The coronavirus pandemic has overwhelmed hospitals, physicians and the medical community. That's pushed telemedicine into the hands of providers and patients as the first response for primary care. Telemedicine isn't new to the medical community, however it hasn't been embraced due to insurance coverage, mindset and stigma. Here's how it works and what it means for the future of health care.

Telehealth: an innovation that should increase access and reduce cost. Instead this show highlights it is simply seen as an opportunity to make more revenue with greater margins. Few studies on quality of outcomes, inequality due to device access (already seen this with my own patients) and cases that are most appropriate for the service.

Lets hope not because that would cost a lot more of this $$$ and could possibly be dangerous because of conditions or viruses such as bladder infection which need an in clinic visit. I personally do not trust it

What I don't get about the discussion around telemedicine is that some people frame it negatively as a replacement for physicians but it's not like we're ever going to hit 100% penetration because customers do not have access to diagnostic tools needed for even basic checkups. In actuality, it may end up being a supplemental source of income for physicians who can now accommodate patients both in office and online.

Could you clarify who benefits more in terms of income, the physicians or the communication services? I am in the health sector and I believe it should be the first line of action, just that people do not see it that way. Calling a physician even on phone to discuss your health issues so that he/she can determine whether you need to visit a hospital is telemedicine.

Telehealth should be adopted more. A big % of patient cconsultations is to go through labs and prescribe meds, you don't need to be present for those.

One specialty not mentioned in the comments is psychiatry. Telehealth has allowed me to see all my patients remotely without exposing them or me to risk of infection. A few patients choose telephone contact for various reasons but the vast majority can use a video link. It gets fairly close to the in-office experience. After the crisis I may continue to offer it as an option to established patients who drive an hour in each direction for one hour of psychotherapy. It may also become an instant alternative for the patient who has to be home with a sick child or whose car will not start, etc. The coronavirus crisis has introduced me (and a large number of my colleagues) to a new modality that can offer more flexibility.

Its not just healthcare america is slow to adapt to new technology. We BARELY got chip technology into cards meanwhile in some places like China they don't even use money. Most transactions are by phone, and are mostly cashless. Like...you can even give money to a homeless person tru a QR code. Here one store goes cashless and people rage and sue the hell out of it.

We are forced to buy health insurance, so if this new medical practice is the norm, then reduce our health insurance cost. Charge patients per diagnostic

I see it as a wonderful advancement for the simple less complicated doctor visits. Taking a day off work, for those who are employed, to go to the doctor for a check-up is a lousy experience. Wouldnt the shift of these types of visits then free up more space for the complex and urgent cases where patients' lives are in immediate danger?

That’s a good way to let something minor turn major or deadly. You aren’t a doctor. So leave it to an actual doctor. If I build concrete patios. I can’t do a televisit on your land and give you an exact. It’s impossible without the info needed. Same with doctor visits on checkups and bloodwork and temperature.

I’m a physician in a rural area, I saw that most of tele health services overload thier physicians with more patient numbers than regular practices on the expense of the quality, if there is a choice for a regular visit I would take it any day , I don’t see real winners other than CEOs and administrators

I think any check up is better than no check up. Tele health is just a phone call but with video. If more detail is required then you need to go to the Dr office.

My sister is a medical intern and she says all the senior doctors stay at home and refuse to come see coronavirus patients .. only the juniors and nurses are treating this !

How many senior doctors do you know of that aren’t senior citizens? They’re at most risk to die of the virus. Why do they deserve to have their lives put at risk because of their field of work?

@@thankyounext365 while completely true, a lot of the doctors in my country and my sisters hospital are in their mid 40s yet the refuse to come see patients and bark orders on juniors who dont even have enough PPE for themselves. The juniors also have families, we live with my grandmother and my sister had to stay at a friend. She and others have been working double shifts, and the senior doctors refuse to give them a day off while they sit relaxed at home !!! They are exhausted, it's only a matter of time before they make a mistake.

Qadreya T I’d do the exact same...why should anybody put their life on the line when the hospitals won’t provide the proper Ppe. Think about it, you’d go and put your life on the line and if something happens the hospital just fires you? I’ll pass. Send the administration staff out there, they make the real $$ so let them earn their wage like everyone else.

Healthcare shouldn't be an area where profitability should be given such a high priority that doctors are churning out a lot of consultations per day at the expense of quality. Hope a balance is reached and that patients aren't sacrificed for profits!!

@Aurobindo Ghosh Providers in the US are given a national provider ID number (NPI). When a doctor enrolls for a platform like American Well, they are given a background check and the telemedicine company verifies their identity and ID number, in addition to their education. Any prescriptions sent to pharmacies also include the ID number so they can verify they RX

You can't do a physical examination with this telemedicine, Anamnesis, Physical Examination & Lab Analysis are the essentials in Medicine

I was recently cured of Hsv1and2, Thanks to Dr Agbonifo for the good work. After living with this virus for 4years, I saw a comment of Dr Agbonifo on a headline, I was skeptical but I had to talk to him about my situation because I've done everything possible for me to get cured, and the doctors doesn't make us believe there is a cure, When I told him of my situation, he told me not to worry that I will be cured. I did exactly what he said, I took the medicine for some weeks just the way he ordered and my Hsv1and2 is gone. Now I'm free with no side effects or whatsoever. Again why universal health care > than private system. In Canada 🇨🇦 we have Telehealth that is 100% covered by our universal healthcare system. No insurance scared patients might use it too much here

I have over 10 different health conditions and I've been in love with this telehealth system, I'm so in love with it. I can talk to my doctor anywhere and I don't have to drive so far away to talk to my multiple doctors; I support this system of health!

12:40 is the real reason telemedicine is an issue for doctors. less $$$ is the problem. all this other crap about not being able to take care of patients is b.s. they only spend 10-15 minutes with their patients while in the office anyway.

unless the doctor needs to physically look at something, exactly what more will you gain by sitting in the office for 15 minutes (the time you're often given) with the doctor? if something is wrong, you will need to get bloodwork and other testing done. you'd probably need to go to a lab for that anyway.

How do you know they arent wheezing? How do you know they dont have appendicitis. How do you know they arent pregnant? How do you know they dont have a meningococcemia rash? St elevations???

If you go to an Urgent care doctor and tell them you are having lower abdomen pain, they will recommend you go to the ER to rule out Appendicitis. True you would need a cat scan to determine but that urgent care visit could have been expedited through Teladoc.

Propagating telemedicine is just like talking good about e-leaning modules. The end user experience is down and would only benefit if there is a financial incentive for the customer. Remember boeing crashes due to bad I-pad based learning that resulted in countless deaths. This will also impact as even face to face doctor visits are not reliable unless the doctor has a reputation & experience and not a new one in the field.

Interesting side bar, In Ontario physicians mal practice insurance is paid for by the OMA. Should one file a claim your dealing with a government bureaucracy. A service that invented the phrase "slower then mollases" . This practice is blatent conflict of interest. Have known individuals on both sides of the spectrum. There are physicians out there that have over 50 mal practice disputes that are still in the profession.

the benefits of telemedicine became so much evident after the Covid 19 pandemic when people were forced to remain at home as part of social distancing. Patients could consult doctors from the comfort of their homes...Only thing is to check the availability of the doctors.

I hope not. The only time I had to use one was in late January, was extremely sick and they refused to give me anything but cough medicine. They were sure quick to take my copay though.

in the end you still need a doctor or a specialist to feel that bump on the neck, or that hernia, or check that mole that needs to be removed to see if it is cancerous. You can't do that all from telecommunication. We hate waiting in the doctor's office, but for some crap like that you have to go to the doctor.

As much as I hate to have to use a virtual doctor I can now see the benefits. I live in a poor rural county in California. Yes, poor does exist in California. But thats because rent is so high and jobs pay poorly. We don't have enough doctors. Why would they come here to live/practice. I only live here because I'm poor. Id really rather live anywhere else. You can't get in to see a docter for weeks (unless you're dying). Even then they blow you off because everyone is on state/federal health care that doesn't pay much. Tele doctors can despense medicine from the comfort of their own home (with no pants on). I get to have prying eyes see the dump I get to live in. A problem may arise when patients don't have a phone, car, computer or the smarts to use said computer. So making tele doctors a good idea is your opinion. If doctors would doctor instead of running a patient mill would be nice. Once upon a time. Now everything is upside down.

Telemedicine is much more effective if the patient is with a primary health worker. Pyramed telemedicne believes in telemedicne through a primary health care worker to a secondary HCW.

My wife has been seeing her patients using telemedicine from our basement for the past month. Hope it continues, or else, she's living down there too (I'm high risk). Not in the US btw.

It will become the new standard if CMS continues to pay at least 100% of current visit charges! The benefit to patients is very low as the comprehension, most of the time from both sides will be poor.

Here's why people need to avoid hospitals 1. Place of antibiotic resistance 2. Time Energy wastage 3. No value added in actual visit to doctor 4. You can see doctors ratings (unless the website is not manipulating it)
