I believe it would be very interesting for you to register because then you can publish your dissertation later on. But if your University do not demand a registration, then I guess is ok. You should check with your professor.
Is preferred to register before you start the research, but is not obligated. Does not matter the type of the review. Although, you cannot finish the review and then try to register, they will no accept it.

I guess in this case maybe review your question, the PICOS. But if the question is appropriated then I believe that you can decrease the number of exclusion criteria and follow with the review. The amount of papers found is not important, your focus is to extract all of the studies regarding your question, that could be thousand or 10.
