In Switzerland this gesture basically means "you've gone crazy/you're blind". It's been around for ages (at least since the 70s). You do it to demonstrate your annoyance at a driver that's doing dangerous things or any person doing crazy, unreasonable stuff in public. It's basically one step down from the middle finger. Thinking about it: Any Italians here? This gesture might well originate with our southern neighbours. Most of the "talking with the hands" stuff we use comes from there.
