A small correction to my live stream: not every milligram of medication given in an IV will reach the brain, as there are some losses throughout the body. Nonetheless, IV administration is considered the gold standard with regards to bioavailability (100%) and precision of administration.
IV Ketamine is used for infusions. It is the most direct way of delivering Ketamine to the body to treat depression, chronic pain, anxiety, addiction, or other conditions. I did an infusion in 2018. It cured my lifelong anxiety and depression. I am autistic. I didn't know it then though. A single treatment was all it took for me. Cured me. No other drug or therapy did that. Ketamine heals.

I'm fascinated at your results. I am also autistic, and just started infusion therapy for my treatment resistant depression, PTSD, and chronic pain. I couldn't believe how much better I felt the first week! I don't feel like the 30mg lozenges do anything, but they tell me I'm not supposed too feel anything, and it's just to keep a baseline level in my system. I was really surprised about the IV experience. I thought I might feel more of an altered state, or hallucinations, or something, and expected to have thoughts or feelings to go over with my therapist. However, the other medications they gave suppress that for some reason, which makes me feel a little cheated on the therapeutic potential. One thing I noticed is that everyone else said they slept through their infusions. I was reading, watching TV, and doing puzzles. Made me wonder if my dose was low, or if it was just my neurodivergence.

@@brendaeberwein8338 Oh me too! I’m up by the border in Bellingham Wa. On disability ( $1347/mo) and they do have 2 new Ketamine treatment centers here in town. But, soon as I mentioned that word to my obviously completely unaware Dr, her tone was that of a shrill freaked sort, like I was looking to get horse trancs, for real. Did not help me. I looked into it, and the cost is too high for my pockets and they don’t accept Medicare up front, yet. I’m hoping though in future they will more readily. It’s established, we are all getting great vibes here anyway. 😊❣️, that goes a long way for me living with chronic pain and anxiety, some depression and bad insomnia. I watch him and he just makes me feel better ❤️‍🩹🌻 Pray you all feel better too!!

I really appreciate your videos. I have been getting ketamine IV and IM's over the past year and a half, for C-PTSD, depression and anxiety. It has been like a miracle for me. I feel like myself again. I am so grateful for this treatment and hope it will be FDA approved for depression soon. People deserve to know about this drug.

Hi Linda, I had the IV yesterday (just one day is given here in the Netherlands) and yesterday my back pain stopped completely, but then today I feel it again. In your case how did the pain stop? Immediately? 😪

Thank you very much for the information. I've been to a Ketamine infusion clinic for Occipital and Trigeminal Neuralgia. My anesthesiologist is the most compassionate and caring Dr I've ever seen. I've had initial rounds of IV Ketamine which took all the pain away for 14 days. Unfortunately, my insurance companies refuse to pay for it, so I pay out of pocket for emergencies. He does also prescribe me oral Ketamine every month which helps me to not have to take addicting and stronger meds nearly as often. Finding compassionate and well knowledgeable Drs who use Ketamine is a key. Thank you for all you do.

Thank you for sharing your experience. I was recently diagnosed with Trigeminal Neuralgia, and just had my first infusion yesterday through my doctor's office. It was a low dose IV over 4 hours. I had pain through the whole session, which was frustrating, and am not feeling any difference yet. I realize I may need more infusions for it to benefit, but I'm curious about your experience with dosage and length of time for IV? I'm struggling not to get really depressed as well, as this has pretty much limited my life to focusing day and night on some level of pain management, so it feels like higher doses to address that would be helpful as well.

I did get ketamine infusions. The first time I felt like I would imagine it would feel like if I was dying. It was horrible. Horrid anxiety and nausea. My blood pressure went up to 199/100. The Dr said not a problem that it will come down. I needed help to get to my car. ( my husband drove me) but horrible experience!! However, the next couple days it was a miracle! I literally felt lighter and even my neighbors noticed. I have severe depression over the loss of my daughter. I went 3 more times at lower dose, then back up to first dose and I never got back to those couple of days I did well. The anxiety and nausea was too much and I couldn't afford to keep going.

I was given ketamine years ago, and felt it was amazing I felt better for a long time. But here, in my state you will not get pain treatment. Question; Is Ketamine addictive? I have Chronic pain, Lupus, absolutely diagnosed neurologic problems throughout my body and so I am looking into going to Colorado to try psychedelics hoping I can get some help. This is tragic that pain is not treated as it wears one out. Thank you Doctor for your kind heart and for educating the masses... We appreciate you!

I receive IV ketamine for managing chronic pain from peripheral neuropathy, an in-patient procedure over five days. I'm admitted on Monday afternoon and discharged late Saturday morning. My vital signs are checked every four hours, every two hours for the first eight. For the IV a peripherally inserted central catheter (PICC line) is inserted. That's routinely done for patients as ketamine is viscous and somewhat caustic. Before this I got IV lidocaine, seventy infusions over seven years. Ketamine is much more effective in reducing (not eliminating) the neuropathic pain. I've had ten IV ketamine infusions over three years. The next treatment is in January, four months after my last treatment. Though this use is off-label, the treatment is paid for by Medicare, and I have secondary health insurance coverage. I live in the SF Bay area, and receive treatment at Stanford Hospital.

My only prescription medicine is gabapentin. I take a dozen vitamins and supplements including bromelain, an enzyme from pineapple, for inflammation and magnesium for muscle tension. A bonus was kicking my cigarette habit with the first IV ketamine. Though I hadn't gone in intending to give up that addiction, I'm so glad I did.

Hi Dr Kaveh, I enjoy the channel so much (PACU RN); periop,by far,has been my career- favourite clinical specialty. I can’t find that you’ve yet done a segment on the potential for anaesthesia, and/or post-operative, complications when one is on Ozempic, Wegovy, etc. It would be timely and important advice for sure! When I had gallbladder surgery, I actually needed so much and many different narcotics. I have to have such large doses of ketamine, fentanyl and versed because I literally detoxify those drugs so fast so it makes everything risky! I have amnesia so I don’t remember how the ketamine was administered. If my surgery was only like a 5 minute scope test I can probably get away with using propofol but anything that takes longer than 5 minutes, I will need something stronger than propofol. I didn’t want to risk waking up during surgery again while having my sternum cut open so I went without sleep and marijuana and I had the anesthesiologist give me ketamine.

I'm given Ketamine via IV during my bone marrow biopsies. It's quite an experience and each time results in an entirely different experience. Because I don't know what to expect, I have a bit of anxiety beforehand. My next biopsy is next week.

I was getting benefits for the first time with ketamine infusion therapy & I had hope for the first time in decades, but it couldn’t afford to even get one booster so because of the fda protocol… they don’t want us better! I haven’t been living for about 3 decades… I feel I just exist. I have suffered through trying almost every antidepressant and dealt with all the negative effects that many have & I even made sure I was on each medication long enough for me to get full benefits of the medications. I finally had hope for the first time in 2 decades after trying the ketamine infusion therapy only to find out that insurance companies don’t cover anything. $750.00 for each infusion and for me it was $2,250.00 for the first set. I couldn’t afford to get any boosters so my hopes were crushed knowing that what actually was showing promise was not an option

And once more, I have to tell a medical related channel a big thanks for not actually showing the entire syringe on their thumbnail! I have a phobia of syringes and needles and I’m being triggered at least twice per week in one of the lessons in school or on thumbnails I see on RU-vid. Really, I am grateful that some medical channels don’t go all in for the shock effect. Thanks!❤

Having been treated with ketamine via every RoA over the years, I would assert that the "best way" is the way that is most accessible to any given patient. I wouldn't challenge the studies that show IV is most efficient, most effective, safest but the difference is marginal in my experience. Some of my most transformative experiences have come via sublingual K. Not every patient has access to IV infusions due to geographic or financial barriers. I'd hate to see suffering patients avoid available, affordable local options because they heard on RU-vid that IV is the "best and safest." Every practitioner has their own rationale for why their RoA is best but I'm reminded of a slogan I saw hidden on the wall of the break room at a car dealer, "The right car for your customer is the one that's on the lot!"

best comment. accessibility is the key, and having to have someone licensed to give IV ketamine reduces that drastically. Everything he says about titrating dose and bioavailability is true. But does it justify the cost of paying another licensed professional (hopefully there's already a therapist involved)?

I’ve never heard of this before! I’ve been in constant pain since about 1980! Why doesn’t it get prescribed? Im tired of government ruling over patients getting a pain reliever that works. I was literally on a fentanyl patch(which was iffy for results!) Can I get it by the industrial drum? I need to dunk my Whole Body!

I am so sorry to hear about what you have been through. This is a tricky medication, because it's anti-pain effects may not be achieved with oral dosing. I'll have to do a video specifically on ketamine for chronic pain. In the meantime, do you have somebody helping you cope with your pain?

Both my family doc and my pain doc have recommended Ketamine for my depression and chronic pain, both caused by my MS. I cannot afford it though, so I am unable to see if it will work. The cost and unavailability just add to my depression.

There are so many instances proving that it helps people with such a variety of things. I really hope that it is covered by insurance soon. The out of pocket cost is insane!! I'd like to try it for several of my issues, but I can't afford it either. I also can't get any other kind of help. So I'm just stuck suffering 🙃 Something needs to change soon. I hope we can get access to it reasonably in the future 🙏

Ketamine infusions have saved me from my chronic pain. We have sublingual as well and its for inbetween infusions so they last a bit longer

I appreciate you going over how effectiveness is measured via bioavailability and that it really is patient, condition and symptom dependent. Thank you. I am both poor and have Bipolar II, so ketamine infusions are off the table for me. Given that, my psychiatrist (a local doctor I meet with monthly, not an online one*) has me on daily, low-dose ketamine troches compounded at a local pharmacy, which is only $65 for 3 months. It took me a long time to go from 25mg daily to 75mg (so 25mg-ish in my bloodstream now), because even at these low doses, the slight dissociation/need to take a nap feeling has never gone away like it does for others. She's told me we can go higher, but I'm really not a fan of that feeling. It definitely took way longer to get me from "I need to do a death cleaning and think about what I wanna write in my notes" to "No, wait, I wanna LIVE" than with infusions - like a year - but we have to go slow so I don't end up in a manic episode. I don't wanna re-blow those circuits we're repairing, lol, which is why I'm still on Lamictal. It's still important to keep that glutamate in check. However, I've been able to get off sertraline and been able to very slowly decrease my dose of Lamictal. My chronic pain is better enough that my pain doctor and I are decreasing my buprenorphine this month. So good things are happening - it's just been much slower than it may be for others. *No shade to patients who use online clinics. All of us with depressive conditions, PTSD, chronic pain, we're doing the best we can out here, and for some of us, online clinics are literally the only option. Like don't tell patients there's this miraculous new treatment, and then say "Just kidding, it's only for the privileged". I only added the caveat that it's my local doctor because a lot of HCP are rolling their eyes at the online clinics, so was just bumping up the "credibility" of my treatment to perhaps prevent that. Some eyerolls are warranted - the marketing for some of these places gets seriously questionable. It's just that that eyeroll ends up pointed at patients a LOT.

Must rule out serotonin syndrome before administering ketamine or psilocybin, or prescribing SSRIs, Tricyclics, or MAO inhibitors.

that's a lot of different things. Serotonin syndrome is a potentially fatal condition that happens when there's too much serotonin activity. This can, for example, happen with MDMA taken along with MAOIs, and maybe MDMA along with SSRIs. MAOIs in general don't combine well with other meds. SSRIs and mushrooms aren't likely to cause serotonin syndrome (check with a doctor don't take the word of a RU-vid comment!), but effects of mushrooms might be diminished. I didn't look too hard but there is a case report of a patient on Prozac (an SSRI) who was given IV ketamine for pain in hospital for 4 days, and had effects consistent with serotonin syndrome.

Ketamine is very powerful strong and good when used by an anesthesiologist or emergency room doctor! I really enjoy donating to the anesthesiologist foundation,since it was hard for me to go to college because of my cerebral palsy, I really wanted to be an anesthesiologist! I went to the doctor for my yearly physical exam today, everything check out good! I have to have physical therapy because my muscles are tight because of my cerebral palsy! You been hocking loogies? 😂, I’m still waiting on a date for my eye doctor, they are trying to find a CRNA and anesthesiologist at the surgery center is busy! You rock man! It takes a special person and skills to be a CRNA or anesthesiologist, and you’re one of them, I’m gonna donate to the anesthesiologist foundation again new years

I was recently prescribed sublingual ketamine and its quite affordable in comparison to IV from what I've gathered. And although it may not be as bioavailable as the IV method I consider it well worth a try. Talk to your doctor about it.

Yesss. IV ketamine for bioavailability. I get 150mg (10mg bolus) over 50 minutes. My last two surgeries I asked for extra to save me a trip to the ketamine clinic and they were like “sure no problem”

I started a nasal treatment a few weeks ago. I tried to talk to them and explain it just did not feel strong enough. I was made to feel even crazier than I am for such a thought. I got the impression they thought I might be an addict or something. It was honestly devastating to almost feel relief, and then it is gone. This makes so much sense. Thank you so much!

I've been on spavada (nasal ketamine) for over a year. I don't think it works the same every time, based on my nasal mucous or other factors. I think this is the only form my insurance will cover. What can I do to ensure it works more uniformly?

I have a couple of questions. Is it possible to provide IV ketamine through a butterfly or does the patient have to have an IV cannula? I have CPTSD that originated during a nine month hospitalization at Stanford in 1971, which included SEMLS and Gallows traction completely without analgesia or muscle spasm control. I then developed osteomyelitis which led to sepsis, blood poisoning, systemic candidiasis and pneumonia. Yeah I come by it honestly. Fast forward to 2023 and I also have severe chronic pain which is going untreated and medication resistant CPTSD related specific phobias, depression and anxiety. Can someone with this type of profile benefit from ketamine therapy? How can someone find a qualified physician practicing in California’s Capital Corridor? I hope you and your loved ones have a wonderful Holiday season. I love your posts.

He’s correct about ketamine. I’ve been receiving ketamine infusions every two weeks for severe crypts. Fibro. Severe degenerative back pain bulging discs etc. depression and anxiety. Also porphyria. I’m a non responder to antidepressants as well. Once been a runner for 30 years and had to stop do to severe body pain and a chielectomy on my right big toe. It helps a great deal. If you can find the right doctors to work together it’s a very helpful tool to have to have a decent quality of life.

So at 58yo and looking at bypass in both legs. I have a diagnosis of PTSD,bipolar , clinical depression and anxiety, and MS , degenerative spine and sciatica nerve issues. Not looking forward to this.still haven't been able to make it clear that I will be totally on my own after. Not supposed to drive for a week after. I'm quite rural. I often fight and break things when waking up. The max automatic feed for my animals is four days and no one exposed to them. So many issues outside of the meds and surgery. Thankfully I've worked myself sick and tired pulling my house together from the last roommate that trashed it 2hile I was sick. I did have a small heart attack on the table while they placed the stint in the right side of my heart about 6 months ago. If they knew I drove myself home they wouldn't have been happy. I hired a 2oman off the street to sit in the lobby and tell them she was my driver. Don't think I'll be able to pull that one off again with the next surgery. I'm not afraid of this, just afraid that I won't have everything in order when it gets here. It's no different than preparing to deploy and be dropped in some toilet somewhere that you may not come home from. You make your peace ,prepare for the worst and go do what must be done.that state of mind is almost addicting. The calm ,overwhelming calm. You go fight the battle ,knowing for better or worse you will come out the other end for ever changed. Thank you for what you do. it's like anesthesia boot camp.

I have had chronic pain due to Ehlers Danlos for over 20 years. I’m curious about using ketamine to help with the pain - obviously I have structural issues creating the pain… loose joints and bones that fall out of place easily, but I’ve also had moments on psychedelics where the pain completely disappeared… so I wonder about something like ketamine that supports neuroplasticity helping minimize the pain, even if the source is structural. I hear you saying IV gets better results than oral however I have a severe needle phobia… I get vitamin C IVs occasionally to help my collagen production but I am basically on edge the entire time and don’t nearly get them as frequently as I should just because my nervous system freaks out at the sensation of something puncturing me. So I am super wary to do IV ketamine because there’s no more uncomfortable setting I can imagine than being hooked up to an IV… so in my case, would oral be better? Or have you ever done IV ketamine with someone who gets a panic response at needles and had their body’s response change due to the influence of the drug?

I used to be the same way about shots and especially IV’s but the clinic I go to have such skilled nurses that do my IV that I hardly ever feel it! They use a butterfly IV because I am old and have fragile veins and once you go into the ketamine treatment I have no pain or anxiety unless I have a bad trip, which is unusual and they also give me an anti-anxiety and anti-nausea medicine either in my IV or sometimes the anti-anxiety is given in pill form before starting the ketamine infusion. I have u diagnosed hED and/or Sjogrens Syndrome depending on which of my medical professionals you talk to because they both have lots of the same symptoms. I also have ankylosing spondylitis, osteoarthritis, scoliosis, and MDD and anxiety with PTSD nightmares. The infusion that helps both my chronic pain and emotional disorders is the chronic pain infusion which is 4 hours. The depression/emotional infusion of ketamine for 1 hour do not help me. I hope you get some relief because I know the torment you are going through. At least I think I know.

Can ketamine be useful in severe nerve damage pain? A friend fell & had a very severe traumatic hematoma, causing nerve damage due the amount of swelling in the leg & also required multiple blood transfusions due to blood going in to the joint. They had talked about doing surgery on their leg due to compression syndrome?

I'd give anything to be able to try this. I've suffered from depression for too many years. You'd think people like me would be at the front of the line. I've nearly died a few times from this and can't work so I can't afford to have it done. I'd love to work, I'm not lazy, but fibro, CFS and other issues have made it impossible for me. Unfortunately we are so based on profit instead of working towards the well being of all.

I contacted wafermine and they emailed me this evening and said a specialist can prescribe it, and gave me their (wafermines) info to give my Dr to get it worked out. The specialist for me would be a psychiatrist or pain doctor. And I just happened to get a referral to a psychiatrist earlier today. we will see what happens. Email them and see what they say. I just told them how I thought it would help and asked if there is a way to get it since it's still in trials.

Being double boarded in Anesthesiology and Pain Medicine ,it was entertaining to watch and hear your presentation. We prescribed ketamine in multiple forms for the indication of pain. More than one patient came to me severely traumatized by unethical ER doctors who had administered ketamine with no education/consent. All one needs do is look at the ‘spring break’ debacle in Florida over the past decade to see how horrible a ‘good thing’ can become when it is wide open with no education/warning and guidance.

I've gotten ketamine in surgery and recovering from surgery. Some doctors scoff and sat no way will they give me that. But with my cyp450 ultra rapid metabolism of narcotics, this is one that works. However just an IV injection of it only last maybe 10 minutes. Do you have any advice for a patient like me?

I recently broke my femur & the ambulance crew used ketamine. I had never heard of it & they said it’s being used more & more for pain. They were going to do 25 mg, but only got in 10 mg & I got so nauseous that they had to stop. In route to the hospital, they tried to give the remaining dose, but only got in another 5 mg & again it made me super nauseous. Once at hospital, they used dilauded. Is this common, now, to use ketamamine in ambulance patients?

It was a really interesting video to watch! Thanks! I am from the EU, Hungary, and I battle with treatment resistant depression for more than a decade now. My insurance! accepted ketamine therapy, but here we only have the Spravato nasal spray. I am on it 3 weeks now and already got really promising effects from it!:) Anyway my point is I weigh 130 kg or 286 pounds and I am 194 cm or 6,36 feet tall male. So one spray contains 28 mg of esketamine. For me one session consists of three 28 mg nasal sprays to achieve the desired effects. My doctor is a neuropsychiatrist so IV is out of question:) But IM too because only vets and anaesthetists have access to it.. But I am more than happy with this nasal form because it WORKS! :) Have a nice day Sir!

I had this subcut for pain & i have given it by IV infusion. The last time i had it after major surgery it didnt make me feel to good after i was given it for 3 days but then had to go back to suegery 3 days later as had a huge bleed into my tummy the second time i was given it for 3 days IV it made me feel very freaked out & emotions were out of control I was in ICU it may be the fact i had no sleep so the ketamin & no sleep made me feel weird. But it does do wonders for pain . Im in the UK tho

I recently dislocated my hip twice within 10 days and at the first visit to the ER they used a mixture of propofol and ketamine to reduce my hip. I did feel outside my body as I was coming out of sedation and felt like I was floating on a cloud like a float in the Macy’s Thanksgiving Day parade and was telling everyone in the room😂 As it continued to wear off I began to feel my body touching the bed and it actually hurt. I was talking up a storm at this point but completely not in control of what I was saying and said, “gravity sucks” as my body grew heavy as the drug wore off. Is this normal and/or is this a k-hole? I have Anesthesia Dolorosa with the not surprising depression on the side. I am in my 4th year and 2nd year of nasal Ketamine. I had a bad reactive with the nasal, over amount and it was not good and gave me panic and pain I think was a bit higher due to panic. AD really is hard to get pain control. I am better suicidal with time but this is not my dream life.

I have had ketamine in surgery a coule times. Had such a scary experience with FENTENAYL, UNABLE TO USE IT FOR SURGERY. Because of complex regional pain syndrome, It also did help with the pain syndrome, as well as the anxiety and stress. Last surgery ketamine had NO side affects FROM KETAMINE UNLIKE MANY OTHER DRUGS. @@MedicalSecrets IT IS DEFINITELY BETTER. I STILL HAVE HORRIBLE MONSTER EPISODES OF SEVERE CRAMPING AT NIGHT OR DAY THAT DROPS ME TO MY KNEES . . . I HAVE TO STRETCH OUT THE HORRIFIC PAIN. IT IS HORRIBLE WHEN IT FLAIRS. NO LONGER ABLE TO DO WHAT I USED TO DO. VERY ACUTELY AWARE OF HOW MUCH I CAN DO. I TRY TO PACE MYSELF. IT IS DIFFICULT. MANY THINGS TO DO!! A JUGGLING PROCESS. NERVE COMPRESSION OF RT BIG TOE. FRACTURED AND COMPRESSING NERVE.

Hello, how are you? I have done Ketamine IV for depression and addiction to heroin. I have been on suboxone for 8 years and am lowering my dose slowly. I feel that I will one day be off of it. I have been dependent on stimulants since I was prescribed them at the age of 13. I have tried and tried to live without them but it feels impossible. What do you suggest?

There is no access to Ketamine through IV where I live in Canada. There is a clinic that uses tablets. I have Fibromyalgia and nothing has helped the pain. It’s constant and it’s everywhere. I plan to try it but it is pretty expensive. Should I not bother? I have spent a lot of money over the past 10 years trying different things.

I am on over 18 medications, 6 or which are for severe chronic pain, depression and anxiety, one of which is an opioid and is so complicated to get and keep. I wonder if this one medication could replace those medications?! Is it easier to get than opioids? I can fully function on my medications, does Ketamine cause drowsiness, fatigue, does it make you feel loopy?! I am so intrigued!!

I would do anything to be able to try ketamine but my doctors (Kaiser) say they don't use it and refuse to talk about Ketamine or LDN saying they don't prescribe it or use it. Its damn near impossible to get help for pain if a kaier patient.

I'm so sorry to hear about that. It sounds like I need to do a whole video on ketamine for chronic pain. There is evidence for use in complex regional pain syndrome, which I recently described in a video. There is also evidence for neuropathic pain conditions, including phantom limb pain.

I've had very profound and positive results with oral Ketamine. I live in Oregon. At the moment, Intravenous Ketamine therapy at a clinic is very cost prohibitive for average folks. Oral Ketamine may not be as effective as IV Ketamine but it comes in at abut 1/10th the cost.

Had it for postoperative pain because typical postoperative analgesia didn't relieve my pain. I was so amazed when my pain istantaneously vanished. Then I was tripping hard. The nurse station moved away from me but I didn't feel freaked out by it. Unfortunately, it wore off fast.

I remember my ravers days we dried the ketimine on a pyrex pan with a blow dryer. We would snort the crystals. See us ravers were on to something! Mdma is being used to ptsd. Ketimine is being used for depression all these taboo drugs we used are no longer taboo.
