Sir in one of my case, patient was asymptomatic after the first sitting lower molar .. only access opening and depulp was placed. On second sitting I completed BMP on all 4 canal. Canals were a bit narrow. I called patient after two days fr obturation. But patient complained of pain after the second sitting ... what might have happened sir?
Remember to always use ultrasonic tips for locating orifice and never use a bur to do it. If u do not have endo ultrasonic tips then use an interdental narrow scaler tip without water for the same. Always follow the dentinal map as a guideline.
@@kaushikghosh2376 first of all it's extremely rare for the entire floor to be calcified. Generally a large pulp stone will be firmed in the pulp chamber. In any case after initial access to the estimated depth use ultrasonic on the periphery at the pulpo axial line angle and the canals will be exposed. Never dig further in the center of an access cavity because it will cause perforation. Canals are always present on the periphery.
Always get the tug back from your GP, then cut the extended part and seal it using bioceramic sealer, if you don't get tug back then the gp may extend beyond the apex and it's a big red flag

There is still some bacteria in the canals, you need to irrigate and do activation with hypo and then finish with edta liquid to dissolve the smear layer(this is important), wait for 2-3 days then obturate
Calcium hydroxide should only be used as an intracanal medicament, if it's crown level or cej level perforation you can seal with GIC or composite, in case of root perforation mta or biodentin works best, if root is exposed then you can fill with GIC
