Well, they don't need to hear that they're a "fat bitch." or an "overweight cow." and I'm pretty sure they've heard that plenty of times. Tell them what they ACTUALLY NEED TO HEAR, without being unnecessarily blunt or humorous about it, because that's just going to encourage them even more. (I'm speaking from a third person POV here.) You're making fun of them, AND being blasphemous towards them, so of course it'll hurt their feelings, like what...What really matters, is what happens to yourself after you're given that true reality check. You'll either fold into the habit even more, or you'll improve for the better. With people like this, you genuinely need to sacrifice most of your time and efforts to help them get back onto their feet, because they can't help themselves. I know from experience, not obesity, depression. You can't teach realistic lessons to people by babying them and constantly reassuring them, sure, but at the same time, you can't do what I stated earlier either, because it's just going to encourage them even more. I personally don't find his commentary to be funny either, I just find their lives to be interesting, that's why I watch sometimes in disbelief, plus it's on the ITC page so why not. Making "fun" of someone by being harshly blunt and calling them terrible names, isn't funny. "That's enough! Fat b-tch!" Then he'll turn around and call it "comedy" as an excuse. Making fun of some deathly obese person for content, all by calling them harsh names. Wow, nice content. Then all of his supporters are going to rush to comments like mine, with their ignorantly bland excuse of "BrO It'S JuSt A jOkE."
