This is great, thank you for sharing. I think I’m sold lol… I have to keep a running log ISH thing, specialists want to see my mothers blood sugar level, blood pressure, her protein intake, her sleep, meds lists (reason for, how often, etc), her movement, & I’m thinking different Dr/specialists contacts is a good idea. She’s bedridden n God knows something small would cut down on the things I have to haul around. I was/am using a passport tn but not ideal, I can’t move things around or when I have to rip one out the whole booklet is shot. Thank you again for sharing.

Hi Sonya! Glad to hear this will work for you. Happy Planner had a care giver extension pack at one time that sound like it would perfect for all your mom’s care tracking and doctor’s appointments. It is a classic size though so not so small. If you think you might one to try one let me know. I have an extra one and would love to send it to you.
