In early January, Nature Communications published a Dutch study called “Muscle abnormalities worsen after post-exertional malaise in long COVID.” The study caused a buzz among patients, clinicians, and other researchers and generated extensive news coverage. Called. (I wrote a blog post about it here.) The investigators identified significant biological differences after an exercise challenge between long Covid patients with post-exertional malaise and matched healthy controls who had recovered from COVID-19. I spoke about the paper’s findings with Rob Wust, one of the main authors and an expert in muscle metabolism at Vrije [Free] University Amsterdam

I'm almost totally bed ridden and have been for over a decade. If I push muyself cognitively...phone call/ admin etc my legs are significantly heavier afterwards. So I always conflated physical and cognitive. To me PEM is systemic and all symptoms are affected no matter what the initial cause. Thank you for the article.

Thank you. So glad to hear research is continuing and of collaboration over cognitive exertion and deficits alongside this. If the PEM threshold could be increased, that would be great! Any improvement welcome after over 25 years. I'm interested to learn you found existing muscle necrosis. This confirms to me the vital importance of avoiding PEM in the early stages of illness if at all possible before too much damage is done.

This was amazing. Great questions. Nice to hear some more background information and such a long interview with the amazing Rob Wüst. Loved the debunking of deconditioning and what a change from the shoddy CBT/GET research we've seen from the Netherlands thus far. The best part is that it's only the beginning of more exciting biomedical research coming our way. Also about ME. Thanks to both of you for the interview!!!

This is truly a breakthrough. We need to see this replicated right away. Clear proof of tissue starvation is a huge step forward at understanding the mechanisms causing fatigue, PEM and brain fog. PLEASE keep pushing this so we know for sure if this is what is happening. I am three years into my sudden transition from an ultra-endurance athlete to being bedridden, in spite of trying everything to get better. This answers so many questions and is an easy research to replicate. It could lead to testing to diagnose long covid. And also potential treatments in the future.

I’ve had ME for 27 years. I’ve tried to get a muscle biopsy several times with no success. Many of us are too ill for CPET. Also some ppl don’t get PEM until 2 or 3 days later so 24 hours won’t always pick it up. Even after being so ill for so long I will never understand why trying to do too much causes an inability to sleep.

Thankyou so much for these videos! I have LC with fatigue, PoTS and PEM and find your interviews very helpful. As a layperson I can understand them rather than trying to directly access studies that aren’t always easy to understand. Very interesting study, keep up the good work both of you 🙌
