Hi arderneroad- thanks for your question. Labral tears do not actually need surgery in many cases. Unfortunately, the improvement after surgery can be pretty low. Often people still have a lot of pain, less strength, and are out of commission for months. We try to help people avoid surgery, and also we see a lot of surgical failures in our office who still have terrible pain, but respond great to Prolotherapy. In fact, we are about to release a hip labrum study in the coming weeks. Take care.

The information is very good. However, the psoas originates from the attachments to the lumbar spine.

Dr. Great video. I have all thode symptons . I should add that my knees are numb. Specially when i am sitting down for a few hours. I live in south america and i never heard the specific type of doctor you mentioned. Any alternatives or treatments ? So i can relatr to my physical theraphist??

Thank you! You have described my symptoms to a "Tee" in this video These conditions in my right hip after someone opened wide a car door while I was biking. I hike, bike and run for ever and have not hiked or run in 8 months and have become a sad person. (I can bike...) At 65, a couple of board certified ortho docs have recommended hip REPLACEMENT. I think they are both certifiably nuts. I have to curb my enthusiasm because you're speaking the words I've been waiting to hear. Can you recommend a facility/practitioner in Los Angeles, CA?

I jab be been dealing with pain over 20 years. Had to stop ridding bycicle. Pain is deep in groin it hurts to lift knee to chest. I had doctors do a labrum Debriedment. Cam impengment but they did not address my Acetabular anterversion. I hope I can get some relief from prolotherapy or PRP

I was in an accident one year ago. I fell on the side and suffered 3 fractures in my pelvic and one fracture in my sacroiliac joint (or sacrum). Anyhow, I believe the fractures have healed by now (although it took much longer than anticipated) although my S.I. joint is still troublesome (when laying down, sitting, prolonged standing). I am still limping and Iliopsoas Syndrome was now diagnosed. I have been doing physical therapy as well as aqua therapy in the last year and it really hasn't helped much. I am also doing approved exercises just about on a daily basis on my own. I have always exercised and enjoy being active. Although right now, that is difficult. Today I got some injections and it helped somewhat. I still feel some tightness in the back. I do know that these injections are only temporary. Those injections were painful. I have a second set scheduled in the next two weeks. My concern is: at what point is the only solution surgery? Can one hope that the physical therapy that I was prescribed now will do more than the 30 sessions of physical therapy and aqua therapy that I already had this year? I am very worried that this will be a permanent disability. I was very active and fit before and now can't even walk 100 yard without pain ... It's like walking "against heavy rubber" when my leg moves to the back. I am easily exhausted ... Anyhow, any advice would be appreciated. Sibylle

Not sure what type of injections you are receiving, but you probably want to speak with the person who performed the injections as to the long term goal. 3 years ago while I was turning and changing direction my right hip had a sharp pain and I couldn't stand on my right leg for 30 seconds. This still happens today and my psoas is tender to the touch. 1st doctor looked at films and said Labral tear and injected with no releif. Now my third specialist injected psoas with 100% relief for 3 months? Yes my hip pops too but not always painful. Any suggestions? This doc doesn't see a Labral tear and tells me to stretch which isn't helping. Thanks

Hello. Great video. Very informative. I had my labral repaired with a Arthroscopic hip surgery about a year ago. My hip is doing great, but I am struggling with Psoas pain now. The pain is deep in my belly and into the groin. I am an avid runner and martial arts instructor, so I use my legs alot. Any advice?

Often with labral tears, there is additional joint instability/ligament laxity which then causes the muscles to overwork and become painful. Once the instability is addressed, the muscles can relax and not have to be burdened with the job of the ligaments, which is to stabilize the joint. Hi. I have groin pain in the iliopsoas area near the lesser trochanter. Pain is caused after walking 400 meters and goes away after 3 weeks of rest and i feel no pain at all. If i go with prolotherapy do i have to stress the muscle first so that the body starts the healing process (for better results)? My condition is chronic.

Finally diagnosed myself. I do weightlifting, am athletic but when i was a kid i would sit cross legged on the floor gaming all day and developed kyphosis in my spine and anterior pelvic tilt. I have protruding abs despite my decent physique. At work i get extremely painful aching in my middle back, but iv never had lower back pain ever. Along with that I have tight hip flexors and tight (or short) hamstrings (especially with squats and deadlifts) and every 6 months or so i randomly wake up and i literally cant lift my leg due to the sharp stabbing pain in my hip flexor right in my groin. The same popping sounds. Cant lift leg out of car. Practically have to crawl along the floor. Not my actual leg but deep in the groin where the leg connects to the hip. It goes after like 3 days and im back to normal. Iv tried everything over the years. Foam rolling my back relieves the thoracic pain but isnt permenant. Stretching my hip flexors helps a bit but i still maintain APT. Stretching my chest doesnt help. All the back strengthening exercises do nothing to help my minor kyphosis or and doesnt pull my shoulders back. I sleep on my stomach religiously which doesnt help. Iv recently put more effort into planking to try to bring my Pelvis into PPT. But seems im structurally kyphotic lordortic

Please help me understand this. 7 months ago i got an injury similar to what you described while playing football. I had a muscle tear from my groin region down to my thigh which got better after few months. However, i still have this big bump around my groin and its very stiff like a bone. I'm scared it could be a bone growing there i cant even bend to tie my shoes, but i can walk and run. Do you think its lliopsoas bursitis? Do you think i might need a surgery? Its very stiff like a bone. Thank you

Dr. Hauser, I had a Labral tear in my right hip (from a fall at 43 in 2009) was diagnosed alongside the tear with hip dysplasia. Had a PeriAcetabular Osteotomy. 1-year post-surgery as a result of the reconstruction my pelvic broke in half, nonunion fracture of the Ischium and Pubis bones. 2 years to heal from that. All has left me with years of debilitating pain in my hips, I was diagnosed with hip bursitis, did therapy, massage, even Chiropractic. Still the chronic pain, almost burning feeling in my muscles surrounding the area keeps me from being active. I used to be a distance cyclist and a kayak guide. These days I'm lucky if I can do a grocery run by myself. I wonder if I have a build-up of lactic acid in my joints and muscles/tendons/bursa. Is there anything you might recommend? I have started a ketogenic eating plan in hopes of reducing inflammation and purging sugar and excess glucose from my body (3 weeks in, not much of a difference yet) any recommendations would be appreciated.

Thank you so much for this video. After a year of walking with a cane and multiple diagnosis, I'm desperate for advice from a Prolo-Therapist, (since there are none in my area). If congenital dysplasia is the root cause of the labrum tear and chronic tendonitis/bursitis, will prolo-therapy still be "curative"? Or could it prolong the need for THR? If so, by how long? I'm a 43 year old athlete and am scheduled for THR in July, but want to be 100% confident in the decision. Thank you so much for any support.

Any forces strong enough to tear the labrum or other structures, is going to overstretch the ligaments that hold the joint together. I have thigh pain. I had surgery fir a labrum tear but the pain has not gone away. I was in PT for 2 months the pain returned...drs aren't sure. Pain in my lumbar that goes down and crosses over to my thigh. Stops right before my knee. The pain is debilitating. Help!

I have a hip in spasm after anterior total hip replacement. this causes the leg to feel shorter, but that's only the start of thr problem .... I cannot put weight on the hip and I'm now 5 weeks post op. Will this resolve by itself or would you recommend therapy? I actually had bilateral hip replacements and the other hip is marvellous.

Im a ballet dancer and think ive been suffering from this for 3 years now. Got so bad last year I had to stop exersize altogether. Can it eventually cause deep groin and inner thigh pain? I also have urination hesitancy

Hi Chameleon, So very sorry to hear how you had to stop dancing and exercising. Yes, pain can be deep in joints or radiate down through the legs. Hi Victoria, Yes, there are various tissues that can be damaged and overstretched during any surgical procedure, including a replacement, even when they are said to be "minimally invasive." Any tissue that has to be cut through or displaced during the surgery in order to reach the bones. This would be something to discuss more with the surgeon when reviewing all of the risks and why they think it is the surgical intervention that they would perform on you. I have been having trouble with my right side pelvic area (going across from my belly button and about an inch down right beside the pelvic bone) have seen 2 doctors I first thought I had a kidney stone and or an appendicitis,..the pain started back in late August,.. I was doing a lot of hevy lifting moving cinder blocks and would sit in a chair wrong and I think that help pinch something,..got an utlra sound was not kidney stones...I would stand up and would feel a sharp pain and I would sit down at times and feel a sharp pain, lay on my stomack and felt the pain creeping in,. I started to do some streaching and it has seemed to help somewhat I spend a lot of time sitting; I deal poker so I'm sitting and I sit at a desk a lot (I know I have bad posture) am I on the right tack with this,..I think the last doctor I talked to thinks Im crazy or something...the pain comes & goes spasms likes.

i have this same problem. I was scared it was appendicitis as well. but i started to do stretching and it is getting better. do you have any suggestions of what i can do to better help it get better?

megamatt53 I was doing a lot of lifting with cinder blocks..I stopped doing that and it got better on its own the doctors were of no help, so I wasted about 700 dollars in fees trying to figure it out...appendicitis usually are soft to the touch vs kidney stones one can pound on your side and the pain does not get worse..if you pee a milky with, the appendix busted.

Hi dandycandy1983, Yes, women can develop various chronic pain/joint instability conditions, including at the ilipsoas, as the ligaments loosen during pregnancy.
