I got it wrong because I thought that the Fever --> due to head-itis High Ca --> due to granuloma (1-alpha hydroxylase from epithelioid histiocytes) + blood in urine = Wegener. Do all granulomas have 1-alpha hydoxylase?

I was able to figure out it was a paraneoplastic presentation of Renal cell Carcinoma, but still confused about how the pt has a fever? any comment on why the pt has a fever?
