Nadir that is brilliant talk, thanks for illuminating us ... and big appreciation for your hard work and time you must have put in to research and get this information out to us. It explains IR much better than anything else… If you put this video and @ted naiman video of adipose tissue on IR together, that just mind-blowing. Thanks again … Amazing work.

I love this. I am not sure if I will ever completely understand it.... but I do like the concept of the occasional carb-up. I don't think anyone should EVER eat sugar but I think a little starch just to activate those carby pathways once in awhile (as in no more than once per month and keeping carbs under 50g or even 100g) makes a lot of sense. Our ancestors ate whatever was available. I have to think that our bodies know how to metabolize carbs for a reason, though I do believe that ketosis should be our default state.

Carbs is not bad if it's in real food, processed food are bad which lack fiber

Great presentation...thankyou. It would be great if you could do a talk about ApoE 4 carriers and how saturated fats in particular, effect them adversley and why.. this makes it very difficult to maintain a healthy keto diet...or are we all just doomed to live on avocados, sardines and olive oil ??? I have listened to several talks on this subject but would greatly value your opinion on this

Timestamp 29:58. Having reached insulin sensitivity by reversing my T2D, this is the $64K question. By regulating my diet, exercise and rest, and using my body as a "lab rat" I can monitor my A1C and glucose levels to formulate ad hoc solutions.

I love Nadir's talks, but I've never really understood his slides. He seems to put so much work into them, and they're very attractive, so maybe it's just me. Anyway, as an APOE3/4, my first question is whether it's best for brain health to be in a state of deep ketosis, or can I skirt along the edge? The resistant starches I'm eating make it hard put up the big numbers, but maybe my brain would be benefit from more of the only source of fuel I want to give it - fat. Secondly, should APOE4s be more or less open to the carb cycling mentioned at the end?

@@nadirmirali Hah, it wasn't even criticism. I said they were attractive. I guess I just slept through the class in law school that would have given me a clue as to what they mean. I'm clearly not as smart as, say, an engineer. ;-)

According to Dr bikman. Glucagon and insulin work completely opposite. The absence of insulin will increase glucagon that will release ketones for fat burning.

Why is the glucagon level higher in the normal subject after phase I insulin response (200 to 400 pg/mL)? On the right, the glucagon level in the absence of phase I insulin response only rose to 150 pg/mL. (31:42)

I think Dr Ali needs to check with Ben Bickman on Unger's work. Glucose is not made from muscle tissue.

Did this channel get hacked? I swear I saw some weird, hour long video that was all buzzword clickbait junk. I commented on it, then came back to comment again the next day and it was gone and no videos at all showed. Now the older videos are back up.

Dr Nadir, as you know wrong cleavage of APP is not happening only in insuline resistance but in the presence and absence of other 40 reasons as Dr Bredesen told us. So it is not that by addressing IR you can have free saturated fat meals if you are a APOe4/4 homozygote. You have medical responsibility to clarify that, otherwise you are sending the wrong message. Which is a pity because you are already advocating the other 39 reasons yourself too. If Ivor Cummins is ApoE4 carrier and he eats saturated fats, it is his problem if something will happen to him. But we cannot send this message to the 15% of the population who is APOE4/4 or APOE3/4 heterozygote to eat saturated fats and meat only as Ivor does. This is a high risk not proven approach

@@nadirmirali I respect you professionally but find your unprofessional treatment of President Trump revolting. I saw that same clip with President Trump in another one of your videos and did comment on it. Why are you using it again when you must know you are antagonizing roughly half of Americans? I suggest you edit and remove or alter the clip. Saying you aren't political does not absolve the clear political hostility showed by that "joke."
