How to undertake a suicide risk assessment in an inpatient setting

This educational film is in 2 parts showing poor practice and good practice as a nurse attempts to assess the potential risk in allowing a recently admitted patient to go out on leave.
As you watch the first part, you can identify the problems in the approach of the nurse and psychiatrist. In the second part, focus on the different skills, verbal and non verbal, that the nurse uses as she tries to understand William and explore the potential risks of agreeing to his request to leave the ward.
If you want to read more about the subject matter of this film, you can try "A Pocket Guide to Risk Assessment and Management in Mental Health", by Chris Hart and published by Routledge. It's available as an e-Book, on Amazon and in book shops.

This demonstration made me very tearful because of how William felt having no hope and wanting to end his life. The empathy and care from the nurse is very comforting. I have been in Williams situation many times and still do now from time to time

Done very well. Choose your words carefully, speak in a low slow voice with no interruptions. In a quiet room, no traffic, phones, the room must not have an echo...

Dear God! Is this supposed to be an example of how it should be done? I am suicidal and if any jerk of a doctor refers to my suicidal plans as “doing something silly”, it is indicative of how little the idiots understand or care. Poor man. What patronising fools.

I wish someone was serious as this about my son’s care! It would have saved him. I feel so helpless I lost a wonderful son from being happy in a matter of months! Healthcare is not the same in the US 😞💔 It is difficult to get a clear direction from the doctor. I had to fight to get therapy counselor help for my daughter after my son passed. It is inhuman that we just work day and night and no one cares !!

the thing i really hate is that they treat you like a child and the second you cant pay they kick you out and you have to act like an adult after they have completely torn you down and treated you like you were worthless how about treating you like an adult and not taring down in the first place its alot easier to rebuild your life
@milesstyles7428

What do you mean "can't pay" also you come across as being hard work, sometimes you have to help them in order for them to help you better.
@milesstyles7428

I really don't think you should be advising "you should avoid these places at all cost" because when discussing mental health issues you have to bear in mind the people who will probably be reading these comments, because what you're telling them to avoid could possibly be the exact thing they need the most.

@@milesstyles7428 when I was 16 I attempted suicide and when I was in the ER, waiting to hear if I could get into a psych ward without insurance, all but one refused to accept me. And the only reason I was accepted was because my father had to sign up for medical insurance immediately. I was lucky to have my own insurance for my last attempt last year so it was never an issue. It's sad but a lot of places won't even accept you if you don't have insurance and you're forced back into the world. I do agree though that it's harmful to say people should avoid hospitals even if they struggle financially. Hospitals can often work with patients to find Healthcare for them as long as the patient tell them about their struggling. I don't regret ever going to the hospital and have been 3 times. About to go back.

@@TheyCallMeN well in my opinion that is mature reasoning considering you're still quite young, you have time on your side & you can turn this around & you can go on & begin to get some enjoyment out of life. OK they're just random words from a random guy. It doesn't mean they're empty & it doesn't mean I'm wrong. I fully understand what Its like to think 'I cannot go on' But I'm still here going on... I'm guessing you're American because how you mention health care/insurance & so money must come into the equation. I'm from the UK & our national health service (NHS) is free (tax payer funded) we are blessed. I cannot imagine it differently. I sincerely wish you the best & hope things start to improve for you, you are not alone. Miles.

@@milesstyles7428 thank you so much. It really does mean a lot to me and I will be better soon..I will be making really big changes in my life within the next few months which will be a blessing to my mental health. Even if it doesn't cure me, which it won't, I will be happier. I do live in America and there are many different types of insurance. Usually the lower income makers can get Medicaid which is free but limited. It covers everything I've ever needed, though. I have Medicaid that is based for my state and actually omits me even more than normal Medicaid. There are other insurances that people can get from work that have ridiculous co-pays. My SO has been unable to see a doctor because the insurance he had as work would force him to pay $1,000 just to be seen by a doctor. My old clinic, it was $50 without insurance each visit. The Healthcare system is rigged here, unfortunately

Last year I went to the psych unit in my town after a failed suicide attempt and the entire time, one of the therapists (not even mine, but she knew my case) kept saying I was in horrible condition and that I would be "clean" soon. Kept telling her I wasn't on drugs (I didn't even attempt with an OD) and she kept saying "suure" and rolling her eyes at me. I finally snapped once and told her to check my clean fucking tox screen. She refused and was extremely arrogant. My own doctor there gave me medication that was extremely harmful to my unborn child and I ended up miscarrying. But that also doesn't mean all staff I've encountered was bad. The nurses were almost always great and the ones that weren't were reported.

High risk - should not be allowed out. Interviews are one on one with nobody else present. Is that safe? Would be great to hear the clinician's risk assessment.

thanks for this video, but isn't it true that a solution to a problem is a good thing? and suicide happens to be a "permanent" solution. wouldn 't a permanent solution be the absolute best possible outcome for any problem? I believe it would be.

Axel Martin a problem is subjective. Gor some people dying is a problem and you being dead is a problem. You dying would create a lot of other problems for them. I hope you're not speaking about yourself
@elgrenas9606

The whole point is to keep the patient alive because while suicide is a permanent solution, it's only a solution that is believed to be needed because of how drastically emotionally distressed someone is. Most people don't want to end their lives, they want things to become better and by living life. I was suicidal everyday and it took a toll on my psyche, I was completely delusional and mentally unstable, but after medication, therapy, and life experiences I feel like a new person. Sure I now and then get depressive episodes, but I am strong enough to live through it. Suicide is not the answer.

He has a brother. He has family. His "solution" would become a permanent, intergenerational burden for the whole damn bunch.
