They are trans, it wasn't exactly specified in the anime but in the manga there is a whole scene where he talks about being recognised as a man and that he is a man

Her identity as Irene was in danger because she was too exposed and also the crown was chasing her, so the Moriartys to save her, faked her death and gave her a new (male) identity so that she could continue living in the shadows. Since her previous identity as Irene was already publicly declared dead, now living a new life as James Bonde, "she" asks to be treated according to that new identity (being he/him). Therefore, he is now a man 🤗

i know that that chapter was confusing for someone, but he just wanted to be with Sherlock as "Irene Adler" one last time. In his character profile he only uses he/they and feels embarrassed when Sherlock or John refer to him as a woman! :)
