With every breath how will conc. of N2O increase if starting ratios are 50:50 :: O2:N20. Acc. to my calculation; they are only decreasing 50 then 37.5 then 28.125 and so on

So u tell me practically in indication of anesthesia with inhalational agent in order to achieve a rapid effect I. Need to turn the N2o in high presentage. So I'm telling that the only benefits of this information practically that I need to indicate anesthesia with specificaly high N2o fraction

Ma'am does MAC value have anything to do with recovery of the pt? And if it says MAC value increases due to a particular reason then shouldn't we decrease the dose as potency Increase???

The MAC is different for all agents , and this is calculated with agent + O2 , no added N2O. So as I have explained you need to achieve the MAC awake and lower to ensure recovery if you are using agent + O2 Don't confuse with MAC that we see on monitor bcoz that is MAC with N2O as agent as well.

Take an example. MAC is decreased in hypothermia / old age / pregnancy- so that means you should reduce the dose as this patient is more sensitive to the agent .

Mam you told that during 1 breath about 1000 ml of N2O will diffuse in the blood but at the same time 20 ml( total 40 ml) of ISOFLURANE will also diffuse in the blood. You have explained only the diffusion of N2O not ISOFLURANE and OXYGEN during 1 breath... So now what is the mechanism?????

The diffusion of gases is occurring along concentration gradient . More N2O will diffuse initially until equilibrium is achieved. N2O diffuses at a faster rate accross Alvelolar capillary membrane. Isoflurane or any other agent - will also diffuse according to the concentration gradient. So initially faster and later slow once the gradient reduces . Oxygen also follows same rule. But remember oxygen won't diffuse that fast as the gradient isn't as big between pulm capillary O2 and Alvelolar O2 . The numbers we have taken are purely for understanding concept of concentration effect and second gas effect.

You should never be priming the circuit, it is a very bad practice causing OR pollution. It is a big "no no" in North America. That is why you start an IV induction and you catch it up with volatile agents. More so, Iso is banned in most states in US and some provinces in Canada for its big carbon footprint.
