I beat A18 on Watcher yesterday and throughout all my playthroughs, I used Fasting maybe once, and it was most likely from a pandora's box. Idea for a run. You have to complete every single floor on an act to move on. It might actually make the game a lil easier so as an added ''bonus'', you have to add a card in your deck every time you get card rewards. You can still remove cards.

I usually don't take fasting unless I have excess energy. to many times I have lost runs because I couldn't play the cards I needed after using fasting.

Personally, fasting scares me. Energy is such an important resource. I suppose energy relics and stance dance can make up the difference, but it's a significant cost.
