First off, thanks for the great videos, always appreciated. I understand it's an older recording but here goes. But about the PE study - 266 patients. 3:43. Abstract number 24. Risk for Recurrent Venous Thromboembolism in Patients With Subsegmental Pulmonary Embolism Managed Without Anticoagulation: A Multicenter Prospective Cohort Study Observational study without a control group and here's the problem. Quote from the study - "At enrollment and at each follow-up telephone call, participants were elicited for new signs and symptoms of deep venous thrombosis and pulmonary embolism. Four interviews were conducted, at day 10 to 14, day 17 to 21, day 24 to 28, and day 85 to 100. Patients with suspected venous thromboembolic events were evaluated by standard testing. " To me this is very telling and this study feels flawed, 1)There is a high chance of overdiagnosis when people are constantly prompted about PE symptoms that are often non-specific. Just think about it - 'Hey, Bob, I am including you in this study now that you have a clot in your lung. Remember to be observant of symptoms'. 2) There's also the thing about CT-PA being a bit too sensitive and picking up all kinds of clots. If we had an electron microscope CT-PA everyone would have a PE. What if all 266 participants were scanned, what % of the would have had a 'recurrent PE?' 3) NOONE died. PE is supposed to be a very deadly diagnosis, or at least it used to be before it got overdiagnosed?
