in the performance of following 6 ADLs
BATHING
DRESSING
TOILETING
TRANSFERRING
CONTINENCE
FEEDING...
So Clients score between 0 that is (dependent)... and 6 means.... (independent)
A score of 6 indicates complete independence, ...
4 indicates moderate impairment, ..
and 2 or less indicates severe functional impairment
