear Dr. Grigg. I have been diagnosed with Stage 1 ER and PR positive in both breasts. The one in my right breast is 9x5 mm and the one in my left breast is 6-7 mm. The radiologist recommended surgical biopsies in two additional places in my right breast. I have had MRI and US guided biopsies in both breasts and the radiologist wants me to have two additional MRI biopsies besides the ones I had. They are scheduled in two weeks. The surgeon said she could either do lumpectomies followed by radiation or mastectomies in both breasts. She is not making recommendations, she said it is my choice. What is your opinion on my course of treatment?

my mother is 78 had a lump remove from her breast was then told by consultant that it had gone to lymph nodes A sample of some sort has been sent to the USA for i guess a very detailed analysis. Chemotherapy was mentioned as a possible treatment. I was out for a walk today with her and she is the picture of health. Inside i am very worried about what is coming down the line.

Stage 1. Had a mastectomy of the right breast. Her2- but one lymph node came back positive and one negative, so now I'm being recommended for chemo and radiation as a preventative method....

I mean is there a necessity for double mastectomy when diagnoses of triple negative tubular cancer or ductal carcinoma grade 2 stage 1 but there is a mutation in BRACA2?

Not everyone has to have a preventititve mastectomy if they have a BRCA2 mutation. Close clinical following and breast imaging are another option.

Did i get it right, if ER/PR negative - most likely to be offered chemo, but if ER/PR positive - might require hormonal therapy? What’s a good news between being negative or positive? :)

Great question. In general, tumors that have ER and PR are considered more "favorable." If ER/PR-negative, chemotherapy is recommended (depending on the size and other medical problems of the person with cancer). If the tumor is ER/PR-positive, hormonal therapy may be added after chemotherapy, or chemotherapy may be omitted altogether. There's substantial overlap in the recommendations, and it depends on the size of the tumor, the lymph node status, the tumor grade, and results of a genomic assay if one has been done.

hello I have a question I had stage 1 breast cancer surgery on august 11 and had the tumor removed then after I had radiation which I did great but not the dr out me on verznio and another pill and I have been so sick with diarrhea which doesn't stop not even with Imodium. I went to the ER twice because it was really bad. I stoped medication a week ago tomorrow which I only did a week of it and still have the diarrhea. I am so scared because I really do. not want to try another med. my percentage for cancer coming back was 4 percent.

I was diagnosed with Stage 1 breast cancer and I had lumpectomy surgery. My lympnode tested negative but the lump which is 3 cm in size is aggressive. Now, the Oncologist is recommending chemotherapy before undergoing Radiation therapy. Is this normal?

This is a good question. Although the lymph nodes were negative, the "personality" of the tumor is a critical driver of prognosis and therefore treatment. While I cannot comment on your specific case, chemotherapy for a tumor this size with this personality is standard. And yes, we do give chemotherapy before radiation therapy. It sounds like this is not what you were expecting, and that can be hard. It's my impression that you are receiving the best care.

My Mom is suffering from breast cancer. Her pathological report is ER, PR is positive and Her-2 negative, Stage 1, Grade 2, and Tumour size 1.5 cm. Lumpectomy and Radiotherapy with Auxila are done. No SLNB and Auxila dissection is done. Now hormone therapy ( Exemastine) is continued. Are we on the right track? Can we skip hormone therapy after taking 4 months because of side effects?

The hormonal therapy is an important part of your mother's treatment plan. If the first medication is causing unmanageable side effects, trying one of the other aromatase inhibitors and, if that is no better, trying tamoxifen would all be reasonable options.

We generally do use chemotherapy to treat triple-negative breast cancer, yes. Radiation therapy will depend on the kind of surgery and the size of the primary tumor. There are some people with exceptionally small tumors (a couple of millimeters) in whom we do not use chemotherapy even for triple-negative cancer, but it is pretty uncommon to find a tumor that small. Your mother is fortunate to have you looking out for her and learning about what she might expect.

HGDCIS ER PR -ve HER 2 +ve, few sub mm foci of IDC present in mastectomy specimen. Margins clear SLN excised all clear. Does she need to take Herceptin?

In people offered the LHRH analogs for early stage breast cancer, the clinical trials have generally given these medications for 2 to 3 years.

My mother is set to receive her last of four rounds of adjuvant chemo. Her diagnosis was Stage 1A Solid Papillary carcinoma. Her margins were clear after surgery and she had no lymph node involvement. The reason she even had to get chemo was because her oncotype score came back at 35 which meant she benefitted greatly from chemo. Right now, doctors are telling us they’re 90% sure she’ll be clear at 10 years. Should I still be worrying about that score?

Thanks for writing. We are not entirely sure how helpful the Oncotype score is in a person with papillary carcinoma in terms of prognosis. Statistically speaking, she has an excellent chance of having been cured. She's fortunate to have you as a daughter.

Although we cannot give specific advice to an individual patient, in general lumpectomy is followed by radiation therapy. For a TNBC, we generally do recommend chemotherapy as long as the patient is relatively healthy and is willing to accept chemotherapy.

Thanks for writing. There are many things that play a role in your treatment options, including the size of the tumor and the number of lymph nodes involved (if any), the estrogen receptor and progesterone receptor status, the HER2 status, and the grade. In people with zero or only 1 to 3 positive lymph nodes whose tumors are positive for hormone receptors and negative for HER2, additional tests of the tumor can be done to see if chemotherapy will offer any benefit. Hi Dr Griggs, I am 48 years old and got the breast cancer 8 years back (at 40 yrs old) at right breast with chemo and radiation done. It is hormone positive. However, 8 years later, i am diagnosed with breast cancer on left breast, which the tumor size is 8x7x5mm, ER & PR positive, lymph node negative, histologic grade : 2. With this, do you think chemotherapy is needed ? Appreciate your comment.

Although we can't give specific advice without being part of your medical team, we can tell you that, if the lymph nodes are negative, a tumor genomic assay (such as the Oncotype DX) can help determine the benefit of chemotherapy (if there is a benefit or not). Wishing you the best.

Many (most) people with the tumor characteristics you describe do not need or receive chemotherapy. It may be helpful to have the primary tumor sent for a genomic assay such as the 21-gene assay (brand name is Oncotype). This will reveal the extent to which chemotherapy will improve outcomes. The grade of the tumor, which you don't describe, can also be helpful. High grade (grade of 3) tumors tend to have a high recurrence score on the 21-gene assay whereas a tumor with a grade of 1 tends to have a low recurrence score on the 21-gene assay.

Hello dr Grigg, I met with my oncologist and I have stage 1, ER positive. Nodes and margin clear and negative. Recommendation, hormone therapy, radiation. Is 6 weeks of radiation the normal length of time? Is hormone therapy absolutely necessary? I'm anxious about the side effects. I'm a worker, go getter, and a busy person. I want to take good care of myself. Is there something else I should be asking for? Your videos have helped so much. Would a integrative oncologist be good to consult?

Thank you for writing. There are forms of radiation therapy that are given over a shorter period of time. If your radiation oncologist does not bring this up as an option, it may be because there are specific criteria that need to be met in order for this to be safe and effective. In terms of your other treatments, many things factor into treatment decisions in addition to the stage and hormone receptors. In general, we do recommend endocrine (hormonal) therapy for people with tumors that are estrogen receptor-positive if the tumor is over 6 mm. As you can tell, that's quite small. Endocrine therapy decreases the risk of cancer coming back in the same breast, in the other breast (a new cancer) and in the other parts of the body sometime later down the road.

I was recently diagnosed with breast cancer again. This time triple negative. Last time, lumpectomy and radiation. This time I had a bilateral mastectomy. Same breast. Final diagnosis after surgery was no malignancy in auxiliary lymph nodes on right or sentimental on left. No malignancy in any breast tissue. It hadn’t begun to spread. The tumor was 7x4mm in size. Very small. Is chemotherapy still necessary? They got it out with surgery. My oncologist still wants chemo due to the type of cancer.

Triple-negative breast cancer of that small size is most definitely associated with a good prognosis. We understand that it can be scary, however. Chemotherapy is an important part of most people's treatment plan if the tumor is greater than 6 mm in any dimension. I hope this is helpful.

Sounds so similar to me. Second time around. First was hormone positive grade 1. This time triple negative grade 3. Small 6mm x 5mm x 3mm. Clear margins, 4 nodes taken, all clear. Oncologist says chemo but it just isn't sitting well with me. Did you do the chemo?

@@judybee6698 I did! Taxotere and cytoxan. It was brutal! Hopefully you’ll get lower dosages than I got. Triple negative is so aggressive they had to hit it hard. 4 treatments every 3 weeks. I’m so glad it’s over! And I’m growing hair! 7 months post chemo and it’s about 2 inches. Sending positive thoughts your way 🫶🏽🌷

@@judybee6698 Communicate everything with your oncologist team. They give you meds to counteract side effects. Go in for fluids and anti nausea meds if you need to in the days to follow. It helps a lot! Nothing will taste right. For me bacon, green olives, V8 juice and beef broth was the easiest to taste. If you run a fever, tell them quick. White blood count will drop. They can give you a booster shot for your WBC level to not drop too low. You will be tired. And ache. Rest as much as you can. I’ll be praying for you!!! 🙏🏽 You’ll get through it too!

hi! my mom is just recently diagnosed with stage 1 cancer. Should i stop taking soy supplements as well for prevention? im doing resistance training and so i needed to increase my protein intake that i am taking from whey protein supplements. Thank you for your information.

Hello! Thank you for asking. Eating whole-soy foods (for example, soy sauce, tofu, soy milk) as part of your normal meal is okay. In fact, there is some evidence that having a soy-rich diet may reduce the risk of breast cancer. If you do enjoy soy-based food, we recommend that you not have excessive (more than 1 or 2 servings per day) amounts. We also recommend that you avoid soy supplements (such as tablets, shakes, or powders) because they may interfere with your treatment. Resistance training can be a great way to stay healthy and reduce your risk of breast cancer, especially when paired with a good diet and cardiovascular exercise. I am being given "targeted radiation " and they never put tattoos or marking for the radiation. I am only getting 5 treatments every other day. Is this common as I haven't heard anyone mention this plan. 😢

Every other day, highly targeted radiation therapy, also called stereotactic radiation, is used to treat small areas, such as the eye, liver, or lung when these areas have cancer. This type of radiation therapy is not generally used to treat the breast itself in people with breast cancer.

Chemotherapy decisions in your case are difficult. The OncotypeDX score of 20 may indicate some benefit of chemotherapy depending on your age. Let your medical team know about all of your questions before you make a decision.

Ask your doctor to also run the RSClin model for you, which takes your tumor characteristics and your Oncotype score and calculates your risk with your clinicals included. It is definitely worth doing, and the Dr can do it in their office in a couple of minutes.

Thank you for watching. The data on cognitive function and estrogen is fascinating. We used to say that our cognitive function declined after menopause (when the ovaries stop making estrogen), but there are a number of studies suggesting that cognition actually gets better after menopause. Clearly, each person is different. We recently uploaded a video about the role of estrogen in breast cancer!

I had stage one grade 2 breast cancer, with microinvasive, is that a bad one ,thank you, and when thay were doing the biopsy it took all the caner out so when thay done the test on the lump there was none in it ,and how bad is precancerous cells ,😅i took arimadex for 10 years ,

You've been through a lot, and it can be so hard not to understand all the details of your cancer report. In people in whom the cancer was all removed with the biopsy, the recurrence rate is low. The endocrine therapy makes it even lower. Precancerous cells are also treated with endocrine therapy, and the risk of progression is much lower than with true cancer cells. We hope this is helpful.

Good point! It appears to have something to do with cytokines (molecules within cells) that are released from our body's cells during radiation therapy. It would be wonderful to understand everything that happens within our bodies even without radiation therapy, but much of how our bodies work remains a mystery.

@@yerbba The immune system is compromised and causes fatigue, the very thing we need to prevent disease. It is a vicious cycle. A study found that the mortality rate of non-radiation patients and radiation patients were the same. I think radiation is not a necessary choice for every Cancer patient.

@@OceanSound100 The easiest answer I have always given is that chemo is a toxin, just like antibiotics are a form of toxin. If it were not toxic to something, it would not kill it. If antibodies were not toxic to infection, infection would live on. If chemo was not toxic to cancer, the cancer lives on. Therefore, toxins make our immune system tired from the fight they have to do along with the chemo (or antibiotic). Imagine being in the ring with two different boxers and trying to keep up with both, one good one and one bad one and they are fighting one another and me.

I'm shocked to learn that mucinous cancer=no chemo.... since I went through chemo with that type of cancer, stage 1!! How is this possible?

It's not clear how decisions were made in your individual case. In people who have positive lymph nodes with mucinous cancer, some people will be offered chemotherapy.

I have another perspective. Soy products contain Isoflavons, which are the same as the building blocks of Estrogen. It doesn't matter whether they are from a plant source. If I or a family member were diagnosed with (or had a family history of) breast cancer, I would totally avoid soy products because it is not worth taking even the infinitesimal risk. Cancer cells feed on Estrogen, so why take a chance?
